I am the parent/guardian of the child identified above and I certify that the information I have provided – AIR LIBAN is true and correct. 1. I hereby request for the Unaccompanied Minor service and give permission for the child to travel alone. 2. I confirm that the child has all the valid travel documents including passport, visa, health certificate and other necessary documents required by applicable laws for all countries on the entire journey. 3. I confirm that the parent/guardian dropping off the child at departure or meeting and dropping off the child at transit airport (if applicable) will remain at the airport until the flight has departed. I also confirm that the parent/guardian meeting the child will be at the airport of final destination for the scheduled arrival time of the flight and will present appropriate identification to MIDDLE EAST AIRLINES – AIR LIBAN to confirm their identity. 4. Should the child not be met as stated above, I authorize MIDDLE EAST AIRLINES – AIR LIBAN to take whatever action they consider necessary to ensure the child's safe custody, including a return of the child to the airport of departure. I agree to indemnify and reimburse the carrier for the necessary reasonable costs and expenses incurred by them in taking such action. I shall hold MIDDLE EAST AIRLINES – AIR LIBAN, their personnel and agents, harmless against any damages sustained and costs incurred by reason of or in connection with the child's journey. 5. I acknowledge that MIDDLE EAST AIRLINES – AIR LIBAN flights provide inflight entertainment to which the child would have access to and that MIDDLE EAST AIRLINES – AIR LIBAN accepts no responsibility for the content viewed by the child. 6. I have instructed the child to comply with all instructions given by a crew member and confirm that the child has understood this. I have read and understood all the above numbered points.
