Instructions to Apply for District Fee Waiver – Form 6191

If you received a letter from CUSD 200 by July 25th of this school year stating that your student is identified by the state as DIRECT CERTIFICATION status, you do not need to complete this waiver. Each eligible student will receive a separate letter. Direct Certified students are eligible for book and fee waivers. For All others: STEP 1. List the names of all students, their grades, school and I.D. number. List all parent(s)/Guardian(s) and legal dependents living in the household. CATEGORICALLY ELIGIBLE – If the student is homeless, migrant, runaway, or has been placed by the Department of Children and Family Services with a foster parent or placed in another type of child care facility, please state this information on the application form in lieu of providing the requested financial information. If the student has been placed by the Department of Children and Family Services with a foster parent or in another child care facility, documentation must be attached to the application verifying the student’s status.

If your household receives SNAP or TANF, and you did NOT receive a letter from CUSD 200 showing Direct Certification by July 25th of each school year, list all household members and submit a copy of each student’s current SNAP or TANF certification letter from the state. Supplying false information to obtain a fee waiver is a Class 4 felony (720 ILCS 5/17-6).

Boys’ tryouts are Mon – Fri, Feb. 29th – March 4th, 2016. The boys’ tennis program has two teams. We welcome freshmen to try out. Previous experience is not required. What are needed are a strong work ethic and a positive attitude. Our philosophy is to keep as many players in the program as possible. ([email protected]) The WNHS Summer Tennis Camp runs from June 15 to July 17 and offers a total of 7 co-ed classes that range from grades 1-12 and cover all ability levels.
