Locum Consultant in Rheumatology Medicine Fixed Term for 2 Months

Applications are invited to join a progressive Rheumatology team as a Locum Consultant in Rheumatology based at Raigmore Hospital, Inverness with outreach services provided at Ross Memorial Hospital, Dingwall. This is a replacement post based following emigration of current post holder and is offered on the basis of 10 Programmed Activities per week. You will join an MDT team with two other full time consultants, 3 specialist nurses.

Applicants must be on the Specialist Register with a Licence to Practice.

The Consultant will assist in providing a modern service to run to national quality criteria for the population of the Highlands. This post is based at Raigmore Hospital, Inverness, but applicants are required to undertake work at other hospitals within NHS Highland. There will be no on-call duties but with mutual agreement the job plan may be modified to include this if the successful applicant has combined rheumatology/GIM accreditation and wishes to be included in the GIM on-call rota. Raigmore Hospital is a modern well-equipped district general hospital with 400 beds, which serves a catchment area of almost 15,000 square miles covering the entire Scottish Highlands, Western Isles, and surrounding area. Rheumatology inpatients are accommodated in a 10 bedded unit at Ross Memorial Hospital which has medical cover from the local GP practice out of hours. The service is supported by a multidisciplinary team with dedicated physiotherapy, occupational therapy and specialist nursing sessions. There is a hydrotherapy pool on site. The unit contributes to multi-centre clinical research and audit.

Section 3 – Job Information

This post will join two other consultant rheumatologists providing services to the population of the Highlands but based at Raigmore Hospital. Historically there has been a specialist service directed to rheumatology patients since prior to 1777 with the first description polymyalgia rheumatic being attributed to Dr William Bruce at Strathpeffer Spa in 1888. The rheumatology beds were transferred to the Ross Memorial Hospital in the 1980s where the MDT team are based today. Outpatient services are currently focussed at Raigmore Hospital, Inverness but with clinics also running at Ross Memorial Hospital, Dingwall.

Main Duties

You will be part of a team of 3 rheumatology consultants providing an extensive range of multidisciplinary outpatient clinics, day case facilities and supervising a small dedicated rheumatology inpatient unit based at Ross Memorial Hospital, Dingwall IV15 9QT . The Department of Rheumatology provides services to the Highlands, parts of and parts of the population of Moray, with a widely dispersed population of approximately 260,000. There is no GIM component to this job though the job plan can be modified to include this if the successful applicant has combined rheumatology/GIM accreditation and wishes to be included in the GIM on-call rota.

1. You will be expected to work with local managers and professional colleagues in the efficient running of the service, and will share with consultant colleagues in the medical contribution to management. Subject to provisions of the Terms and Conditions of Service, you are expected to observe NHS Highland’s agreed policies and procedures, drawn up in the consultation with the profession, on clinical matters, and to follow the standing orders and financial instruction. In particular, where you formally manage employees of NHS Highland, you will be expected to follow the local and national employment and personnel policies and procedures. You will be expected to make sure that there are adequate arrangements for hospital staff, involved in the care of your patients, to be able to contact you when necessary.

2. You are required to comply with the Board’s Health & Safety Policies.

3. You have agreed that you will be responsible for the training and supervision of the junior medical staff who work with you, and you will be expected to devote time to this activity on a regular basis. In addition you will be expected to ensure that junior staff have access to advice and counselling.

4. The main duties and responsibilities of your post include: a. providing a Board-wide Rheumatology service b. organising and integrating with specialist visiting services

c. supervision and training of junior medical staff d. undergraduate and post-graduate education e. taking part in in-service training for medical, nursing and AHP staff f. to work with many other associated agencies including Social Work, Employment Agency, Voluntary Services, schools, colleges and private residential units.

Facilities

Outpatient facilities currently exist at Raigmore Hospital and Ross Memorial Hospital at Dingwall which are 15 miles apart. NHS Highland recognises the importance of quality of care for complex rheumatology patients and builds in two 15 minute slots for musculoskeletal ultrasound but which could also be used for other procedures such as joint injections or videocapillaroscopy. Specialist return clinics such as CT clinics would be allocated 20 minute return appointments. We currently run Early Arthritis clinics and CT clinics. The post also offers the opportunity for the successful candidate to contribute to the ongoing development of the department. If the candidate has a specific area of interest (e.g. disease area, research) we would be keen to support and develop this within the service. Videoconferencing facilities are available between various hospital sites where this is thought to be practical.

We have 3 full time rheumatology specialist nurses who take their share of return rheumatoid and psoriatic arthritis patients, coordinate the biologic service, run a telephone helpline and are prescribers for biologic and targeted synthetic DMARDs. The nurses in collaboration with the physiotherapists run the axial spondyloarthritis clinic and are developing procedures for transition of young adults from the paediatric service. Paediatric rheumatology is provided by local paediatricians in collaboration with visiting tertiary centre specialists.

Rheumatology inpatients are in a 10 bedded unit at Ross Memorial Hospital which has medical cover from the local GP practice out of hours and a ward doctor 3 sessions per week. The service is supported by a multidisciplinary team with dedicated physiotherapy, occupational therapy and specialist nursing sessions. There is a hydrotherapy pool on the Dingwall site. In patient facilities are also available at Raigmore Hospital for more complex patients requiring more intensive medical input - these beds are supported by rotating general medical junior staff and acute medical consultants.

Day case and infusion facilities are available at Raigmore Hospital, Inverness and Ross Memorial Hospital, Dingwall with satellite facilities available at General Hospital, Wick and , Fort William. The Infusion Unit at Raigmore hospital has 5 chairs and runs 5 days per week. Protocols exist for all rheumatological infusions including iloprost and pulse cyclophosphamide. The unit is shared with other medical specialities.

DXA and fracture liaison services are based at Ross Memorial Hospital which houses a Lunar prodigy scanner with radiographer team and administration staff. DXA scans are reported by an experienced GP.

We have two dedicated ultrasound machines at Raigmore Hospital (GE Logic E9 and Siemens Acuson X300) and a GE Logic E9 based at Ross Memorial Hospital. Other equipment includes a polarising light microscope and a Capiscope videocapillaroscope.

We are currently participating in a number of academic and commercial studies. Research facilities are available at the RD&I department at the Centre for Health Sciences. This on site facility provides both clinic rooms and an extensive team of research nurses, many of who are participating in rheumatology clinical trials and there is scope to facilitate any research interests of the candidate.

Staffing

 3 WTE Consultant Rheumatologists including this post, 0.2 WTE Hospital Practitioner (Osteoporosis service and DXA scan reporting)  0.3 WTE ward doctor (Ross Memorial inpatients)  3 WTE Rheumatology Specialist Nurses.  Biologic Infusion Unit based at Raigmore Hospital.  Dedicated rheumatology ward nurses at Ross Memorial Hospital who provide outpatient cover for clinics run there.  0.83 WTE Rheumatology Specialist Physiotherapist  2 x 0.5 WTE Rheumatology Specialist Physiotherapists  0.8 WTE Rheumatology Occupational Therapist  0.6 WTE Rheumatology Occupational Therapist

We also have access to podiatry, orthotics, and have close working relationships with pharmacy.

Teaching

Members of the department play an important role in the undergraduate programme of the Department of Medicine at the University of Aberdeen. There are also students rotating from Dundee and St Andrews.

The new consultant will share in the department’s formal teaching programme which includes weekly seminars and attachments to 4th year medical students, provision of ward teaching to 5th year medical students, contribution to weekly seminars and monthly audit sessions and MRCP teaching for junior medical staff.

Clinical investigation facilities at Raigmore include: a Radiology Department with 2 MRI and 2 CT scanners, an interventional radiology service and a full range of other diagnostic capabilities including CT coronary angiography. PET scanning is available at Aberdeen Royal Infirmary. There is a comprehensive Cardio- respiratory investigation department, Nuclear Medicine Service and Medical Physics Department. There are excellent in-house laboratory services and a regional Blood Transfusion Service. The comprehensive regional IT system includes results reporting, discharge documentation and clinical information support. There is a Medicentre which is a multi agency project supported by HIE, NHS Highland, NES, University of Stirling and the Centre for Rural Health. It includes a comprehensive library.

The Medical Unit

The Medical Unit at Raigmore Hospital undertakes about 8,000 admissions, mostly acute emergencies, 3,000 outpatient attendances and 1,000 day cases annually. In addition to the new post in Rheumatology there are 32 consultants in charge of 227 beds as follows:

Section 4 – Job Plan

Job Planning/Programmed Activities

The draft job plan will be dependent on the successful candidate’s subspecialty training and interests.

The appointment will be on a whole-time basis under the terms of the New Consultant Contract. The job plan contains 10 programmed activities, including two allocated to supporting professional activities. There may be an opportunity to increase the total commitment by a maximum of 2 EPAs per week by mutual agreement.

It is a requirement for all consultants to have a job plan. Agreed job plans will be prospective and will set out:

All job plans will follow the model format contained at Appendix 4 of the New Consultant Contract. The job plan will be subject to review at least annually, or more often, if changes to staffing resources, or working practices, or the consultant’s circumstances require it. On taking up post a Job Plan will be agreed between the person appointed and the Clinical Director / Associate Medical Director.

The indicative timetable identifies those fixed commitments which are dependent on other hospital resources, both physical and human.

Section 5 – Person Specification

Essential Criteria - these are attributes without which a candidate would not be able to undertake the full remit of the role. Applicants who do not clearly demonstrate in their application that they possess the essential requirements will normally be eliminated at the short listing stage.
