Department Office of Chronic Disease Prevention Health actions to prevent obesity, diabetes, and heart disease coordinator

The Baltimore City Health Department (BCHD) is seeking an obesity, diabetes, and heart disease prevention coordinator to work on a four year CDC chronic disease prevention grant. The program is seeking a coordinator with experience working on health systems interventions to improve the quality of delivery to populations with hypertension and prediabetes. This position will work with Health Department, other city agencies, community partners, FQHCs, Hospitals, and funders. This program is part of the Office of Chronic Disease Prevention.

Responsibilities include:

 Implement systems to facilitate identification of patients with undiagnosed hypertension and people with prediabetes  Work with Federally Qualified Health Centers (FQHCs), private practices, and hospitals to help identify areas for change  Collaborate with Hospitals and (FQHCs) to implementation of a systems based approach to improve hypertension control  Build sustainable relationships that result in increased access to health care and community‐ based health education  Increase engagement of non‐physician team members (i.e. nurses, pharmacists, and nutritionists, physical therapists, and patient navigators/community health workers) in hypertension management in community health care systems  Increase engagement of community pharmacists in the provision of medication‐/self‐ management for adults with high blood pressure  Write grant progress reports and track participation data  Coordinate with government agencies, health care providers, non‐profit organizations  Participate in the Cardiovascular Disease Taskforce

Qualifications

Requirements ‐ A bachelor's degree from an accredited college or university and three years of experience in planning, supervision, evaluation, or coordinator of a program

Equivalencies ‐ Equivalent combination of education and experience.

Successful candidate should have an interest in working on lead with communities in Baltimore City:  Excellent written and verbal communication skills.  Familiarity with the challenges and barriers to health care access for vulnerable populations including the social determinants of health  Experience and interest in chronic disease prevention  Need to work effectively with members of city government, for‐profit and non‐profit organizations, community leaders, and members of the public.
