La ectoparasitosis se desarrolla cuando la hembra grávida penetra la piel de un hospedero susceptible, como el ser humano, y sufre un proceso de hipertrófica en el cual genera miles de huevos que expulsa al ambiente donde se completa el ciclo de vida. Se caracteriza por lesiones papulares, negruzcas, únicas o múltiples, que suelen afectar generalmente los pies, principalmente en las zonas sububgueales y periungueales, y son muy pruriginosas. La enfermedad es autolimitada y tienden a resolverse espontáneamente en 4 – 6 semanas; sin embargo es frecuente la reinfección y la enfermedad puede asociarse a múltiples complicaciones. La mejor estrategia para controlar la enfermedad es la prevención de la infestación; sin embargo se ha establecido, el mejor tratamiento es la extirpación quirúrgica de la pulga bajo técnica aséptica. It belongs to the neglected tropical diseases and is prevalent in South America, the Caribbean and sub-Saharan Africa. Tungiasis predominantly affects marginalized populations and resource-poor communities in both urban and rural areas. In the endemic areas, patients do not have access to an effective and safe treatment. A proof-of-principle study in rural Kenya has shown that the application of a two-component dimeticone (NYDA®) which is a mixture of two low viscosity silicone oils caused almost 80% of the embedded sand fleas to lose their viability within 7 days. -

The sucking and chewing lice (Phthiraptera) are hemimetabolous insects that spend their entire lives on the bodies of their hosts. Fleas (Siphonaptera), on the other hand, are holometabolous. The diversity of these ectoparasites is limited by the diversity of the birds and mammals available as hosts. Determining the community diversity of lice and fleas is essential to understanding ecological structure and interactions, yet offers a number of challenges to the ectoparasitologist. The chapter explores medical and veterinary importance of lice and fleas. They are more likely to be considered detrimental parasites, perhaps even a threat to conservation efforts by their very presence or by the disease agents they transmit. Perez-Osorio emphasized the importance of a more objective approach to conservation strategies by abandoning overemphasis on charismatic fauna and setting priorities in ecological management of wider biodiversity issues. When most people see a bird or mammal, they don’t look beneath the feathers or hair of that animal to see what is hidden. They see the animal at its face value, and seldom appreciate the diversity of life before them. The animal is typically a mobile menagerie, infested by external parasites and their body laden with internal parasites and pathogens. -

Arthropod Infestation and Envenomation in Travelers Traveler Summary Key Points Ticks: Ticks found in grass or brush transmit a large variety of infections, some serious or fatal. Travelers should wear long, light-colored trousers tucked into boots and apply a DEET-containing repellent. The longer a tick is attached, the higher the risk of infection. Ticks should be pulled straight out with tweezers by grasping close to the skin to avoid crushing the tick. Fly larvae (myiasis, maggots): Botfly or tumbu fly infestation results from deposition of eggs under the skin, which causes a boil-like bump to form. Simple surgical removal may be necessary. Spiders: Most spiders do not have toxic venom. Harmful species include recluse, black or brown widow or hourglass, and Australian funnel-web spiders. Investigate damp, dark spaces (such as outdoor toilets, kayaks, and damp shoes) before entering. Fleas: A flea engorged with eggs burrowing into the foot may result in painful tungiasis; surgical removal is always required. Fleas rarely may transmit plague. Travelers should avoid dusty areas and exposure to rodent fleas. Scorpions: Most fatal scorpion bites occur in tropical and dry desert regions. Size of the scorpion does not indicate potential toxicity. Favorite hiding places for scorpions are cool, shaded areas, such as under rocks or furniture. The affected area should be immobilized, iced (if feasible), and immediate medical help sought. Lice Lice are blood-eating insects found worldwide but most commonly transmitted in conditions of overcrowding and poor hygiene. Budget travelers staying in basic accommodations may encounter lice under these conditions. Lice not only cause itching and rash, they can also cause disease. -
