REGISTRARS AND MEDICAL STUDENTS :

We are involved in the training of GPs. Each year we have one or two registrars who are qualified doctors that spend a period of 12 months for further training and experience in general practice. The Practice is also involved with the training of Medical Students, Practice Nurses and Other staff. The reception staff will inform you when a student is present with a particular doctor or nurse. You can choose whether or not you wish a medical or nursing student to be present during a consultation.

2 ROUTINE APPOINTMENTS :

You will normally require an appointment to see your doctor or practice nurse. You will be registered with a named doctor; however you are free to choose any (available) doctor you wish for your appointment. To book an appointment, please contact the receptionist at your registered site. Routine appointments are booked at 10 minutes intervals. If you feel that your problem(s) will take longer, please ask the receptionist to book a double appointment

URGENT AND EMERGENCY APPOINTMENTS Telephone the surgery after 08:00 am to be seen for an urgent medical problem. You will be offered an urgent appointment with the doctor who is available the same morning. Patients may be asked to re-book for a normal appointment for non-urgent matters. Urgent or emergency appointments are usually allocated only a 5 minute slot and should be restricted to the one urgent problem. These appointments can often result in long waiting times as they are normally added to the end of a normal surgery. In order to minimise waiting times we may ask you to see an alternative or junior doctor.

TELEPHONE ADVICE If you wish advice from the Practice Nurse or Doctor, Please inform the receptionist who will take the necessary details to arrange this with the Practice Nurse or Doctor. You will be asked for a contact telephone number. Telephone Consultations with a GP are available on a daily basis in the morning and afternoon.

APPOINTMENTS & PRESCRIPTION REQUESTS ONLINE

EMIS Access is a convenient way to book appointments with a GP over the Internet. It is quick and convenient to use, and can be particularly useful during those times when the Practice is closed or telephone lines are busy. Easy to follow screens and prompts ensure it’s simple for you to view, book or cancel appointments online – whatever time of day or night. Please ask at Reception for further details .

HOME VISITS :

We offer home visits to those who are house bound patients or the seriously ill who are unable to travel to the surgery. Please make requests for home visits early in the day, before 10:00 am on the day required. Doctors will attend home visits after the morning surgery. Please give the receptionist necessary details in order that the doctor may assess the urgency of the visit. Please leave any helpful directions for the doctor to get to your home.

REPEAT PRESCRIPTIONS :

Requests for repeat prescriptions may be arranged by leaving the repeat prescription request slips at the surgery. Requests can be made by telephone between 8:00 am – 6:00 pm . Written requests can be returned by post if accompanied with a stamped addressed envelope. Local pharmacies operate a free collection and delivery service if required. Repeat prescriptions are monitored by the doctors, who may request that you periodically attend for a medication review. Two (2) working days (48 hours) notice is required to process requests for repeat prescriptions.

Electronic Prescriptions – You can now have your prescriptions sent electronically to a Pharmacy of your choice who has the appropriate software installed. Please advise a member of staff or your GP if you wish to take up this option. 3

On one Tuesday afternoon each month the Practice will close at 13:00 pm for essential training and development for the Practice Staff. The precise date(s) will be displayed in the Reception notice areas.

ACCESS :

Bus Route near to both surgeries Car Parking is available on the Surgery grounds and nearby

DISABLED ACCESS

Wheelchair access is available at both sites. Reserved parking areas are available. Broadway patients should inform the receptionists if you are unable to climb stairs. There is a lift at the Broadway Surgery site for patients who require assistance to the upstairs consulting rooms.

CANCELLING APPOINTMENTS

Please let the surgery know if you are unable to attend for your appointment. Missed appointments result in other patients having to wait longer for appointments. Failure to attend multiple appointments without a reason may result in your removal from the Practice List

COMMENTS OR COMPLAINTS

The practice welcomes comments and suggestions to improve its service for you. A comment box is available in the reception area for you to provide us comments and suggestions. We are currently developing a “Patient Participation Group”, if you are interested, let us know!

Complaints are dealt with according to the NHS Complaints Procedure and should be addressed to the Practice Manager. Please ask the receptionist for a leaflet, detailing “how to make a complaint”.

4 PATIENT PARTICIPATION GROUP

We have now established a Patient Participation Group (PPG) and we are looking to ensure that the members we have are representative of our practice population. If you are interested in joining the group, please contact the surgery and we will provide you with an application form or visit our website at http://www.broadwaysurgery.nhs.uk/ . We would be particularly interested in hearing from younger, disabled and ethnic minority patients as we have no representation from these groups at the moment.

MOBILE PHONES

The use of mobile phones may interfere with medical equipment. We operate a low tolerance policy for mobile phone use. Please do not use them within the surgery building. Please turn off your phone before entering the building.

NEW PATIENTS

Patients wishing to register with the Practice will need to complete a Registration Form. We request that you complete a health questionnaire and we will offer you a new patient medical with the practice nurse. Please bring all your medication with you for this appointment. You must live in our Practice Area to register.

NON-NHS SERVICES

The doctors offer certain services not covered by the NHS, these services include the completion of Insurance Forms, Driving License Medical Examinations, letters and Passport applications and so on. The current fees for these Non NHS services are displayed in the reception area. If you wish further details or require an appointment, please contact our reception staff.

FOREIGN TRAVEL

Advice and vaccinations for foreign travel is available from our Practice Nurses. We encourage you to make appointments to discuss travel advice and vaccinations at least six weeks or earlier before travelling.

Note: Fees are payable for many travel vaccinations.

REFERRALS

You will receive confirmation from the hospital that your referral has been received and details of the estimated waiting time. You will then be contacted prior to an appointment slot becoming available, in order to arrange a mutually convenient date and time for your appointment. You may have to wait up to thirteen weeks for an appointment in some busy departments.

6 FAMILY PLANNING SERVICES

Family planning and a range of contraceptive services are available from all the doctors and Practice Nurses. You can also attend local Family Planning Clinics for IUCD and Implant advice.

The main reasons for which your information may be need are:

The Training and education of staff; which you can choose whether or not to be involved personally.

Research approved by the local Research Ethics Committee. You will be contacted to see if you are willing to take part, you would not be identified in any published results without your prior agreement.

If at any time you would like to know more about how we use your information, you can speak to the person in charge of your care.

You have a right of access to your medical records. ( See separate paragraph )

Everyone working for the NHS and the Practice has a legal duty to keep information about you strictly confidential.

CHANGE OF PERSONAL DETAILS

Any changes to your name, address or telephone/mobile numbers etc should be notified to the surgery as soon as possible, so that we may update your records accordingly. If you change address and wish to change surgery site, please inform the receptionist who will arrange for the transfer of your medical records. If you move out of the Practice area, you may, in certain circumstances, be removed without notice.

Note: you must remain in our Practice Area to remain registered.

PROMOTING GOOD HEALTH & DISEASE PREVENTION

The Practice is committed to a policy of promoting good health and preventing disease. We encourage a no- smoking policy. There is no smoking permitted on the Practice premises or surrounding area. Health promotion advice is available to all patients if required

ADVICE AND INFORMATION

If you are unclear about the advice or treatment given to you, please do not hesitate to ask us any questions. Please remember that the staff are not authorised to disclose confidential information especially over the telephone to a friend or a family member without your consent.

7 ACCESS TO MEDICAL RECORDS

Competent patients may apply for access to their own medical records, or may authorise a third party, such as a solicitor, to do so on their behalf. Parents may have access to their child’s records if this is in the child’s best interest and not contrary to a competent child’s wishes. There may be a fee payable where access and copies of records are required. Medical information may be withheld if it is likely to cause serious physical or mental harm to the patient or a third party who has not given consent for disclosure. Formal applications for access to records must be in writing accompanied by written consent and the appropriate fee.

PATIENT RIGHTS AND RESPONSIBILITIES

The Practice staff will make every effort to ensure you have the best care and service available. In return we would hope from you will show courtesy towards all members of the Practice Staff. Inform us if you are unable to keep an appointment. Missed appointment results in others having to wait longer for an appointment.

ZERO TOLERANCE

The Practice is committed to the Government’s policy of Zero Tolerance with regards to threatening, abusive or discriminatory behaviour or comments towards members of staff.

Violent or abusive behaviour towards any member of staff will not be tolerated, which will result in your immediate removal from the Practice List

WHAT TO DO IN A BEREAVEMENT

If death occurs at home:-

1. Telephone the doctor. He/she will visit to confirm that death has taken place.

2. Contact the funeral director

When death occurs in hospital: -

1. Contact the Funeral Directors to inform them that their services will be required.

2. Collect Doctor’s death Certificate from hospital.

3. Take this to the Registrar for the area in which the death took place. Also take deceased medical card if available and birth certificate.

4. Take green form to Funeral Director who will take over complete responsibility for arranging the funeral.

Message from the Practice Manager

The practice endeavours to provide a comprehensive and efficient service to all patients registered at our surgery. We welcome any constructive comments from patients to help improve the services that we offer. Please feel free to put your ideas in writing and address them to the Practice Manager who will discuss them at the monthly meetings with the partners.

All employed staff receive appropriate training for their respective roles in providing the best possible care and service for you. All staff will respect your confidentiality whenever you use our service.

Please remember that the receptionists have a difficult and demanding job and I ask for your patience and understanding when your requests are being dealt with. There may be occasions when they ask for details of your condition to help the doctor to help you.
