    What are the Risks of Chiropractic Neck Adjustments? By William J. Lauretti, DC From time to time, news items have reported on the supposed risks of chiropractic manipulation to the neck. As a practicing chiropractor who uses neck manipulation everyday in my practice, I find these reports exaggerated and alarmist. I’ve been in practice for over 15 years, and I’ve personally performed well over 15,000 neck adjustments without a single serious complication that I am aware of. I often perform neck adjustments on my friends, family and loved ones, and often receive them myself from colleagues. Based on the available scientific evidence and my past clinical experience, I’m personally convinced that neck adjustments are a very safe and very effective form of treatment. Clearly, the risks of neck manipulation are not as extreme as some of the more irresponsible reports imply. Stroke and Neck Manipulation Criticisms regarding the safety of chiropractic neck treatments usually focus on the remote risk of a stroke or cerebrovascular accident (CVA) occurring as a result of the treatment. The most likely mechanism of stroke from neck movement involves injury to the vertebral artery (VA) as it goes through the sides of the upper cervical vertebrae and into the base of the skull. The part of the artery that passes through the cervical spine is called the vertebral artery. The continuation of the artery into the base of the brain is called the basilar artery. Both sections together are usually called the vertebrobasilar system. Rotation of the neck may injure an abnormally fragile vertebral artery along its course by partially tearing the inner artery wall (the intima).

A second objective was to estimate the regional incidence of these complications and to compare it with the very low incidences reported in the literature. Methods. During a 5-year period, practioners at a single group neurosurgical practice in Tulsa, Oklahoma, treated 22 patients, who were markedly worse during, or immediately after, CSMT. The details of these cases are reported. The 1995 US Government National Census was used to define the regional referral population for Tulsa. The pub- lished data regarding the incidence of serious CSMT-related complications and the rate of CSMTs undertaken nation- ally were used to estimate the expected number of CSMT-related complications in the authors’ region. The number (22 cases) reported in this series was used to estimate the actual regional incidence. Complications in the series included radiculopathy (21 cases), myelopathy (11 cases), Brown–Séquard syndrome (two cases), and vertebral artery (VA) occlusion (one case). Twenty-one patients underwent surgery. Poor outcomes were observed in three, outcome was unchanged in one, and 17 improved.

    Medical Malpractice Medical hiropractic is the most widely-used alternative medical treatment in the United States, with between 6 and 9 percent of the U.S. population seeing a chiropractor on at least an annual basis. Chiropractic has been Ctouted as a treatment for back and neck pain, headaches, repetitive strains, arthritis, generalized fatigue, vertigo, autism, infertility, pediatric acid refl ux, irritable bowel syndrome, varicose veins, and just about any other condition one can imagine. Indeed the founder of chiropractic, Daniel Palmer, claimed that ninety-fi ve percent of all human disease was caused by misalignment in the vertebra – called “subluxation” – that a chiropractor could cure by manually manipulating the spine. Th e primary way chiropractors treat these “subluxations” is through spinal adjustment. Th ough techniques vary, in general terms an “adjustment” involves the use of force (either manual or through the use of specialized equipment) to correct the alignment, motion, or function of parts of the spine. Like many types of health treatment, the spinal adjustments used by chiropractors have risks. Th ese risks can be relatively benign, like increased soreness or muscular strains. Th e risks can also be quite serious, such as ruptured discs or nerve compression. Abstract Background Despite widespread utilization of chiropractic treatment for various ailments, there is a paucity of documentation regarding intracerebral hemorrhage related to chiropractic trauma. Stroke has been reported from cervical manipulation, although with a suggested low incidence. Activator treatment, an instrument that produces a high-velocity, low-amplitude impact to the spine, is considered especially safe. There are no prior reports of intracerebral hemorrhage resulting from a chiroprac- tic activator treatment. Case Description A 75-year-old woman was admitted to the hospital with a history of headaches, visual difficulties on the right, and speech disturbance of relatively acute onset. CT scan showed a brain hemorrhage in an unusual location. Extensive evaluation was undertaken because this was thought to be a spontaneous event. No cause was found on imaging.
