NHS Forth Valley seeks to appoint one Consultant in Old Age Psychiatry, leading a well established Community Mental Health Team for Older Adults (CMHT OA) in West area covering approximately 12,000 older people.

The successful applicant will be responsible for the comprehensive mental health care of patients within the catchment area, supported by a well developed Community Mental Health Old Age team and associated Care Home Liaison services. The catchment area size reflects the redesign of services leading to well established teams who take on the majority of initial assessments and respond to emergencies with appropriate input and supervision from the consultant. Inpatient beds are available in Forth Valley Royal Hospital and continuing care beds are provided across three Community Hospitals in Clackmannanshire, Stirling and Bo’ness. The post has additional Specialty Doctor support and well established links with local Primary Care teams and Social Work partners.

There is a Consultant led acute hospital liaison team for older people and also an integrated admission ward run by Ageing and Health medicine, with mental health support. This is an exciting time as you will be able to influence ongoing service redesign and planning for health and social care integration.

You will be expected to take part in the Psychiatry on call rota which is currently 1 in 15. The appointment will be subject to the terms and conditions of service of the Consultant Grade (Scotland).

Consultant Psychiatrist Old Age Psychiatry

As a Consultant working in Falkirk Community Mental Health Services for the older adults you will be expected to participate and deliver mental health promotion, timely detection, diagnosis and management of dementia and functional illnesses, plus support education for carers.

The delivery of Specialist Old Age Psychiatry services is provided through a variety of setups including acute admission and rehabilitation beds, day hospital and memory clinics, domiciliary and outreach care, and outpatient /community clinics. The services are designed around well developed sectorised community mental health teams. A small number of Hospital Based Complex Care beds are provided for people with severe and persistent behavioural problems that necessitate specialist medical and nursing care, located in Stirling, Sauchie and Bo’ness.

Consultant offices, community teams, Specialist nursing services and day hospitals for this post are co located in Falkirk Community Hospital.

Community Mental Health Team for Older Adults (West) The community mental health team for older people in Falkirk provides integrated assessment, care planning, psychiatric treatment, care co-ordination and follow up to older people with mental health problems in the community. Referrals are accepted from Primary Health Care teams at practice meetings, directly from GPs, locality social workers, from acute hospital staff and from secondary care psychiatrists.

The community mental health team provides, support by working with patients, their families and carers in their own homes and in other community settings, including residential and nursing care, and acts as a gateway to other services. Good practice involves the identification of a key worker and regular case review. The South sector CMHT currently operates a sectorised model with CPNs aligned to designated GP practices.

The team operate a same day service for emergencies Monday to Friday 9am - 5pm. CMHT 2 senior band 7 RMN, 2 band 5 RMN along with access to a health care assistant, part time physiotherapist and 2 Senior Occupational Therapists; team is supervised by team leader with support from a clinical nurse manager. It undertakes range of assessments including domiciliary functional assessments, therapeutic interventions and post diagnostic support groups for those with dementia.

The majority of initial assessments are carried out by a band 7 nurse, who, in conjunction with the sector Consultant, will determine a diagnosis and develop a treatment plan.

A wide range of psychological and pharmacological interventions are available including anxiety management, CBT and cognitive enhancer therapy.

Treatment interventions will be undertaken by any member of the sector team deemed to have the necessary skills. Regular case supervision is carried out and joint working with other healthcare providers and Social Work is encouraged.

Local representatives from Area Social Work teams attend the weekly case reviews to allow fast track referrals from one service to another and to highlight any concerns regarding existing patients.

Psychiatric Day Hospital The day hospital service has 20 places per day for older people with mental illness. It provides:  Intensive multidisciplinary assessment and treatment for older people with complex mental health needs.  Aims to reduce hospital admission  Focus on rehabilitation  Sessional and home based treatment  Interventions that are time limited  Comprehensive extended multidisciplinary mental health assessment and risk assessment  Individual and group psychological interventions (counselling, CBT, AMT, coping strategies, coping with loss)  Education and advice to carers  Monitoring of psychotropic medications and health maintenance  Support care staff in mainstream services in order to maintain people at home

It currently provides services from Tuesday to Friday with extended day hospital services on:

Tuesday Functional patients Wednesday Post-diagnostic support clinic for Dementia Thursday Living life to the full course for anxiety and depression. Friday Functional patients Monday Day hospital staff deliver alternative Community based services which include Health Maintenance Clinic, Fast track Psychiatric Support Clinic to Ageing & Health medicine OP services, Neuropsychiatric education and support group for complex co morbidity.

Outpatient Clinics Currently clinics are offered at Falkirk Community Hospital including Consultant led community based clinics in selected care homes.

Clinics include:  General Outpatient including medications monitoring  Cognitive enhancer clinic  Memory clinic  Neuropsychology clinic

A domiciliary service is available for those unable to attend clinics.

Memory Assessment Services Memory assessment services aim to provide timely diagnosis, treatment/management (psychological and social as well as pharmacological) and aftercare for people with dementia and their families.

You will be well supported by the community mental health team in providing post diagnostic support. You will be involved in diagnosing dementia, providing advice about prognosis and the options for packages of care and support as necessary, providing psychological and social interventions for people with dementia and their carers, providing pharmacological treatment for cognitive impairment, behavioural and psychological symptoms and follow up of patients as appropriate in clinical setup.

Inpatient beds 20 Organic and 20 Functional acute assessment beds are located in Wards 4 & 5 at Forth Valley Royal Hospital. These beds are shared across Forth Valley and each Consultant will retain responsibility for their own inpatients supported by three junior doctors and their Specialty Doctors.

The purpose built unit offers single room accommodation for each patient. Each ward is led by a band 7 Charge Nurse. The unit offers excellent facilities including a modern and well equipped OT kitchen and physiotherapy gym. Both the wards have regular input from psychologists. The dementia ward uses the stress and distress model to manage distressing behaviours and has access to an activity coordinator. This has recently won an award for outstanding care at Board level.

Community beds Hospital Based Complex Care beds are offered in three community hospitals, patients living in Falkirk District are largely placed in Bo’ness Hospital. This is a purpose built unit for dementia sufferers with severe and enduring behavioural problems. It is co-located with a large general practice and medical rehabilitation unit. Day to day medical cover is provided by portfolio GP team and regular Consultant ward rounds.

Demetia Outreach Team Dementia link nurse support was developed in Forth Valley to continue the ongoing support to patients prescribed cognitive enhancers. This was an established service much before the recent initiative through the National Dementia Strategy. This has now been redesigned to provide rapid access to all the patients with dementia diagnosis experiencing stress and distress. This service currently has 4 dedicated nurses working at Band 6 and 5 level and 2 health care assistants with supervision from medical team

Care Home Liaison Service This service delivers education and training to care home staff across the sector, as well as regular clinics and individual patient assessment and reviews. Additionally, specialist clinics in two homes, Torwood and Burnbrae support staff in the care of patients with severe and enduring mental health problems who otherwise would

have required LT hospital care. The care home liaison service at present has 1 Band 7 RMN specialist nurse and a band 5 nurse.

Teaching, audit and research The Consultant will be expected within his/her NHS responsibilities, to undertake audit, and to partake in undergraduate and postgraduate teaching as required. The consultant will be expected to take an active part in the teaching of psychiatry trainees on placement in the Old Age Psychiatry service.

Job plan The post holder is required to agree a detailed job plan which will be reviewed at least annually. Efforts will be made to complete job planning prior to the date of commencement in post, or, if this is not possible, as soon as possible following this date.

NHS Forth Valley consultant posts are initially for 10 PAs. We are keen that SPA- related activity over and above the 1 PA of SPA for individual requirements (CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings) is offered to new consultants. TERMS AND CONDITIONS

Hours of Work 10 programmed activities. NHS Forth Valley will require each consultant to agree with the General Manager or his / her deputy and the Associate Medical Director a job plan of fixed and other commitments, which will be reviewed on an annual basis and revised as necessary to meet the requirements of the local and national service issues.

Salary £80,653 - £107,170 per annum pro rata based on seniority. NHS Forth Valley does not negotiate salary placements. On commencement of employment, salary will be determined in accordance with Section 5 of the terms and conditions of service.

Tenure Permanent

Responsibility The post holder will be professionally responsible to the Associate Medical Director, Psychiatry and managerially responsible to the General Manager, Forth Valley Community Services Directorate.

Notice Period The employment is subject to three calendar months notice on either side.

Annual Leave Annual leave entitlement is six weeks per year (pro rata for part time post holders), with additional statutory holiday entitlement as agreed by the Local Negotiating Committee and published annually.

Relocation The post holder will normally be expected to reside within 30 minutes travelling distance of Forth Valley Royal hospital. If NHS Forth Valley Health Board considers that the there is a requirement for the post holder to relocate his or her home in order to take up the responsibilities of the post, relocation expenses will be available in accordance with the Board’s Relocation Policy. Support to the level of 15% of starting salary up to a maximum of £10,000 may be available, subject to the provisions of the policy.

Superannuation This appointment will be superannuable under the NHS Superannuation Scheme, which is contracted out of the State Earnings Related Pension Scheme (SERPS).
