Community Mental Health Program

Transitional Case Management (TCM) The goal of transitional case management is to proactively intervene, so that individuals receive timely, appropriate support to address needs and avert potential crisis. TCM will provide transitional follow-up until ongoing supports are in place (e.g, Intensive Case Management, Outpatient Psychiatry follow-up, Addiction Services, Counselling) and in some circumstances, extended hours of an individual's existing supports.

Who is an appropriate referral? Those requiring linkage and system(s) navigation and/or community support until ongoing services are in place. This includes needs for brief support (one-time situation) or transitional supports up to 6 months.

Transitional Case Management Services will:  Add provision of responsive & engaged linkage and system(s) navigation;  Add mobility and follow-up where necessary to the Central Access Service for addictions and mental health operated at CMHA;  Provide enhanced MH&A outreach services through alignment with several new local health and social service hubs (SSM Police - Gore Street Hub, SSM Soup Kitchen - Health & Child Care Hub);  Provide enhanced hours of community mental health and addictions system support to Sault Area Hospital emergency department and mental health and addiction department by adding mobility and follow-up where necessary.

Financial assistance & budgeting Family support Crisis support Supportive counselling Medication monitoring

The referral process includes an intake assessment to determine needs, strengths and goals. Call 705-759-3935. Supportive Housing The Housing Support System is comprised of two components:

(1) Provision of ongoing support from a Case Manager and/or Community Rehabilitation Worker as needed and as determined by each tenant. Along with Psychiatric Case Managers, services are provided by Community Rehabilitation Workers (CRW's) to support consumers to live in accommodations of their choice. The services offered include:

 Flexible and individualized support which varies in intensity.  Assistance in locating and maintaining housing.  The priority of the program is to provide services to people who are homeless or at risk of being homeless.

CRW's assist in building an individual's activity of daily living skills through one on one and group interventions, including Community Kitchens and Walking Groups.

(2) Provision of a rental subsidy may be available to enable people with a severe mental illness who are homeless or at risk of becoming homeless to secure safe and affordable accommodation.

Safe Affordable Housing APH also collaborates, working directly, with other community housing organizations to ensure all individuals living with mental health and addiction issues have access to safe affordable housing.

Community Treatment Orders (CTO) Some individuals may receive case management services and support through a Community Treatment Order. A CTO is a treatment and service plan, which is issued by a physician in consultation and coordination with the consumer and service providers. Peer Support If you receive services from Algoma Public Health and are dealing with issues related to mental health or addiction, you can now access peer support. Peers are those who share something in common. In our group, we all share the experience of mental illness and/or addiction issues. We understand because we've been there. We get together to help each other live better lives,

23

and to share our wisdom about what works to keep us well. Whatever your personal goals are, your peers can help you get there. Respect, equality, empowerment, and positive identities centered on strengths, are some of the principles that are practiced in peer support. You can meet with the peer support worker individually or in a group. Meetings are confidential, so you can share your story in a safe environment, free from judgment.

Rapid Access Addictions Medicine Clinic (RAAM)

What is the RAAM Clinic? The RAAM clinic is a Rapid Access Addictions Medicine Clinic for people looking for help with their addictions. The doctors and other staff at the clinic have experience treating people who are struggling with their drinking or substance use, and they know how difficult it can be to ask for help. You’re welcome to come to the clinic even if you’re not sure your addictions are a problem, or if you know there is a problem but you’re not yet ready to stop. If you have questions about starting treatment, please ask.

HISTORY The RAAM clinic staff will begin by asking you about your history of substance use: how old you were when you started using, how much and how frequently you use, and so on. The point of these questions is not to make you feel guilty or defensive, but to get a complete picture in order to determine how to best help you. Try to remember that the staff are not there to judge you, and will be most able to treat you if you’re completely honest.

DIAGNOSIS Based on your history, the RAAM clinic doctor will provide you with a diagnosis. If you sometimes drink/use more than is recommended but, are often able to drink/use moderately, do not suffer serious social harms from drinking/using, and do not go through withdrawal when you stop, the doctor may determine that you are at-risk.

On the other hand, if you often avoid your responsibilities due to drinking/using, have trouble drinking or using moderately, continue even when you know it’s a bad idea, and go through withdrawals when you stop, the doctor may diagnose you with an alcohol/opiate use disorder. Both of these conditions are treatable. Those found at-risk will usually be given brief advice, while patients with use disorders may be offered counselling and medication

MEDICATION If you’re struggling with alcohol cravings, your doctor may suggest that you try a medication that will help to lessen these cravings. If you use Opioids, medication may be recommended to support withdrawal from these substances. Medication to support withdrawal usually makes other types of treatment much more effective and reduces the risk of relapse. These medications are safe, effective, and non-addictive. You and your doctor will work together to determine the appropriate one for you.

COUNSELLING & SUPPORT SERVICES The RAAM clinic staff may refer you to counselling as part of your treatment plan. Counselling programs can include education on alcohol/opiates and healthy lifestyle choices, group and individual therapy sessions, help with developing coping skills, cognitive behavioural therapy, and self-help groups like Alcoholics Anonymous (AA) Narcotics Anonymous (NA). The doctor

45

and other staff will work with you to determine what form of counselling and support services would be the most helpful for you.

BRING SUPPORT If you’re feeling anxious or hesitant about going to the RAAM clinic, please feel free to bring a supportive person with you. Changing your habits can be very difficult, and you may find that having someone with you while you speak to the doctor makes you feel less overwhelmed and less alone.

REFERALS ARE ACCEPTED BY: The clinic will accept referrals from hospital emergency departments, primary care physicians, nurse practitioners (NPs), community addictions and mental health services, social services, families and is open to walk-ins.

Please call ahead for an appointment. Valid Health Card is required.
