56) Physical Mode ======Step 1:- Share holder should have an ACTIVE trading account holder with Integrated. Step 2 : BSE Segment Activation Step 3 :-If the trading code is not active, then collect the dormant activation request signed by the respective client . 1) Original Share Certificate 2) Shares Transfer Form duly signed by all the share holders in the same order as mentioned in the physical certificate.(Available in the Offer document) 3) Bid Cum Application Form signed by all the share holders in the same order as mentioned in the physical certificate. 4) Self Attested copy of the Pan Card of all the share holders. 5) Incase of change in address, collect self attested copy of Address Proof (Voter Id/AADHAAR Card/passport) 6) Transaction Registration Slip(TRS) from Exchange (will be provided by us after placing the order in exchange system)

Branches are requested to collect the above mentioned documents and send the scanned copy to [email protected] with the following information for scrutinisation.
