Icenogle, Ph.D. Martha Arrieta, M.D., M.P.H., Ph.D. Center for Healthy Communities Research Office University of South Alabama Mobile, Alabama Disaster Preparation in the Florida Panhandle: Improving Continuity of Care for Those with Chronic Medical Conditions CHC works closely with churches, community groups, schools and health practitioners to eliminate health disparities, help marginalized communities access health care and support individuals to participate in decision making regarding their health through outreach activities, community economic development and research. CHC also seeks to enhance indigenous capacity around issues of race, class, culture and political links within a social justice frame. A major program of CHC is Project EXPORT (Center for Excellence in Partnerships for Community Outreach, Research on Health Disparities and Training), an initiative of the National Center on Minority Health and Health Disparities at the National Institutes of Health.
