PROBABLE CAUSE: At approximately 12 :30 p. m. on Wednesday, December 31, 1997, the northbound 11 :22 #6 train in route from Brooklyn Bridge Station, Manhattan to Pelham Bay Park Station, Bronx experienced an emergency brake application while departing Buhre Avenue Station, Bronx, New York. The train operator reported the occurrence to Command Center and that the lead car had only moved approximately 10 to 20 feet out of the station when the brake application occurred. The train operator was instructed by the Command Center to secure the train and investigate . The train crew's initial investigation revealed that an emergency brake valve had been activated at the rear of the 10th car (# 1773) . The crew reset the brake valve and attempted to reset the train's brakes but they failed to recharge. The crew again inspected the train and while doing so, heard air blowing from between the 8th and 9th cars. Their investigation of this condition revealed a body under the 9th car #1665 . The crew immediately reported their findings to the Command Center and requested additional assistance. Police and Fire Department Emergency Medical personnel responded and the person was pronounced dead at the scene. Passengers on board the train were discharged to the station platform without incident. The body was removed from the roadbed and at approximately 2 :45 p.m., normal service resumed. The crew was taken to the local medical facility where they tested negative for drugs and/or alcohol . Post accident inspection of the equipment involved revealed that the safety chains between the 8th and 9th cars had been removed from their applied position and were hanging . Investigating New York City Police personnel determined this fatality to be accidental and concluded the deceased person was apparently trying to exit the train from between cars when he fell to the roadbed. There were no witnesses to this accident identified. The Public Transportation Safety Board staff finds that the probable cause of this accident was the unsafe actions of the passenger attempting to exit the train from between cars while the train was in motion and makes no recommendations concerning this accident. 11 .     Brooklyn/Queens Atlantic Avenue in Queens at Rockaway Boulevard SAFE STREETS NYC 222 Atlantic Avenue Safety Improvements (Brooklyn and Queens) Description Atlantic Avenue is a primary local arterial roadway that connects the Brooklyn waterfront and the Brooklyn- Queens Expressway on the west with Conduit Avenue and the Van Wyck Expressway in Queens to the east. Atlantic Avenue is one of the major truck routes in Brooklyn as it traverses the entire borough along an east-west direction. For the most part, the Atlantic Avenue corridor ranges between four and six moving lanes with two parking lanes. Several portions of the roadway also have a raised center median separating the roadways. In Brooklyn, land use along the Atlantic Avenue corridor is heavy with a bustling mix of residential and commercial land uses. In areas with a heavy concentration of land uses, such as the area in the vicinity of the Atlantic Avenue Long Island Rail Road Station (LIRR), there is a high convergence of pedestrians and vehicles. These contribute to high pedestrian volumes at several intersections and conflicts between motorists and pedestrians. These same conflicts are also apparent at locations where schools, high-density residential developments and commercial land uses are prevalent. In Queens, adjacent land uses are primarily lower density residential developments with some industrial and commercial development along the corridor. There are also several public facilities such as schools and religious institutions along Atlantic Avenue, especially between Woodhaven and Rockaway Boulevards. Traffic volumes in both Queens and Brooklyn are high, with both a high volume of passenger vehicles and trucks as Atlantic Avenue is a major truck route for Brooklyn.
