This document was last updated in December 2020. Please confirm with your financial adviser that this is the most up-to-date document for your product or servicing needs. This form must be completed for all clients, including trustee (individual and corporate trustees) and corporate clients, where an application form does not already contain both a source of funding and verification of customer identity section. All references to Quilter International, we, us and our in this form mean Quilter International Isle of Man Limited.

Important information This form needs to be completed by both the client and the financial adviser. The client needs to complete sections A, B, C (but not C.6) and E. Please make sure that the declarations in section F are signed and dated by each party to the application. The financial adviser needs to verify the client’s identity by completing the following: – Sections D.1.1 and D.1.2 to verify the identity of an individual , directors of a or the beneficial owners/ trustees of a trust – Section D.2 to verify the identity of a company – Section D.3 to verify the identity of a corporate trustee. For ease of reference we have used an arrow to highlight certain facts and sometimes to let you know that you can find further information and explanations in the Guidance notes for the know your client form (referred to as the guidance notes) at the back of this form. All supporting information listed in this form should accompany the relevant application form and be certified by a suitable certifier as a true copy of the original where the original document is not provided. Occasionally, additional information other than that listed in the guidance notes section C, may be requested. If this form is being completed in respect of an additional investment for Quilter International Trust Company , the form should be signed by the settlor and financial adviser.

Completing the form To complete this form: – Use CAPITAL LETTERS only – Use blue or black ink – Complete all relevant sections – Do not use correction fluid; any amendments should be crossed out and initialled by all clients. We will contact you regarding any missing information which you will need to provide us with in writing, and this may delay your application. If you need guidance on how to complete the form you should contact – your financial adviser, if you are the client; or – your Quilter International regional office, if you are the financial adviser.

C.5 Withdrawals

This is only applicable if withdrawals have been requested in the application form. Payments will only be made to a bank account in the policyholder’s name; we will not accept requests to make payments to third parties other than those specified in the exceptional circumstances described below. In exceptional circumstances, payments can be made to UK Financial Conduct Authority (FCA) recognised financial , financial advisers/solicitors/accountants, who have the appropriate authorisation to handle client money. Please discuss such requests with Quilter International before submitting a withdrawal request. If you have requested that your payment is made to a third party, please state the reason for this in the box below. If funds are to be withdrawn from the policy, we will require additional information about the payment and the third party.

Please note: we may not comply with this request, in which case the payment will be made directly to the policyholder(s).

6 of 19 C.6 Financial crime – Risk rating – Financial adviser to complete Please refer to the Source of Funds and Source of Wealth guidelines (available from Quilter International) for information on how to complete the table below and section C of the guidance notes for information about the documentary evidence we may need. To prevent financial crime, Isle of Man authorised life adopt a ‘risk-based approach’ when obtaining evidence of the source of a client’s wealth. In order to speed up the application process we have provided you with the table below to allow you to find out your client’s risk rating before submitting the application form to us. We highly recommend that you complete the risk rating as it allows you to determine if you need to enclose further documentary evidence with the application form or not. If we do not receive the necessary documentary evidence with the application form, then it will take longer to process the business. In order to decide what risk rating applies to your client’s investment you need to take into account the following factors: (a) your client’s country of residence (b) which country the premium is paid from. Quilter International reserves the right to request further documentation if it is felt appropriate. Please note that each new application, or application for an additional investment, will be reviewed individually. If you are unsure about a particular application, please contact your Quilter International business consultant, or alternatively contact Quilter’s International Relationship Management Team directly on +44 (0)1624 655 009.

Section D – Verification of client identity – financial adviser/suitable certifier to complete Please note that, if you are applying for a new contract, we will not be able to set the contract live until this section has been completed and you have provided the necessary identification documentation. Please refer to section F of the guidance notes for more information on certifying documents. Covered in this section are the standard minimum requirements for: – an individual (in respect of a natural person) – see part D.1 – a company – see part D.2 – a trustee – see part D.3. Please complete the part(s) of this section that is relevant to your application. In some circumstances we may need additional information.

D.1 Verification of each individual’s identity (in respect of a natural person) We require one document from part D.1.1 and one from part D.1.2. If a passport or national identity card cannot be provided, please provide the reason why and two formal documents from D.1.2 both of which contain personal details and verifiable reference numbers. Identification documentation should be current and valid. Evidence of address should be the latest available, but no more than six months old. Any previous name or names or any aliases used must also be disclosed.

I understand that Quilter International Business Services, Quilter International Isle of Man Limited and/or Quilter International Ireland dac (Quilter International) will process personal information about me and any other party whose personal information I have provided. The type of personal information processed about me will depend on the purpose for which it has been collected and will include: – my contact details – information to verify my identity – information about my family, lifestyle, health and – my payment details. The processing of my personal information may take place in a number of jurisdictions and may be shared with other parties within or outside of Quilter plc for the general purpose of establishing, maintaining and servicing an insurance policy. The sharing of my personal data may be used for any or all of the following purposes, to: – check against credit reference or other databases to verify information provided for regulatory due diligence purposes and to prevent or detect financial crime including money laundering, terrorist financing, bribery and corruption, sanctions or fraud; – allow for the provision of services relating to enhanced due diligence, underwriting, , data hosting, online services, payment or reporting of any tax or levy, or any other services provided from time to time; – enable an appointed financial adviser or fund adviser to assist in the provision of services to the policyholder; – compile statistical analysis or market research, where information is not specific to the individual; – comply with any legal obligation which includes the releasing of personal information to regulators, law enforcement authorities or other bodies where there is a legal requirement to do so, including the sharing of information under regulations relating to the U.S Foreign Account Tax Compliance Act and The Organisation for Economic Co-operation and Development Common Reporting Standard; – enable an appointed discretionary asset manager or custodian to meet their legal or regulatory requirements, where that discretionary asset manager or custodian providing services in relation to a policy requests the personal data of an individual linked to an application, and where we are satisfied that such a discretionary asset manager or custodian has a legal or regulatory requirement to make such a request. Where my personal information is shared with a third party for the provision of services relating to my policy, my personal information will only be used for the purposes for which it was collected. In some circumstances this may involve a transfer of my personal information to a third party outside the European Economic Area (EEA). Note: We will be unable to proceed with the application if this document is not dated by the signatories.

Section A – Completing the form All clients need to complete sections A, B, C and E. The financial adviser needs to verify the client’s identity by completing the following: – sections D.1.1 and D.1.2 to verify the identity of an individual investor, the directors of a company or the beneficial owners/ trustees of a trust. – section D.2 to verify a company. – sections D.3 to verify a corporate trustee. In addition to the above mentioned, the financial adviser also needs to verify the identity of other connected parties, if applicable, for example an attorney or a guardian. Details of any other person(s) from whom we may take instructions should be provided with evidence of their authority and specimen signatures. Details should be provided in D.1.1 and D.1.2.

Trust business If policies are to be settled into trust after issue, please complete the relevant trust details in section D.3 of this form. If this form is being completed in respect of an additional investment to a policy where Quilter International Trust Company is the trustee, the form should be signed by the settlor and financial adviser.

Existing business Please note that when this form is completed in respect of an additional investment, assignment, or where information/ documentation has previously been provided in accordance with the current Isle of Man anti-money laundering and prevention of terrorist financing legislation, only the relevant sections of the form need to be completed. Any sections which are not relevant may be regarded as ‘Not Applicable’ (N/A).

Section B – Politically exposed person A politically exposed person is someone holding an important public position, or a person clearly related to them. Examples of these are: – Heads of State or Government – Senior politicians – Senior Government officials – Judicial or military officials – Senior executives of publicly owned – Important political officials

16 of 19 Section C – Source of Funds As part of global efforts to prevent money laundering and counter terrorist financing, all Isle of Man based companies have to collect additional information on source of funds for all investments. This should not be viewed as questioning the quality of your investment. It is a safeguard that will benefit the industry as a whole and ultimately protect you as an investor. The first step is to complete all the relevant information in section C of this form. In some circumstances we will require documentary evidence before we can process your application form. Your financial adviser can establish if documentary evidence is needed by contacting us before sending your application form to us. It is in the interest of all parties that you and your financial adviser make sure as early as possible what documentary evidence we need in order to avoid delays in starting your investment. We also recommend that you do not send us your premium payment until you have supplied us with all the information we require. The reason for this is that if we for any reason are unable to accept your application then we will return your payment to the original source. The list below reflects what documentary evidence is required for each source of funds; please state what evidence you are enclosing in this form. In certain circumstances, when an application form falls into a high risk group for money laundering purposes, we may require further documentary evidence.

Section D – Verification of your identity Additional documents to evidence your identity and residence In line with our anti-money laundering and countering terrorist financing requirements, we need the following documents to support your application: – Proof of identity – copy of your passport or national identity card. If these documents are not available please provide two formal documents carrying appropriate personal details and verifiable reference numbers (for example, bank statement, pension book or council tax bill). – Proof of residence – eg copy of a utility bill or bank statement. This should be the latest available, but no more than six months old. All documentary evidence, where the original document is not provided, must be certified by a suitable certifier (see section F of these guidance notes) as a true copy of the original; copies will not be accepted. Please note we are not able to return the documentary evidence to you. In some circumstances we may need further documentation to support your application, for example in relation to your source of funds. Your financial adviser will be able to provide details on any additional needs by carrying out a ‘risk assessment’ with you. We would encourage you to do this as without this further documentation, if applicable, the processing of your application may be delayed.

Section F – Financial adviser/suitable certifier – verification of customer identity As part of global efforts to prevent money laundering and terrorist financing, all Isle of Man based companies have to verify the identity and address of all individuals party to a contract (for example applicant(s), trustees, power of attorney). Parts D.1.1 and D.1.2 in section D of this form set out the various documents Quilter International are required to obtain, by the Isle of Man Financial Services. Please note that the requirements may change if the regulations are altered, in which case Quilter International may need further documentary evidence. Certifying documents Unless the documents provided are originals, they will all need to be certified by a suitable certifier. For some cases we may need official documents or extracts to be certified by an independent third party, see below. A suitable certifier can be any of the following: – a regulated introducer, or authorised employee of a regulated introducer, that is based and regulated in a Schedule 2 country as defined in the Isle of Man Financial Services Authority Guidance Notes (www.iomfsa.im); or – a person appointed in writing by Quilter International (suitable certifier); or – a solicitor, lawyer or advocate; or – notary public; or – an authorised representative of an embassy or consulate of the country who issued the identification document. When certifying a document, the suitable certifier must include the following wording: ‘I hereby confirm that this document is a true copy of the original which I have sighted and the photograph represents a good likeness of the client.’ The suitable certifier must also sign and print their name, state their company name, their position within the company and the date. UK lasting powers of attorney and UK enduring powers of attorney can be certified by the settlor, a solicitor or a notary public.
