    Landing at Narita Prior to landing, the flight attendants on your flight will hand out two forms to fill out. Fill in your immigration and customs forms (front and back) before landing in Tokyo. (1) Immigration Form: One is the Embarkation/Disembarkation Card, a rectangular document with a detachment in the middle. This form is for the immigration authorities. (2) Customs Form: Another is the Declaration Form for Japanese customs authorities that has the words “Customs Declaration” at the top. NOTE: Some travelers on certain flights, or arriving from certain countries, must also fill out a yellow Quarantine Form from the Ministry of Health and submit it upon arrival.     October 15, 2019 EAST JAPAN RAILWAY COMPANY Present condition and prospect of the Hokuriku Shinkansen, the Chuo Main Line (Between Takao and Otsuki) Due to the influence of typhoon No.19, suspension of operation are still continuing in some sections of the Hokuriku Shinkansen and the Chuo Main Line. We deeply apologize for the inconvenience caused to our passengers. We would like to inform you about the present condition and prospects as of 4p.m., October 15(Tuesday). 【Hokuriku Shinkansen】 〇Due to the flooded railtracks between Nagano and Iiyama, the operation of Hokuriku Shinkansen is being suspended between Nagano and Joetsu-myoko. At the moment, the trains between Tokyo and Nagano, Joetsu-myoko to Kanazawa are operating according to a special diagram with trains acting as shuttles between in those sections. 〇Since the flood alert was released in that region, the maintenance for the flooded railtracks has started since October 15(Tuesday) between Nagano and Iiyama. At the moment it is confirmed that there is serious damage at the power supply apparatus related to signals, and it is expected to take about 1 or 2 weeks for the apparatus to recover. However, it could take more time if there is any other defects of equipment found such as defects related to traffic signal controllers. The train operation between Nagano and Joetsu-myoko will be restarted as soon as all the recovery of those equipment is finished. 〇 After the restart of operation between Nagano and Joetsu-myoko, the operation of Hokuriku shinkansen will be started in all the sections (between Tokyo and Kanazawa).
