STEP 4 To receive incentives, you must have a copy of this agreement on file with PGA Golf Enterprises, Inc. d/b/a Golf Retirement Plus. The form must be completed and signed by you and your employer (if applicable) on an annual basis. PGA Golf Enterprises, Inc. reserves the right to verify the signature and/or the authorization of employers at PGA Golf Enterprises, Inc.’s sole discretion, which may include but not necessarily be limited to, submitting copies of this form to your employer for verification.

There are more than 45 Golf Retirement PlusTM (GRP) Sponsor Programs available to PGA Professionals. The majority of them offer incentives that are truly incremental to other financial benefits or discounts available to the facility. The programs listed below allow the facility to choose whether to apply discounts to invoices or authorize the contribution of incentives to their PGA Professionals’ GRP accounts.

• Ashworth – GRP incentives are in lieu of Ashworth’s off-invoice discounts.

• Callaway Golf – GRP incentives are in lieu of Callaway’s off-invoice discount.

• Callaway Golf Apparel – A component of this program offers a choice between the entire GRP incentive going to the PGA Professional or the facility.

– A component of this program offer a choice between the entire GRP incentive going to the PGA Professional or the incentive being split between the PGA Professional and an off-invoice discount.

• FootJoy – Some components of this program offer a choice between a GRP incentive or a credit to the facility’s FootJoy invoice.

• Nike Golf – Offers an option to select a competitive sales program, allowing a percentage of the net sales to be taken as GRP incentives or as an off-invoice discount.

• TaylorMade Golf Company – Discounts earned for meeting the Inside Sales Policy conditions can be taken as a GRP incentive, account credit or no-charge merchandise. Regardless of this election, an additional 1% is available as a GRP incentive only.

• Titleist – Most of the GRP incentives are in lieu of Titleist Sales Programs’ incentives to the facility.

• Town Talk Headwear – The Twelve Dozen Basics Cap Program offers a choice between a GRP incentive or a lower price point.
