The EOI must be submitted in sealed envelopes to be superscribed as "EOl for supplying of spectacles under NPCB&VT'. 2. The EOI should be accompanied by duly signed photocopies of the valid (i) Trade License Certificate (ii) PAN card, (iii) lncome Tax Retum for the last financial year, (iv) P. Tax clearance certificate, (Y) GST Registralion Certificate/retum, (vi) Credential Cerlilicate (vii) others. Financial proposals must be submitted as per attached ANNEXURE-I 4. The EOI documents are to be addressed to BMOH Nayagram, Kharikamathani Rural Hospital, JhalEram, PIN-721159". 5. The submission of EOI may be done by registered post, courier or by hand on or before I9'h February 2021. to the address as given in D.4. Openine of EOI: proposals will be opened by the Superintendent/BMOH, Nayagram Multisuper Speciality Hospital. Jhargram or his authorised representative on the specified date and time. One authorized representative representing a bidder may remain present at the time of opening of Technical as well as the Financial proposals, if they so desire, at O/o the Kharikamathani, Nayagram Block, Jhargram, PIN-721 I 59 F. General Terms & Conditions; 1. Rate shall be inclusive ofall i.e. taxes, cost ofspare parts,-job work, labour charges, transport etc. 2. Rate inust be quoted in both wo[ds and figures. In case of discrepancy between figure and word amount, the word amount will be taken into consideration. 3. In case of Syndicate of transporters, the photocopy ofvalid Memorandum of Association shall be required to be submitted along with the tender paper. 4. In case ol Co-operative, the photocopy of valid (i) Memorandum of Association, (ii) Society Registration Certificate, (iii) Latest Audit Report conducted by the Statutory Authority shall be required to be s bmitted along with lhe tender paper. 5. Income Tax and other incidental charges shall be levied from the bill as per I.T. Act, l96l and relevant Acts & Rules. The rates and procedures shall be revised as the parent Acts, Rules, Orders will be revised time to time. 6. In case of lessee of a workshop, the photocopy of valid proper deed with the owner of the workshop shall be required to be subrnitted along with the quotation paper. 7. No advance payment will be made 8. Maximurn Priority will be given to the quality of frzme and lens & ensuring proper fitting during hand over. 9. The vendor must provide hard plastic box-/ease with soft cloth with each spectacle. This box/case will have embossed (preferably)/sticker fixed on it on either ofthe following trvo (number'i' will be a must) or all ofthe three logos :- (i) NPCB&VI logo (ii) NHM logo (iii) Biswa-bangla logo and there would remain the dotted space for writing the name ofthe beneficiary after supply. 10. Specifications of spectacles frame:-Good quality of cell lrame (cellulose acetate) with spring side. Frame should be fitted with face and nose properly (shape and size). Different Colours of the frame may be made available. Vendor will take the colour choice lrom the student. Sample of the frames to be preserved at the facility for checking in future. 11. Specification of the Ophthalmic Lens:-good quality of CR-39 glasses for students and presbyopic patients (scratches and bubbles free), high index glasses in case of more than 4D power of the sttLdents. Bilocal glasses should be according to power (distance & near) plescribed by MT (optometry). PD should be strictly maintained. Sample ofthe lenses to be preserved at the facility for checking. 12. Defective spectacles detected during hand over to the beneficiary should be charged fr& ofcost. 13. THREE months warranty period for the spectacles frame (replace or repair) should be there without any additional cost involvement. 14. Optician, lrom the vendors' end, will need to remain present as a mandate during the OPD/Outreach sessions (if is needed so) and during SES program sessions for distribution ofthe spectacles to the beneficiaries to check its proper fittings etc. 15. Refractive error should be determined for students up to the age of l2 years, only at the Eye OPD ofthe nearest Hospital/Vision Centre at the RIyBPHC in presence ol the MT (Opto) for testing of the refraction under the use of cycloplegic drugs elc. 16. lf any student requires bifocal glass e.g., in post-operative cases of the congenital cataract the vendor will provide the same according to the prescription. 17. Each beneficiary witl get a written paper, in the prescription-equivalent mode, mentioning the power of the glasses s/he has been supplied with. 18. 'l'he spectacles will need to be supplied latest within l0 days after receiving the work-order from the authority. 19. Spectacle Delivery System: For the spectacles for the school children, the vendor will submit the supply to the MT (Opto) at the RH/BPHC/ Hospital concerned for a preliminary checking (power, quality of fiame/glasses etc.) by him. Then after the approval from the MT (Opto). the BMOH/Hospital authori!/HOD will allow the vendor to deliver these checked spectacles to the schools. Belore that, the BMOH/Hospital authority or his authorized person (Mo/pharmacist/store- keeper',?HN/ANM/other nursing staffl will pick up a sample randomly from the lot on which the quality check will be done by the district quality committee located at the office of the CMOH. Thereafter the Headmaster or his authorized school-teacher will officially receive the supplies frorr.r the vendor with a detailed list or students & the frame, lens details. He will sign the received voucher with mobile number and with official rubber seal. Two receipt copies of Challan may be handed over to the vendor to claim the reimbursement bill. The sample that was picked up will reach its beneficiary within next 3 weeks through local anangement if/after it passes through the quality check at the district committee. Acceptance of EOI: The best quality and the lowest rate sample supplying bidder, within Rs. 350i- (lncluding all Taxes), would be accepted. However, the Tender Accepting Authority does not bind himself to do so and reserves the right to reject any or all the EOIS, for valid reasons and also reserves the right to distribute the work amongst more than one bidding firm / agency. The authority reserves the right to accept/ r'eject any or all EOIs without assigning any reason thereof. The contractor so detennined by the office shall enter into a contract by executing terms of the contract in a Fifty Rupee Non Judicial Stamp Paper with the Office. The contractor shall purchase the NJ stamp paper in the name of the BMOH Nayagram, Khadkamathani Rural Hospital, Jhargrarn. Cost of execution of agreement shall be borne by the contractor. Final work order shall be issued only after execution ofthe agreemelrt by the successful contractor. Apaft ftom other things, all clauses of this EOI document shall fonn the integral part of the agreement. The work must be commenced within a period of03 days from the date ofaward ofwork. Period of Contract/Extetrsion: The vendor selected through the tender process will remain valid for supply for next three years or till the tirne then the approved rate under NPCB&VI gets changed whichever is earlier. The contract will remain va.lid only for three years unless terminated earlier by the Tender inviting authority. Alter expiry ol the extended period, the contract will automatically come to an end and no separale notice rvill be given.

India has shown a remarkable increase in its crude literacy rate since its independence. But still it is well under the average literacy rate of the world. West Bengal being one of the most populous states of India has shown the same increasing trend in literacy rate. Paschim Medinipur is a very highly populated district of West Bengal. The western part of this district consists of Jhargram Subdivision which is economically very backward compared to the other Subdivisions of this district. This paper analyses the trend of literacy rate in different C.D Blocks of Jhargram Subdivision spatio- temporally over a period of forty years. It is found that the literacy rate is continuously increasing in the area but has remained lower than that of National and State levels. It has been found that although the educational level has increased in the area but in reality a large number of population of Binpur I, Binpur II, Gopiballavpur II and Nayagram, who are included in literate group, cannot write anything other than their names. Thus there exists a gap between actual number of educated people and literacy rate produced by Census of India. Challenge:- The Western Districts of West Bengal are mostly covered by Laterite soil & Jungles. Population of these area are mainly dominated by various tribal people. They depend on the jungle produce to maintain their livelihood. But it is hard to earn their livelihood only on jungle produce as deforestation is regular phenomenon in this area. 4. Initiative:- Sericulture, an Agro based Cottage Industry, plays a key role in ameliorating the economy of Rural Bengal. Out of the 4 (four) commercially exploited varieties of silk, Tasar, a major one has been practised in newly created district of Jhargram. In the Jungle Mahal areas of Jhargram district, Tasar culture is one of the major ways for income generation and maintenance of livelihood of the rural people who mostly belong to Tribal community. It is a commercially attractive economic enterprise providing employment and livelihood to about 2000 rural families of Jhargram district because of its low requirement of fixed capital and higher returns at regular intervals. Tasar culture is also an age old tradition of the Tribal people residing mainly in the blocks of Nayagram, Gopiballavpur-I, Gopiballavpir-II, Jamboni, Binpur-II, Sankrail etc. of Jungle Mahal areas of Jhargram district. 5. Key Result:- At present, there are 2(two) Technical Service Centres (T.S.C ) in this district to serve these huge number of Tasar families. Despite of acute shortage of staff, normal activities of these units for providing Govt. assistance under various schemes as well as technical guidance among the Tasar rearers have not been affected badly due to engagement of 8 (eight) Tasar Bandhu under Rashtriya Krishi Vikas Yojna (R.K.V.Y.).
