Long-term Uncontrolled Hereditary Gingival Fibromatosis: A Case Report Abstract Hereditary gingival fibromatosis (HGF) is a rare condition characterized by varying degrees of gingival hyperplasia. Gingival fibromatosis usually occurs as an isolated disorder or can be associated with a variety of other syndromes. A 33-year-old male patient who had a generalized severe gingival overgrowth covering two thirds of almost all maxillary and mandibular teeth is reported. A mucoperiosteal flap was performed using interdental and crevicular incisions to remove excess gingival tissues and an internal bevel incision to reflect flaps. The patient was treated 15 years ago in the same clinical facility using the same treatment strategy. There was no recurrence one year following the most recent surgery. Guideline # 18 ORAL HEALTH RATIONALE Dental caries, commonly referred to as “tooth decay” or “cavities,” is the most prevalent chronic health problem of children in California, and the largest single unmet health need afflicting children in the United States. A 2006 statewide oral health needs assessment of California kindergarten and third grade children conducted by the Dental Health Foundation (now called the Center for Oral Health) found that 54 percent of kindergartners and 71 percent of third graders had experienced dental caries, and that 28 percent and 29 percent, respectively, had untreated caries. Dental caries can affect children’s growth, lead to malocclusion, exacerbate certain systemic diseases, and result in significant pain and potentially life-threatening infections. Caries can impact a child’s speech development, learning ability (attention deficit due to pain), school attendance, social development, and self-esteem as well.1 Multiple studies have consistently shown that children with low socioeconomic status (SES) are at increased risk for dental caries.2,3,4 Child Health Disability and Prevention (CHDP) Program children are classified as low socioeconomic status and are likely at high risk for caries. With regular professional dental care and daily homecare, most oral disease is preventable. Almost one-half of the low-income population does not obtain regular dental care at least annually.5 California children covered by Medicaid (Medi-Cal), ages 1-20, rank 41 out of all 50 states and the District of Columbia in receiving any preventive dental service in FY2011.6 Dental examinations, oral prophylaxis, professional topical fluoride applications, and restorative treatment can help maintain oral health. -

Ludwig's angina is also otherwise commonly known as "angina". Previously this disease was deemed as fatal but later on it was concluded that with proper treatment this infection can be removed and the pateint can recover. It mostly occurs in adults and children are not affected by this disease. As the infection spreads further it would affect the wind pipe and lead to swellings of the neck. The skin around the neck would also be infected severely and lead to redness. The individual would mostly be febrile during this time. Since the airway is blocked the individual would suffer from difficulty in breathing. If the infection spreads to the internal ear then the individual may have audio impairment. Although there are variations in the mesio-distal widths of the teeth in any individual when proportions are used, the sizes of the unerupted permanent teeth can at least be fairly accurately pre-determined from the mesio-distal measurements obtained from the measurements of already erupted permanent teeth. As the mandibular permanent central breaks tissue, a mesio-distal measurement of the tooth is taken. The size of the lower adult lateral is obtained by adding 0.5 mm.. to the lower central size (see a). (a) Width of lower lateral = m-d width of lower central + 0.5 mm. The sizes of the upper incisors then become important as well. The upper permanent central is 3.25 mm.. wider than the lower central (see b). COHG Prioritisation Exercises 2011-14
COHG Prioritisation Exercises 2011-14 Cochrane Oral Health Group has undertaken an extensive prioritisation exercise to identify a core portfolio of the most clinically important titles to maintain. COHG defined 8 areas of dentistry, fit 234 existing titles within the scope of those areas, and subsequently contacted all authors of Cochrane titles relevant to these areas for their opinion of which they felt to be most important. After analysis of the authors’ rankings, lists of the titles our authors thought to be most important were ratified and discussed by panels of experts in August 2014 for each of 6 remaining areas of dentistry (periodontology, dental public health, oral medicine, oral and maxillofacial surgery, cleft lip/palate, and operative and prosthodontic dentistry), with orthodontics (January 2011) and paediatrics (February 2013) having been undertaken in previous years using the same methodology. For some topic areas, in addition to prioritising existing titles, expert panels also identified new titles of clinical importance considered to be currently missing on The Cochrane Library. The resulting list of priority titles will be subject to revision periodically (as the evidence-base in oral health grows over subsequent years), to ensure that COHG’s editorial resource continues to be invested in the most clinically important reviews. Each dental area’s panel was comprised of members considered to be internationally-renowned experts within their field, and who are currently active in clinical practice and/or research. For the August 2014 cluster of dental areas, each panel also included membership from IADR’s Global Oral Health Inequalities network to assist in ensuring that global burden of oral health conditions/disease was considered fully when discussing how each area’s research ought to be prioritised. -

4 TRENDS Dentistry A PROFESSION IN TRANSITION Dentistry in the United States is in a period of transformation. The population is aging and becoming more diverse. Consumer habits are shifting with Americans increasingly relying on technology and seeking greater value from their spending. The nature of oral disease and the financing of dental care are in a state of flux.1 Are you prepared? Utilization of dental care has declined among working age adults, a trend that is unrelated to the recent eco- nomic downturn. Dental benefits coverage for adults has steadily eroded the past decade. Not surprisingly, more and more adults in all income groups are experiencing financial barriers to care. Total dental spending in the U.S. slowed considerably in the early 2000’s and has been flat since 2008. The shifting patterns of dental care utilization and spending have had a major impact on dentists. Average net incomes declined considerably beginning in the mid-2000s. They have held steady since 2009 but have not rebounded. Two out of five dentists indicate they are not busy enough and can see more patients, a significant increase over past years.1 Below you will find some statistics regarding the decline in the utilization of dental care. Tooth Decay Information
ToothMasters Information on Tooth Decay Definition: Tooth decay is the destruction of the enamel (outer surface) of a tooth. Tooth decay is also known as dental cavities or dental caries. Decay is caused by bacteria that collect on tooth enamel. The bacteria live in a sticky, white film called plaque (pronounced PLAK). Bacteria obtain their food from sugar and starch in a person's diet. When they eat those foods, the bacteria create an acid that attacks tooth enamel and causes decay. Tooth decay is the second most common health problem after the common cold (see common cold entry). By some estimates, more than 90 percent of people in the United States have at least one cavity; about 75 percent of people get their first cavity by the age of five. Description: Anyone can get tooth decay. However, children and the elderly are the two groups at highest risk. Other high-risk groups include people who eat a lot of starch and sugary foods; people who live in areas without fluoridated water (water with fluoride added to it); and people who already have other tooth problems. Tooth decay is also often a problem in young babies. If a baby is given a bottle containing a sweet liquid before going to bed, or if parents soak the baby's pacifier in sugar, honey, or another sweet substance, bacteria may grow on the baby's teeth and cause tooth decay. Causes: Tooth decay occurs when three factors are present: bacteria, sugar, and a weak tooth surface. The sugar often comes from sweet foods such as sugar or honey.
