Brannon 5 pounds, keep searching for an easy, guaranteed weight- reducing diet. Nutrition and Food Science Promoters of such diets are delighted to aid in this consumer quest, so the number of fad diets continues to increase along with the pro- moters' profits. Fad diets tend to be nutritionally unbalanced, to focus attention on only one type of food or nutrient, to appeal to the consumer with faulty logic, to neglect the issue of weight maintenance, or to make excessive claims for weight loss. The body stores excess energy, measured in kilocalories, as fat. One pound of fat represents 3,600 kilocalories of energy. To lose one pound of fat, you must expend 3,600 kilocalories more than the number in the food you eat. It is not unusual to see claims by a fad diet that you can lose 10 pounds in one week. Take a closer look at this excessive claim: Ten pounds of fat means 36,000 kilocalories. To lose that by dieting in one week, you would have to eat at least 5,000 kilocalories per day less than you normally would.

Part One of this thesis is a systematic review of research into protective factors against eating disorders and disordered eating in proximal social systems. A range of potential protective factors in families, schools, peer groups and neighbourhoods are identified. Many of these factors may be non-specific to eating difficulties, promoting a range of positive outcomes, while others may be more specific to disordered eating. Methodological issues in the literature which limit the ability to draw firm conclusions are discussed. Part Two presents empirical research into the impact of intermittent fasting (IF) diets on eating psychopathology, binge eating, food craving and mood. Contrary to expectation, starting a 5:2 IF diet did not result in increases in disordered eating or binge eating in healthy adult dieters, and in fact appeared to result in improvements in all outcomes. Higher scores on measures of risk factors for eating disorders at baseline were associated with greater reductions in disordered and binge-eating over the 28 day IF period.

Commentary Maintenance of weight loss is often unsuccessful because of metabolic adaptations that conserve energy. Studies in rodents suggest that a reduction in leptin level during weight loss signals to the brain to increase feeding and decrease energy expenditure. In this issue of the JCI, Rosenbaum et al. examined this concept in obese patients who lost weight and were maintained at 10% below their initial weight (see the related article beginning on page 2583). Brain activity responses to visual food stimuli were visualized using functional MRI. Leptin levels fell during weight loss and increased brain activity in areas involved in emotional, cognitive, and sensory control of food intake. Restoration of leptin levels maintained weight loss and reversed the changes in brain activity. Thus, leptin is a critical factor linking reduced energy stores to eating behavior. Potentially, leptin therapy could sustain weight loss by overriding the tendency toward energy conservation.
