COVID-19 PROTOCOL: Every person who comes onto the show grounds must sign the USEF Release & Waiver of Liability form and the Region 5 COVID-19 Release. Compliance with all Covid-19 Protocol is required. Every person on the grounds must be on the Essential Personnel List sent by the trainer. DOGS are not allowed with-in the fairground fences. They may be in the RV areas. EARLY ARRIVALS are limited to those who travel in excess of 16 hours and must be coordinated with the Show Manager, Richard Galarza. HIGH POINT AWARDS: One award to the high point junior and one award to the high point adult amateur. All JTR, AATR, ATR and AOTR classes will count and you are automatically entered. New this year and sponsored by the Marcinkowski’s of Hidden Meadows Arabians. Be sure to thank them! FUTURITIES: The Young Sport Horse Jackpot Futurity Rules and Entry Form are on page 12. Both require entry and payment in addition to entering the Region 5 SH Championship Show. Don’t miss the deadlines! PAYMENT OF ENTRIES: Payment using PayPal is preferred. Payment is due the same day you email your entry. Be sure to use “Sending to a Friend” or any incurred fees will be passed on to you. There is a “how to” on page 18. Personal checks, if necessary, are to be mailed with a postmark the same day as entry is emailed. RV PARKING 2021: Full hook-ups are limited & are $35 per night. Electric & water hookups are $30 per night. RV’s, LQ trailers, nor trailers of any type are allowed to park in an RV spot without paying the fees. Please purchase on entry form. SPECTATORS are not allowed at the show in 2021. STALL CLEANING: The facility has made the show responsible for stall cleaning after the show; therefore, the show is charging a fee of $15 per stall to hire labor to clean stalls. Stalls which are vacated and left with more than a 6” depth of bedding (no banking against walls) will be invoiced an additional $10 per stall. Please do your part to ensure this show can continue to provide bulk bedding. Tack rooms are to be swept clean and all lumber, etc. removed from stalls and barn front. SUPREME CHAMPION IN-HAND: Champions from each in-hand class are eligible to compete in the new Supreme Champion In-Hand class. Participation is encouraged but not required. There is no entry fee for this class. USEF SAFE SPORT: Be sure you are current on USEF Safe Sport Training before the show.

IN-HAND INFORMATION ❖ Entries open 0001 May 15. In-Hand times may be limited, so please email your entry early, but not before 0001 May 15. Entries received earlier will be returned. The close of entries is May 30 at midnight. If slots fill, a wait list will be established and available slots offered from it in the order received. ❖ In-Hand classes will be run in class schedule order. Please check in at the gate when your class is called. If you have a conflict, let the paddock master know ASAP. Only the paddock master has the authority to assist you. If you are not at the gate and checked in prior to the last horse in your class, you will have missed your class. The gate will not be held. ❖ The Supreme Sport Horse In-Hand Champion is open to each first place in-hand horse (Arabian & Half-Arabian) from every in-hand class except the two futurity classes. There is no entry fee nor a requirement to show back. Judges will confer and choose one champion. Not an AHA class. ❖ Futurity entries may cross enter into a SHIH class. DRESSAGE INFORMATION ❖ Classes will not be scheduled in this order. ❖ Dressage warm-up will be in Arena 101 (small covered). ❖ A single judge, either Rene Johnson or Janet Curtis, will be assigned to Classes 573-596. Assignments will be made at the time rides are scheduled. Rene Johnson will judge all Western Dressage Classes, 597-604. ❖ Entries open 0001 May 15. ENTRY REQUIREMENTS – Need to know information ENTRY is to be made via email using the form in this Prize List. Make certain you have updated your Adobe Reader (links at www.regionv.com). Please double check that all numbers you type in are accurate. FORMS: Download the forms & save. Using the tab key, navigate through the fields. (You can manually move your cursor as well.) Enter your class number and the fee for the class. The form will calculate the fees as you enter. When completed, save this form and attach to an email to [email protected]. The Entry Form is one horse per form but if you own another and are entering it, save a copy of the first form (title file with horse name) and just over-type the horse info, classes entered and change the rider information if different. No need to re-enter owner, trainer, etc. Save this form with the new horse’s name to attach to the email. Repeat as needed. In other words, over-type any fields you need to, the others will save. “Saving” is the key to success. SIGNATURE PAGES are required to be emailed with the entries. Be sure to sign for all involved. As stated on the forms, electronic signatures are binding. There are three signature pages this year and an Essential List: 1) The normal back of entry form, AHA/USEF release: each owner, trainer, rider, and parent/guardian for a minor owner, trainer or rider. 2) The new USEF Waiver of Liability: each owner, trainer, rider, and parent/guardian for a minor owner, trainer or rider, every person appearing on the Essential Personnel List. 3) The Region 5 COVID-19 Agreement & Release: All persons coming on the show grounds for any reason. ESSENTIAL PERSONNEL LIST: Each Trainer is to complete this form with the names of all persons in his/her group who will come on to the show grounds. Remember, the person signing as Trainer is the person responsible for the care, custody and control of the horse while on the grounds. If you are an amateur who trains your own horse, you are the “Trainer” and need to sign the Trainer line. If you are the Parent/Guardian of a minor under the age of 18, you will sign all signature lines and, as signor, do not need memberships. This form has to be completed only once per Trainer. Additional people can be added at the facility entrance gate. Spectators are not allowed but minor riders may have one adult in attendance. MEMBERSHIP CARDS, HORSE PAPERS: Copies of AHA, USEF cards and horse papers do NOT need to be sent. A “Competition” AHA membership is required for everyone (including Walk/Trot riders) at this Regional Championship Show. A “Competing” USEF membership (except Walk/Trot 10 & Under riders) is required or a Non-Member fee of $45 will be charged. All adult riders entering amateur classes must have amateur certification and therefore a competing membership. My software searches AHA & USEF: I’ll be checking for Amateur Certification AND that Safe Sport Training is current. Junior exhibitors who are 18 years of age must also complete Safe Sport. See more about Safe Sport and Canadian Amateur riders in General Rules. Copies of registration papers are not required; just make certain you enter numbers correctly and HA/AA numbers must be preceded by 1A, 2A etc. The owner(s) on the entry form need to match the owner(s) on the horse’s papers. If purchasing on contract or leasing, include that agreement. PAYMENT: PayPal is preferred on the same date as entry is emailed. Checks must be mailed on the same day as the entry is emailed. The Jackpot classes are not approved by AHA. There are deadlines to meet to enter the classes. ARENAS: Arena footing is sandy loam or sand. Measurements are: Main arena – 120’x248’, Covered Warm-up – 100’ x 200’, Front sand ring – 128’ x 254’, NE sand ring – 130’ x 200’, sand ring between barns 105/106 – 73’ x 164’. Several sand lunge rings are available. AWARDS: Region V Champions & Reserve Champions will receive a prize, a neck garland, neck ribbon and plaque. The remainder of the Top Five will receive a neck ribbon and plaque. A full Top Five will be awarded regardless of the number of entries in a class. However, only awards made to the top 50% of the class (rounded up and not to exceed 5 awards) will receive Arabian Breeders Sweepstakes payouts, AHA Horse Achievement Awards, Amateur Achievement Awards, High Point Horse Achievement Awards, High Point Amateur Achievement Awards & Distance Award points. AWARDS ROOM: Located at the out-gate corner of the main arena. Champion and Reserve Champion awards must be picked up and signed for at the awards room. Open during sessions. Unclaimed awards will not be mailed. BIKES, MOPEDS, ROLLERBLADES, RAZORS, SCOOTERS are not allowed inside the fairgrounds fences to comply with fairground rules. CLASS CONFLICTS: Exhibitors are cautioned against entering the same horse in successive classes. Due to full schedules, the gate will not be held for riders beyond the normal gate hold allowed. No refunds will be given due to class conflicts. DOGS: Dogs are not allowed within the fairground fences, barns or arenas. They may be in the RV areas. DRESSAGE WARM-UP: Arena 101 is the dedicated dressage warm-up arena when Dressage classes begin. The competition rings (Main Arena and The Covered Outdoor Arena) will be available for warm-up as much as possible. Please vacate promptly when requested. DRIVING DIRECTIONS TO THE SHOW: FROM THE SOUTH: I-5 North then merge onto I-405 N via exit 154 toward Renton and Bellevue. Merge onto WA 522 East via Exit 23 toward US 2 to Woodinville/Wenatchee. Turn right onto US 2. Turn right onto 179th Ave SE. The trailer entrance is approximately .3 mile on right. FROM THE NORTH: I-5 South to Seattle then merge onto I405 S via exit 182 to Bellevue/Renton. Merge onto WA522 East via Exit 23A toward WA 202/Monroe/Wenatchee. Turn right onto US 2/Stevens Pass Hwy. page 8 General Rules & Information, cont’d GATE/GATE HOLDS: When it is found that a gate hold must be requested by an exhibitor, such request shall be made to the Paddock Master prior to the close of judging of the class immediately before the class requiring the gate hold. The reason for the re quest must be stated. The gate-hold will be a maximum of three (3) minutes. Appropriate warning will be given to all exhibitors if there is a gate hold in effect. AR 107.1: When horses are competing collectively, if a first horse does not enter the in-gate ready to participate within one minute from when the class is called into the ring, a warning is issued. a. Exhibitors will then have one additional minute to start the steady stream of horses into the in-gate. b. If no horses have entered the gate at the conclusion of the additional one minute, the gate shall be closed and the class is deemed concluded. c. Once the first horse has entered the in-gate, a steady stream of horses must be maintained. d. Should the steady stream be broken after the two minutes have elapsed, the gate shall be closed and judging will commence. e. A steady stream is defined as a constant flow of horses moving forward through the in-gate with a reasonable and safe distance between them (i.e. two to four horse or horse and buggy lengths in between). f. Show Management in conjunction with the Steward may deviate from this procedure as circumstances warrant. A starting order will be established in all classes in which horses compete individually and one minute allowed for an entry to enter the ring. GOLF CARTS: Are allowed with proof of insurance on file in the show office. Please read and comply with the USEF Motorized vehicles rule below. Please park in golf cart designated areas. MOTORIZED VEHICLES: Minors who do not have a valid driver’s license which allows them to operate a motorized vehicle in the state in which they reside will not be permitted to operate a motorized vehicle of any kind, including, but not limited to, g olf carts, motorcycles, scooters, or farm utility vehicles, on the competition grounds of licensed competitions. Minors who have a temporary license may operate the above described motorized vehicles as long as they are accompanied by an adult with a valid driver’s license. The parent/legal guardian or individual who signs the entry blank as a parent or guardian of a minor operating a motorized vehicle i n violation of this rule are solely responsible for any damages, claims, losses or actions resulting from that operation. Violations of this rule will be cause for penalties against those responsible for the child committing the offense. Wheelchairs and other mobility devices for individuals with disabilities are exempt from this rule. The unsafe use of personal electronic transport devices, as determined by the competition officials and management in their sole discretion, that do not require a driver’s license to operate, including but not limited to segways, hover boards, and single wheeled scooters is prohibited on competition grounds. Operating such devices in areas where people gather may be deemed “unsafe use” if there is a risk of harm to others. If the operator of the device is a minor, the parent/legal guardian or individual who signs the entry blank as a parent or guardian of a minor operating the device in violation of this rule is accountable and may be subject to penalties. Wheelchairs and other mobility assistance devices for individuals with disabilities are exempt from this rule. OFFICE: The show office is located at the North end of the main arena. The office may be closed during meal hours. Office hours are as follows: June 30 from 8:00 AM to 9 PM. July 1 – July 3 from 7 AM to 30 minutes after the last class. PARKING: Parking is not allowed in the fenced fairgrounds area. Truck & trailers must move immediately after unloading. PAYMENT OF ENTRIES: PayPal is preferred; payment is to be made on the day the entry is emailed. PayPal details are on the entry form. There is also PayPal "Sending to a Friend" information on page 18. All fees charged to us by PayPal due to not "Sending to a Friend" will be passed on to you. Checks, if necessary, are to be mailed to the show secretary on the day the entry is emailed. PRIZE MONEY: AHA Sweepstakes monetary awards will be paid by AHA in accordance with program rules. Futurity checks will be mailed as stated in the program rules on page . PROTECTIVE HEADGEAR: All juniors riding in hunter, jumper and hunter seat equitation sections, while riding anywhere on the competition grounds, must wear properly fitted protective headgear which meets or exceeds ASTM/SEI standards for equestrian use and carries the SEI tag. Harness must be secured and properly fitted. Any rider violating this rule at any time must immediately be prohibited from further riding until such headgear is properly in place. Any exhibitor may wear protective headgear (ASTM/SEI) and/or a protective safety vest, specifically designed for use in equestrian sport in any division or class without penalty from the judge. The Federation recommends that the vest meet or surpass the current ASTM standard or be certified by the Safety Equipment Institute. QUALIFICATIONS FOR REGIONALS: Qualifications are not required for the 2021 show. RV/CAMPER PARKING: Please pay for your RV spots on your entry form. See the FYI page for details. All towing and additional vehicles must be parked in the parking areas; no vehicles or horse trailers without living quarters are allowed in an RV spot. SCORING SYSTEMS: All classes are judged by a single judge except the Supreme In-Hand Championship for which judges will confer. STALLIONS: Stallions may be shown in any class except Walk/Trot/Jog 10 & under classes. SPECTATORS are not allowed at the show due to COVID-19. Updates to this policy, if any, will be on our website & your entry acknowledgment. SUBSTITUTION OF JUDGES: Region 5 reserves the right to substitute, re-assign or hire additional judges if the need arises. STABLING: Stalls are wood and/or metal and have full doors. Most stalls are matted.

page 9 General Rules & Information, cont’d USEF NON-MEMBER PARTICIPATION: If membership documentation is not submitted to the competition and the competition cannot verify such information, the exhibitor will be required to complete a Show Pass form and pay a non-refundable Show Pass fee of $45.00. Purchase of a Show Pass fee does NOT allow a competitor to show in amateur classes. To show in an amateur class, a competitor must provide proof of membership with amateur certification. USEF MEMBERSHIP: Life, senior active and junior active members shall be eligible to participate in all classes at Regular Competitions, Competitions at the Modified Level or above and Competitions at the Advanced Level, Dressage, and Vaulting Competitions and Endurance Rides. A nonmember may participate as a handler, rider, driver, owner, lessee, agent, coach or trainer at Regular Competitions, Eventing Competitions, Dressage Competitions, Reining Competitions and Combined Driving Competitions upon payment of a $45 Show Pass fee. Participants in the following classes are exempted from the Requirements of this rule: 1) leadline; 2) exhibitions; 3) games and races; 4) classes for 4-H members; 5) walk trot (exception: Friesian performance FR222) and academy classes (academy classes are classes limited to horses used regularly in a lesson program); 6) USDF introductory level tests, pas de deux and quadrille classes; 7) NRHA Endorsed Reining Competitions. 8) Opportunity classes, 9) citizens of other nations who have proof, in English, of current membership in good standing of their own National Federation, 10) USEA beginner novice division; and 11) assistant handlers in Dressage Sport Horse Breeding classes. USEF SAFE SPORT POLICY: As of January 1, 2019, if you are a USEF competing member 18 years of age or older you are required to take the core Safe Sport Training within 30 days of activating your membership. Take the Safe Sport Training at www.usef.org by logging into your member dashboard. Please remember to complete the most recent required course before your due date. Reminder: All USEF Life and Senior Active members, as well as participants who have regular contact with minor athletes, are required to comply with the U.S. Center for Safe Sport training requirements to be eligible to participate in the competition. All participants must adhere to the U.S. Center for Safe Sport Code. USEF Safe Sport Policy, and the Minor Athlete Abuse Prevention policies. USEF VACCINATION RULE: effective 12/01/15, all horses competing at a USEF recognized show must have proof of current vaccinations for rhino and flu. Exhibitors must furnish proof upon the request of show management. Please do not send proof of vaccination with your entry. WITHDRAWAL OF ENTRY: All fees, except the office fee of $25, will be refunded with a veterinarian note or medical doctor note which must be presented to the show secretary by 6 PM June 30, 2021. The North Cascades Arabian Horse Club is pleased to present the 4th Annual Young Sport Horse Jackpot Futurity, an exciting opportunity for handlers to win money while showcasing the most athletic young fillies, geldings, and colts at the Region 5 Sport Horse Championship Show. The Futurity will be split into two sport horse in hand jackpot classes for horses age two and under and divided by Purebred and Half Arabian/Anglo Arabian entries. Jackpot money will be divided up among the Top 5 winners in each class and consist of each class’s jackpot futurity enrollment fees, in addition to money provided by jackpot futurity sponsors.

GENERAL RULES

❖ Current AHA Membership with Competition Card is required for participation. ❖ All entries must be Purebred or Half Arabian/Anglo Arabians registered with AHA or CAHR. ❖ The jackpot classes will be a combination of yearling and two-year-old fillies, geldings, and colts and will not be split except for by Purebred and Half Arabian/Anglo Arabian registration status. ❖ Entries will be scored in hand on the triangle and adhere to the sport horse in hand class procedure as outlined in ARABIAN SPORT HORSE SUBCHAPTER AR-17 of the USEF Rulebook. ❖ Jackpot payout divided: 5 or more entries: champion 35%, reserve 25%, third/fourth 15%, fifth 10%. 4 entries: champion 40%, reserve 30%, remaining entries 15%. 3 entries: champion 50%, reserve 35%, remaining entry 15%. 2 entries: champion 60%, reserve 40%. 1 entry: champion 100%. ❖ Winners will be mailed a check within ten days of the completion of the show. page 15 USDF Statement on Animal Welfare The United States Dressage Federation, Inc. (USDF), a federation of individual members and over 100 independent organizations throughout the United States, supports the concept of ethical treatment of animals. Dressage is a classical method of training horses, based upon the horse’s natural responses, as carried out in a gradual, planned program during which the horse’s athletic abilities are improved in a systematic manner. As a result, the horse becomes capable, comfortable, and responsive in partnership with the rider without the use of force. Dressage competition at various levels of achievement is the ultimate test of the training program to determine whether the desired harmony between horse and rider has been achieved. The rules pertaining to dressage competition prohibit the use of artificial appliances, training aids, and drugs, which would force the horse to submit. Instead, the emphasis is upon training and a cooperative partnership with the rider. Major competitions are carefully monitored to assure that there is no unnecessary force or harshness displayed. Unlike other horse sports in which horses enter training at a relatively immature state, dressage training is usually not seriously begun until the horse is three or four. The top levels of achievement require five or more years of progressive training and it is not unusual to see horses in international competition in their late teens. This, in itself, places a premium on good care and soundness throughout the horse’s life. While supporting the concept of animal welfare in its broadest form, the United States Dressage Federation, Inc., also recognizes that it is unrealistic to ascribe human emotions and responses to the horse which in its wild state is a creature of flight, living a hard and precarious existence. Under domestication, the horse is assured of ample food, water, and shelter, as well as protection against infectious diseases and damaging parasites, thus leading a longer and healthier life with a high rate of survival. As an educational organization, USDF has numerous programs, which teach people how to pursue the accepted, humane training methods. A program of certifying instructors also emphasizes protecting the horse’s welfare in pursuit of dressage training. For competition details on member benefits, including educational programs, awards, and other opportunities, refer to the USDF Member Guide or is recognized the USDF website at www.usdf.org. by USDF for 2021 Membership Requirements for USEF-licensed/USDF-recognized Competitions: Owners and riders, including foreign riders and owners who are not residents of the US, wishing to participate in a USEF-licensed/USDF-recognized competition as a rider or owner/lessee, must have either a USDF Group Membership (GM), Participating Membership (PM), or have a USDF Non-Member (NM) identification number and pay the USDF Non- Member (NM) fee. (Note: USDF Business Members can own horses but cannot compete as riders with their USDF Business Membership.) Horses competing at USEF-licensed/USDF-recognized competitions must have either a USDF Horse Identification (HID) number or a USDF Lifetime Horse Registration (LHR). Proper credentials or verification certificates must be presented to the competition secretary. Riders and owners wishing to participate in a USEF-licensed/USDF-recognized competition, and not able to present the competition secretary with verification of a USDF membership or NM identification, and USDF LHR or USDF HID, must complete a USDF Competition Affidavit Form and pay the $5 affidavit filing fee. A copy of a completed affidavit form from another competition may also be used for verification purposes for up to 60 days. (Exception: Affidavits cannot be used at Great American Insurance Group/USDF Regional Dressage Championships.)

Exceptions: • Horses competing only in Individual Breed Classes (IBCs) at DSHB competitions, sires and dams of horses in DSHB group classes, where those sires or dams are not actually competing in the same competition, horses competing only in breed restricted dressage or DSHB classes at Regular Competitions (e.g. all Arabian, all Friesian, or all Morgan classes), or in USDF Introductory Level tests, pas de deux or quadrille, are exempt from this requirement. Also exempt are horses ridden in leadline, exhibitions, games and races, classes for 4-H members, walk-trot, academy, and opportunity classes. • Riders and owners/lessees competing only in classes which are exempt from the USDF HID requirements are also exempt from the USDF NM identification number and NM fee requirements. Also exempt are handlers of horses competing in DSHB In-Hand or Group classes.

Memberships Defined Participating Membership (PM): A membership issued to an individual directly by USDF. The membership year is December 1 through November 30. PMs are available for 1-year ($90), 5-years ($360), or life ($1,800). A USDF Youth Participating Membership (YPM) is available for individuals who have not reached their 21st birthday by December 1 of the current membership year ($72). A PM allows the member to compete at USEF-licensed/USDF-recognized competitions and to participate as a rider and/or owner/lessee for all Adequan®/USDF Year-End Awards and the Great American Insurance Group/USDF Regional Dressage Championship program. Group Membership (GM): A membership issued to an individual by USDF when the member name and dues are submitted to USDF through an affiliate Group Member Organization (GMO). The GM year begins December 1 and ends November 30. A GM allows the member to compete at USEF- licensed/USDF-recognized competitions and participate in the USDF Rider Awards Program and the USDF Regional Schooling Show Awards Program. Business Membership (BM): A membership that runs December 1 through November 30 and is issued to a business or organization directly by USDF. The USDF BM ($240) offers a wider variety of advertising benefits. BMs satisfy requirements of horse ownership only. Non-Member (NM) Identification Number: For each USDF NM owner/lessee and USDF NM rider there will be a USDF NM fee of $35 per competition that must be collected by the show secretary and submitted to USDF, along with the USDF Report of Fees document and the $5 affidavit fee and Competition Affidavit Form if the person cannot provide a copy of their USDF NM card. Owners/lessees or riders exempt from the NM number requirements are also exempt from paying the USDF NM fee. If there are multiple owners of a horse and all are NMs, only one of the owners must pay the NM fee. If the horse has at least one owner who is a current USDF PM, GM, or BM there is no NM fee assessed. Competition management will be invoiced for all applicable USDF NM fees not received with the USDF post-competition paperwork, along with a report listing all USDF NMs. Competition management is then required to submit the NM fees within 30 days to the USDF. An individual with a USDF NM identification number is not eligible for any USDF membership benefits. Once a USDF NM number has been assigned, a USDF NM card can be obtained from the USDF website. Horse Identification (HID) Number: A one-time tracking number for horses ($35). Scores earned by a horse with an HID number will be recorded, but will NOT be counted towards Adequan®/USDF Year-End Awards. A horse with an HID number is not eligible to compete in Great American Insurance Group/USDF Regional qualifying or championship classes. An HID number can be upgraded to a Lifetime Horse Registration (LHR) for a fee of $80. If a horse has a USDF LHR it does not need an HID number. An HID number is a one-time fee that does not need to be renewed. Lifetime Horse Registration (LHR): A horse with an LHR ($115) is eligible for Adequan®/USDF Year-End Awards, and eligible to compete in Great American Insurance Group/USDF Regional qualifying and championship classes, assuming requirements for the owner and rider are met. If a horse has an LHR it does not need an HID number. An LHR does not need to be renewed.

page 20 USEF WAIVER AND RELEASE OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT For and in consideration of United States Equestrian Federation, Inc. dba US Equestrian (“USEF”) allowing me, the undersigned, to participate in any capacity (including as a rider, driver, handler, vaulter, longeur, lessee, owner, agent, coach, official, trainer or volunteer) in a USEF sanctioned, licensed or approved event or activity, including but not limited to equestrian clinics, practices, shows, competitions and related or incidental activities of the Arabian Horse Association Events (“USEF Event” or “USEF Events”); I, for myself, and on behalf of my spouse, children, heirs and next of kin, and any legal and personal representatives, executors, administrators, successors,, and assigns, hereby agree to and make the following contractual representations pursuant to this Agreement (the “Agreement”): A. RULES AND REGULATIONS: I hereby agree to be bound and abide by the rules, regulations, and policies of USEF as published in the USEF Rule Book and on the website at www.usef.org, as amended from time to time. B. ACKNOWLEDGMENT OF RISK: I knowingly, willingly, and voluntarily acknowledge the inherent risks associated with the sport of equestrian and know that horseback riding and related equestrian activities are inherently dangerous, and that participation in any USEF Event involves risks and dangers including, without limitation, the potential for serious bodily injury (including broken bones, head or neck injuries), sickness and disease (including communicable diseases), trauma, pain & suffering, permanent disability, paralysis and death; loss of or damage to personal property (including my mount & equipment) arising out of the unpredictable behavior of horses; exposure to extreme conditions and circumstances; accidents involving other participants, event staff, volunteers or spectators; contact or collision with other participants and horses, natural or manmade objects; adverse weather conditions; facilities issues and premises conditions; failure of protective equipment (including helmets); inadequate safety measures; participants of varying skill levels; situations beyond the immediate control of the USEF Event organizers and competition management; and other undefined, not readily foreseeable and presently unknown risks and dangers (“Risks”). EQUINE ACTIVITY LIABILITY ACT WARNING: CAUTION: HORSEBACK RIDING AND EQUINE ACTIVITIES CAN BE DANGEROUS. RIDE AT YOUR OWN RISK. Under the laws of most States, an equine activity sponsor or equine professional is not liable for any injury to, or the death of, a participant in equine activities resulting from the inherent risks of equine activities. C. ASSUMPTION OF RISK: I understand that the aforementioned Risks may be caused in whole or in part or result directly or indirectly from the negligence of my own actions or inactions, the actions or inactions of others participating in the USEF Events, or the negligent acts or omissions of the Released Parties defined below, and I hereby voluntarily and knowingly assume all such Risks and responsibility for any damages, liabilities, losses, or expenses that I incur as a result of my participation in any USEF Events. I also agree to be responsible for any injury or damage caused by me, my horse, my employees or contractors under my direction and control at any USEF Event. D. WAIVER AND RELEASE OF LIABILITY, HOLD HARMLESS AND INDEMNITY: In conjunction with my participation in any USEF Event, I hereby release, waive and covenant not to sue, and further agree to indemnify, defend and hold harmless the following parties: USEF, USEF Recognized Affiliate Associations, the United States Olympic & Paralympic Committee (USOPC), USEF clubs, members, Event participants (including athletes/riders, coaches, trainers, judges/officials, and other personnel), the Event owner, licensee, and competition managers; the promoters, sponsors, or advertisers of any USEF Event; any charity or other beneficiary which may benefit from the USEF Event; the owners, managers, or lessors of any facilities or premises where a USEF Event may be held; and all directors, officers, employees, agents, contractors, and volunteers of any of the aforementioned parties (Individually and Collectively, the “Released Parties” or “Event Organizers”), with respect to any liability, claim(s), demand(s), cause(s) of action, damage(s), loss, or expense (including court costs and reasonable attorney fees) of any kind or nature (“Liability”) which may arise out of, result from, or relate in any way to my participation in the USEF Events, including claims for Liability caused in whole or in part by the negligent acts or omissions of the Released Parties. E. COMPLETE AGREEMENT AND SEVERABILITY CLAUSE: This Agreement represents the complete understanding between the parties regarding these issues and no oral representations, statements or inducements have been made apart from this Agreement. If any provision of this Agreement is held to be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from this Agreement and shall not affect the validity and enforceability of any remaining provisions. page 21 REGION 5 COVID-19 AGREEMENT To be read and signed by all trainers, owners, show staff, trainer staff on the show grounds and sent with entry.

• I certify that to the best of my knowledge I am free of COVID 19 and have been following the guidelines set forth by the CDC and the governing agencies of my primary place of residence. I have not experienced any of the COVID 19 symptoms, tested positive, or been in contact with someone who has tested positive for COVID 19 in the last fourteen days. • I certify that at least once each day of the horse show I am monitoring my temperature. If my temperature is over 99.5 F I will not enter the showgrounds. • I understand that the horse show will operate as a “Show and Go” experience and that I will not spend extra time on the grounds before and after competing. • I understand that all of the Best Practices, rules, and other requirements are in effect from that date horses are first allowed to move in until the show concludes, regardless of when competition is running. • I understand that the stable area will be restricted to essential staff only. Trainers will provide a list of essential staff and owners with entries. • I agree to wear a face covering at all times I am on the show grounds except when mounted on a horse. Social distancing (6 feet) will be maintained at all times including when mounted and anywhere on the show grounds. Masks may be worn without penalty when mounted in warm-up or while showing. • I will follow all rules regarding arenas and authorized riding arenas and times. • I understand that I may not bring friends or family with me to the show. Exception: junior riders (minors) may be accompanied by one parent or guardian. That person must adhere to all the same rules and procedures as the exhibitor. • I understand and agree that I, and as applicable, my clients will abide by the above and follow all rules implemented due to the Covid-19 health emergency during the show and/or at any time on the horse show grounds. I understand any violations will be cause to be asked to leave the grounds.
