This method successfully achieved fracture healing in femoral nonunion complicating osteomyelitis. 1 SICOT Online Report E038 Accepted June 2th, 2003 Introduction Chronic osteomyelitis (COM) is commonly sequel to acute haematogenous osteomyelitis and is frequently seen in children and adolescents. It is a common clinical entity in Nigeria, associated with immense morbidity, which is aggravated when a fracture occurs in the infected bone. Osteomyelitis associated with such fracture may lead to nonunion. Achieving healing of such nonunion is difficult and may require multiple surgical procedures. The difficulty may in part be due to depletion of endogenic bone morphogenic protein (BMP) at the fracture site, which has been observed in animal models [18]. It remains to be shown what the contribution is of local infection on such depletion and especially in human beings. Success with treatment depends on controlling the infection with prolonged antimicrobial therapy, inducing osteogenesis and ensuring stability at the fracture site, as illustrated by these case reports. Although internal fracture fixation is rarely indicated in the presence of an infection, the procedure can be useful, occasionally, in osteomyelitis of the femur. The aim of this case report is to highlight the morbidity associated with nonunion complicating chronic osteomyelitis of the femur and describe a method of achieving healing of such nonunion, in selected patients, using internal fixation devices. Case 1 A four year old girl was seen with COM of the right femur that started at the age of One month and a pathological fracture of the diaphysis of the same femur that occurred at the age of nine months and that has remained ununited, till presentation, despite treatment with skin traction and antibiotics at another hospital. A discharging sinus in the distal right thigh had persisted. There was painless abnormal movement at the fracture site but no demonstrable sequestrum on X-rays. Her standing height was102 cm and she had an eight cm shortening of the right lower limb. Pus from the sinus, on culture, grew staphylococcus aureus1. Treatment included skin traction to enforce rest, and oral erythromycin initially for three weeks to control the infection. To achieve healing of the nonunion, open reduction, debridement and internal fixation with an intramedullary Rush pin with allograft cancellous bone grafting was then done. Oral erythromycin was changed three weeks postoperatively to cefuroxime and rifampicin due to a flare of infection and 2 SICOT Online Report E038 Accepted June 2th, 2003 discharge. The sinus healed at five weeks and the new antibiotics were used for eight weeks and five months postoperative respectively. She was ambulated six weeks in a high above-knee cast, which was continued until radiological union was confirmed 20 months postoperative, fig.1. The intramedullary pin was removed 24 months postoperative and she remains well. Figure 1. Case 2 A 14yr old boy was admitted with a four month old osteomyelitis of the right femur that was pathologically fractured in the midshaft. The fracture was not united and the fragments were displaced with 5cm overlap. There were two profusely discharging sinuses in his distal thigh (pus was sterile) but radiographs showed no sequestrum in the femur. He was treated empirically with oral erythromycin, and skin traction for nine weeks without fracture healing. One of the sinuses had by now healed but the other still discharged intermittently. To achieve healing of the nonunion, open reduction, 3 SICOT Online Report E038 Accepted June 2th, 2003 debridement and internal fixation with a broad dynamic compression plate and screws was then done. Oral pefloxacin was commenced and continued for four months thereafter and radiological union of the fracture was confirmed six months postoperative (fig2) but a sinus discharged occasionally, at follow-up three years postoperative. Discussion Staphylococcus aureus remain the commonest pyogenic organism associated with COM, representing 60% to 90% of organisms isolated [1,7]. However, isolation of other gram positive, and gram-negative organisms is now more common than before [2]. The preeminence of S. aureus in causing osteomyelitis may be multifactorial. This includes the frequency of its asymptomatic carriage that range from 25% to 50% in the community [3], growth in a biofilm when in contact with living tissue like bone 4 SICOT Online Report E038 Accepted June 2th, 2003 [5, 9], and binding to sialoprotein in bone matrix [15]. Successful treatment of nonunion in a chronically infected bone depends on operative debridement, stable internal or external fixation, as is appropriate and antimicrobial therapy, which is most beneficial when used for several weeks [8,16]. Parenteral beta lactam agents yield clinical success for many patients while oral fluoroquinolones has proved as useful as parenteral beta lactams [6,13]. Combinations of beta lactam/rifampicin as in case1 and fluoroquinolone/rifampicin have also been found useful, achieving bacteriological cure rate of over 80% even in the presence of orthopaedic implants [6]. The antibacterial activity of rifampicin and beta lactams are not affected by hypoxia that occurs in infected bone tissue whereas aminoglycosides are affected and thus are less efficacious than the former [2]. It must be noted however that rifampicin is better avoided where tuberculosis is endemic so as to reduce the risk of resistance of mycobacteria to it. The standard operative treatment of nonunion is stable fixation and bone grafting [4,14] (osteosynthesis) but different procedures exists. External fixation can be done but the procedure has associated complications and failure of treatment (nonunion) does occur [6,12,17]. External fixators can be rather bulky and inconvenient for the patient [11]. Internal fixation on the other hand is convenient for both surgeon and patient as the fixation device is unobtrusive and does not require daily care. Infections associated with metallic devices can be eradicated with implant removal [16]. It must be emphasized that internal fixation for nonunion in an infected bone should be reserved for carefully selected patients. A larger series is required to establish selection criteria and its advantages. In conclusion, treatment of fracture nonunion in an infected bone is difficult. When the femur is affected, success can however be achieved by a combination of prolonged antimicrobial therapy, operative debridement and internal fixation in selected patients. References 1.     Special RepoRt Special RepoRt Imaging of osteomyelitis: the key is in the combination An accurate diagnosis of osteomyelitis requires the combination of anatomical and functional imaging techniques. Conventional radiography is the first imaging modality to begin with, as it provides an overview of both the anatomy and the pathologic conditions of the bone. Sonography is most useful in the diagnosis of fluid collections, periosteal involvement and soft tissue abnormalities, and may provide guidance for diagnostic or therapeutic interventions. MRI highlights sites with tissue edema and increased regional perfusion, and provides accurate information of the extent of the infectious process and the tissues involved. To detect osteomyelitis before anatomical changes are present, functional imaging could have some advantages over anatomical imaging. Fluorine-18 fluorodeoxyglucose-PET has the highest diagnostic accuracy for confirming or excluding the diagnosis of chronic osteomyelitis. For both SPECT and PET, specificity improves considerably when the scintigraphic images are fused with computed tomography. Articles were obtained from PubMed and screened for relevance to the topic of diagnostic imaging for osteomyelitis. The authors conclude that plain films are an appropriate first step, as they may reveal osteolytic changes and can help rule out alternative pathology. MRI is often the most appropriate second study, as it is highly sensitive and can detect bone marrow changes within days of an infection. Other studies such as CT, ultrasound, and bone scintigraphy may be useful in patients who cannot undergo MRI. CT is useful for identifying necrotic bone in chronic infections. Ultrasound may be useful in children or those with sickle-cell disease. Bone scintigraphy is particularly useful for vertebral osteomyelitis.

Circulating antibody to such antigens could not be detected by immunodiffusion, hemagglutination, immunofluorescence or complement mediated chondrocyte cytotoxicity as assessed by 51Cr release. Similarly, radiolabeled incorporation studies attempting to detect de novo synthesis of such antibody by circulating peripheral blood lymphocytes as assessed by radioimmunodiffusion, immune absorption to neuraminidase treated and untreated chondrocytes and immune coprecipitation were negative. Delayed hypersensitivity to cartilage constituents was studied by peripheral lymphocyte transformation employing [3H]thymidine incorporation and the release of macrophage aggregation factor. Positive results were obtained which correlated with periods of overt disease activity. Similar results were observed in patients with classical rheumatoid arthritis manifesting destructive articular changes. This study suggests that cartilage antigenic components may facilitate perpetuation of cartilage inflammation by cellular immune mechanisms. She was given cyclical etidronate after this vertebral collapse as she had suVered a previous fragility fracture of the left wrist. The age ranged from 1–12 years. There were 65 osseous lesions (excluding spinal and synovial). Seven had mul- tifocal bone involvement. Four basic types of lesions were seen: cystic (n=46), infiltrative (n=7), focal erosions (n=6) and spina ventosa (n=7). The majority of lesions were in the metaphyses (n=36); the remainder were in the diaphysis, epiphysis, short tubular bones, flat bones and small round bones. Bone lesions resembled chronic infections, simple and aneurysmal bone cysts, cartilaginous tumours, osteoid osteoma, haematological bone lesions and certain osteochondroses seen during the same period of study. Histological confirmation is man- datory to confirm the diagnosis of tuberculosis as several bone lesions can mimic tuberculous osteomyelitis. Weisbrode, DVM, PhD, Diplomate ACVP CASE I: 2173 (AFIP 2790938). Signalment: 3.5-month-old, male intact, Chow-Rottweiler cross, canine (Canis familiaris). History: This 3.5-month-old male Chow-Rottweiler mixed breed dog was presented to a veterinary clinic with severe neck pain. No cervical vertebral lesions were seen radiographically. The dog responded to symptomatic treatment. A week later the dog again presented with neck pain and sternal recumbency. The nose was swollen, and the submandibular and popliteal lymph nodes were moderately enlarged. The body temperature was normal. A complete blood count (CBC) revealed a marked lymphocytosis (23,800 lymphocytes/uI). Over a 3-4 hour period there was a noticeable increase in the size of all peripheral lymph nodes. Treatment included systemic antibiotics and corticosteroids. The dog became ataxic and developed partial paralysis. The neurologic signs waxed and waned over a period of 7 days, and the lymphadenopathy persisted. The peripheral blood lymphocyte count 5 days after the first CBC was done revealed a lymphocyte count of 6,000 lymphocytes/uI. The clinical signs became progressively worse, and the dog was euthanized two weeks after the initial presentation. Laboratory Results: Immunohistochemical (IHC) staining of bone marrow and lymph node sections revealed that tumor cells were negative for CD3 and CD79α. Gross Pathology: Marked generalized lymph node enlargement was found. Cut surfaces of the nodes bulged out and had a white homogeneous appearance. The spleen was enlarged and meaty.

It is considered an extreme form of osteitis fibrosa cystica that occurs in the settings of persistently elevated parathyroid hormone. Case Report: This a unique lesion presented in a 48 year old male with recurrent bone pain and known End Stage Renal Disease (ESRD) on maintenance haemodialysis. The main clinical complaints were weak and painful legs and the initial presentation was after the patient collapsed at home and fractured spinal level C2. The initial assessment included blood tests and radiological imaging. CT scanning of the spine revealed a destructive lytic lesion with loss of height and architectural changes of the C2 vertebral body and cord compression. The differentials included an acute fracture, a metastatic lesion and Brown’s tumour. Further imaging with an MRI of the spine and PET-CT were performed which confirmed the above lesion and excluded metastatic disease and bone marrow infiltration.
