Endotracheal intubation under medical emergency conditions based on direct laryngoscopy is not effective enough; this effect is additionally reduced in the case of the difficult airway resulting from reduced visibility of the en- trance to the glottis due to tongue or epiglottis oedema, trauma, etc. The aim of the study was to compare the intubation time and its effectiveness using two different stylets for difficult airway intubation. matERIAL AND METHODS: The study involved 37 nurses who participated in training on advanced life support procedures. The experiment was designed as a randomized, cross-over simulation study. During the training, participants were instructed to perform endotracheal intubation using the tested intubation methods and had 20 minutes of practical training during which they were able to intubate with the tested stylets under normal airway. In the study, participants performed endotracheal intubation using a laryngoscope with a Macintosh blade and a difficult airway Bougie stylet (ONTEX, Chennai, India), or the Flexible Tip Bougie (MDSS GmbH, Hannover, Germany), which was designed to allow to guide the distal end of the anteriorly and posteriorly to facilitate entry into the larynx. RESUlts: The effectiveness of the first intubation attempt using a standard Bougie stylet was 37.8%, and that of the new Bougie stylet was 51.4% (p = 0.037). The mean intubation time was 55 s (IQR; 34–65) vs. 37 s (IQR; 25–41) (respectively, p = 0.021). The median ease of intubation was 7 (IQR; 5–9) points for a standard Bougie stylet and 5 (2.5–7) 2 points for a new Bougie stylet (p = 0.018). CONCLUSIONS: In a simulation study, the use of Flextip Bougie by nurses compared to a standard Bougie stylet was associated with higher efficacy and shorter intubation times in difficult airway. Utiliza-tion of alternative options and resources for low acuity, non-emergent patients will help increase the avail-ability of EMS to higher acuity patients, decrease offload delays at the Emergency Room (ER), and decrease re-admittance rates. Problem Fire Service based EMS can be considered the gatekeepers to the healthcare system. The number of 911 habitual users has increased, with these users accessing the system for a plethora of reasons. These reasons include having limited access to primary care, assisted care, or follow up care; a lack of awareness of how to access resources other than 911 for low acuity issues; or suffering from mental health issues or substance abuse problems that interfere in appropriate determination of the 911 system. The increase in the frequency of habitual users places a strain on EMS operationally as well as financially. This affects EMS resources, including the fire department, Emergency Medical Technicians (EMTs), paramedics, law enforcement, dispatch, ERs, hospitals, Psychiatric Emergency Response Team (PERT) clinicians, and neighboring agency resources. Previously identified potential solutions involve delivering patient care follow-up, establishing patient care plans and education, alignment of resources for low acuity needs, and local partnerships with law enforce- ment and hospitals.

    DOUGLAS COUNTY COLORADO OFFICE OF THE COUNTY ADMINISTRATOR RISK MANAGEMENT AUTOMATED EXTERNAL DEFIBRILLATORS (AED) Purpose: The practices and procedures described in this document comprise the program through which Douglas County Colorado will administer its Automated External Defibrillator Program. This program will be reviewed annually, and revised whenever new, or modified tasks, or procedures are implemented. Relevant Statute: Persons rendering emergency assistance through the use of automated external defibrillators – limited immunity. Colorado Revised Statute 13-21-108.1 13-21-108.1 (4)(a) Any person or entity whose primary duties do not include the provision of health care and who, in good faith and without compensation, renders emergency care or treatment by the use of an AED shall not be liable for any civil damages for acts or omissions made in good faith as a result of such care or treatment or as a result of any act or failure to act in providing or arranging further medical treatment, unless the acts or omissions were grossly negligent or willful and wanton. (b) The immunity provided in paragraph (a) of this subsection (4) extends to the licensed physician who is involved with AED site placement, the person or entity who provides the CPR and AED site placement, and the person or entity responsible for the site where the AED is located. Definitions: A. Automated External Defibrillator “AED” means an emergency medical device that can detect and recognize certain heart arrhythmias and can determine, without intervention by the operator, whether defibrillation should be performed. If defibrillation is appropriate, the AED automatically charges and requests the operator to deliver an electrical impulse to the patient’s heart.
