We report a 66-year-old woman with a mixed corticomedullary tumor of the left adrenal gland. The patient was found to harbor an adrenal incidentaloma while investigated for a spigelian hernia. Due to the atypical radiological features and the relatively large size of the adrenal lesion she underwent a left adrenalectomy following endocrine testing to exclude a functional lesion. Subclinical Cushing’s syndrome was suggested by the failure to obtain adequate cortisol suppression (less than 1.8 μg/dL) following dexamethasone administration pre-operatively; cortisol suppression was restored post- operatively following the excision of the tumor. Histology was consistent with a corticomedullary mixed adenoma, a lesion for which, there is paucity of published data regarding its natural history and long term outcome. The finding of this case highlights the importance of this extremely rare entity which should be included in the long list of causes of adrenal incidentaloma since cases with intra-operative complications have been described. The previously reported reappearance of this tumor in the contralateral adrenal gland emphasizes the need for prolonged follow-up.

REVIEW ARTICLE History of medicine on the border between philosophy and science Mirela Radu 1 Abstract: Physicians have represented a long time the main transmitters of knowledge as they were real scholars. If Renaissance promoted the study of the human body anatomy and physiology, the next step made by practitioners of medicine was to spread the enlightenment. That meant the shift of the very purpose of their profession: from passive opposition to ailments towards an active involvement into the lives of the impoverished. In order to change the odds in the battle against diseases, physicians had the great burden to enlarge the cultural horizons of those whose health was in their hands. Therefore, one way of imparting knowledge was by publishing and spreading their attainments to the general public in a comprehensible way. Once people gained awareness of the dangers entailed by bad hygiene, the physicians’ role in society switched towards more cultural realms. At the beginning of the 20th century health care professionals achieved the next step in the becoming of medicine: setting up a new science to link humanities with pure science. In Romania, the main promoters of this new border science were Victor Gomoiu and Valeriu Bologa and they co-opted other intellectuals. Keywords: philosophy, science, history of medicine, alchemy, folklore The new involves acknowledging the past, gathering various ethno- transforming it and bypassing mistakes. Drawing on the evidence from everyday medical practice and experiences of women, users of Serbian maternity care services, this paper will tend to point to some of the important conflicts and inconsistencies that are being reproduced in Serbian health care system that is going through transition. Analysis will mainly focus on the gap between the creation of new policies and clinical guidelines and their actual implementation in practice, as well as on the unsynchronized transformation of different actors involved in medical practice that leads to tensions between sources of knowledge/models of practice in medical care.
