POST: Consultant Breast Surgeon

SALARY: DIRECTORATE: Surgery

BASE: Princess of Hospital. Post Holder may also be required to provide services at other hospital sites.

1. Introduction This post will join the team of consultants responsible for the provision of general breast surgical services to the University Health Board in Cwm Taf Morgannwg. The consultant will be based at Princess of Wales Hospital (Bridgend) and Neath Port Talbot Hospital (Baglan Moors) but may also be required to undertake adhoc sessions across other sites within the organisation dependent on service demand.

The post will be required to share with Consultant colleagues in the responsibility of providing outpatient and inpatient services for the local population and in doing so foster excellent working relationships within and outside the General Breast Surgical Department, including medical, nursing, secretarial and support staff, General Practitioners and Management Teams.

The post will also work with the Directorate Management team to: assist in the strategic planning and modernisation of general surgical services; deliver internal and external targets for patient activity and achieve financial balance.

The new appointee will be encouraged to develop special interests complementary to the existing consultants.

The General Surgical service is provided by Consultants based at Princess of Wales Hospital, Prince Charles Hospital and the Royal Glamorgan Hospital. Additional outpatient clinics are also provided from the community hospitals.

Princess of Wales Hospital There are around 450 beds and provides a comprehensive range of in-patient, day case and outpatient facilities together with Accident and Emergency and Diagnostic facilities. There are a range of Critical Care services and six Operating Theatres. There is a dedicated 2 theatre Day Surgery Unit and Eye Unit. The majority of the clinical activity for Breast Surgery is undertaken at Neath Port Talbot, with dedicated ward beds, out-patient clinics and theatre time.

Royal Glamorgan Hospital There are around 570 beds and provides a comprehensive range of in-patient, day case and outpatient facilities together with Accident and Emergency and Diagnostic facilities. There are a range of Critical Care services and nine Operating Theatres.

Prince Charles Hospital Prince Charles Hospital has around 430 beds, and provides emergency and elective medical and surgical services for adults, adult Intensive Care and Coronary Care; consultant-led obstetrics services with Special Care Baby Unit, inpatient consultant-led paediatric medicine and has a busy new Accident and Emergency unit. The hospital also provides sub-regional oral and Maxillo facial services. There is also a dedicated Day Surgery Unit.

Clinical Duties  Provide professional and managerial supervision of junior medical staff

 Have continuing responsibility for the care of patients under his/her care: allowing for proper delegation to, and training of staff; ensuring that concise, agreed management plans are clearly displayed in the notes of patients with complex conditions; wards rounds are conducted to identify and assess problems and plan management of care; and be actively available during the working week so that medical and nursing staff can communicate and consult freely.

 Ensure that an appropriate model of care is adopted which recognises the involvement of the multi-disciplinary team, the patient and family.

 Ensure that the use of medical and nursing resources within the department are utilised to their maximum potential.

 Assist the multi-disciplinary team in the production of referenced, evidence based guidelines, which are dated, signed and reviewed at least bi-annually or more frequently in light of new evidence.   Work with the rest of the consultant team to provide strong professional leadership within the department, and be mentored in the delivery of a high quality service provided by well-trained staff within clear departmental policies and procedures.

 The post holder will be provided with a shared office facility and a secretary.

 Develop and maintain good communication with clinical colleagues across the Health Board, regarding the treatment of their patients, and the appropriate use of the department’s resources.

 Required to participate in the Health Board Risk Management process and in Clinical Audit and other clinical governance activities within the department, the directorate and the Health Board. The post holder will be positively encouraged to participate in continued professional research and development in line with Health Board, Commissioner and NHS priorities.

 Revalidation will be required to maintain full GMC Registration and revalidation should follow the GMC code of Good Medical Practice.

 In liaison with consultant colleagues have responsibility for the proper functioning of the department in line with the operational policy and strategic plan of the Health Board.

 Work to ensure that services within the department fulfil contractual performance indicators and work closely with directorate colleagues in identifying problem areas and planning and implementing appropriate solutions

Training  Undertake teaching, examination and accreditation duties to junior medical staff as required and contribute and participate in postgraduate and continuing medical education activity, locally and nationally.

 Participate in the departmental weekly teaching/audit sessions

Research  The post holder will be encouraged to initiate research projects

Management  The post holder will be required to co-operate with local management in the efficient running of services and is expected to share with consultant colleagues the medical contribution to management.

 There is a requirement to work within the financial and other constraints decided upon by the Health Board. Additional expenses of any kind must not be committed without the approval of the appropriate manager/budget holder.

 The post holder will be expected to comply with local and national employment and personnel policies and procedures in the management of employees of the Health Board.

 Subject to the Terms and Conditions of Service, the post holder will be expected to observe policies and procedures of the Health Board, drawn up in consultation with the professions where they involve clinical matters.

Additional information

 An annual job plan review will be provided.

 The post holder will be entitled to pro rata annual leave plus 2 additional statutory days as per the NHS Medical and Dental Staff (Wales) Handbook 1st. December 2003.

 A Study leave allowance is supported at the relevant rate. This will be pro rata.

 There is a comprehensive medical library at both the Royal Glamorgan General Hospital and at Prince Charles Hospital under the control of chartered librarians. Excellent computer laboratories are also available and are situated adjacent to the libraries.

The Health Board Profile:

Prince Charles Hospital is located 25 miles outside the Capital City of Cardiff with excellent transport links and is within easy access of the M4 motorway. Royal Glamorgan Hospital is 2 miles from junction 34 of the M4. Princess of Wales Hospital is 1 mile from junction 36 of the M4.

There are many areas of outstanding beauty nearby, to the north is the Brecon Beacons National Park, and to the south is the rolling countryside of Vale of Glamorgan and the South Wales Heritage Coast. Cardiff, the capital city of Wales, is a cosmopolitan city of many contrasts offering a wide variety of bars, restaurants, concert venues, theatres, art galleries as well as several shopping centres and Leisure Facilities. There is an excellent choice of housing within easy travelling times of the hospitals. There are established air, rail and sea links, which provide easy access for travel throughout the UK and beyond.

MAIN TERMS & CONDITIONS OF THE POST

 The terms and conditions of service including pay which apply to the post are determined by the UHB who at present take into account the national agreements reached by the General Whitley Council and The NHS Medical and Dental Staff (Wales) Handbook 1st. December 2003, and any changes to those conditions which the Secretary of State for Wales may authorise from time to time. Copies of these documents are available for you to inspect in the Personnel Directorate.

 The post is superannuable unless the appointee opts out of the scheme or is ineligible to join. Remuneration will be subject to deduction of superannuation contributions in accordance with the Department of Health and Social Security regulations.

 The post holder will be professionally accountable to the Medical Director, managerially accountable to the Clinical Director of the Surgical Directorate.

 The salary applicable is on the consultant pay scale and will be specified in the contract.

 The consultant appointed will be required to live within a 10 miles or 30 minutes distance by road of their base hospital unless specific approval for a greater distance is given by the UHB.

 Reimbursement of removal and associated expenses will be subject to the criteria laid down in the General Whitley Council Conditions of Service, together with the UHB policy.

 The Consultant will be required to maintain his/her private residence in contact with the public telephone service. Assistance can be given with the cost of installation and rental charges.

GENERAL INFORMATION

 Applicants who are related to any member of staff in a senior officer position at Cwm Taf Morgannwg UHB should clearly indicate in their application the name of the officer to whom related and indicate the nature of the relationship. A candidate deliberately concealing such a relationship would be disqualified.

 The UHB offers both married & single accommodation subject to availability.

 Travelling expenses will be reimbursed for only one pre-interview visit and only then to those candidates selected for interview or at the specific request of management, who will be granted travel and appropriate subsistence expenses on that occasion too.

JOB LIMITATIONS

Risk Management  To act as representative of the department as required.

 To contribute to the management of clinical risk, through the monitoring of clinical incidents, complaints and litigation.

 To participate in the achievement of Welsh Risk Pool Standards and be involved in regular audit of standards to ensure compliance.

 To monitor and advise on multi-disciplinary record keeping that meets both the UHB and Directorate Standards.

 To participate in clinical incident analysis, de-briefing and regular multi-disciplinary meetings e.g. case review to provide the opportunity for constructive, objective dialogue.

 To ensure personal practice and the practice of trainee medical staff complies with health and safety guidelines and that any deficiency in health and safety systems or documentation are reported to the manager of the area concerned and the Directorate Manager.

 To attend training for health and safety purposes when required.

Health And Safety Management  The post holder has a responsibility to attend Occupational Health for a health survey and vaccination following accidents and for monitoring as laid down by LHB policies.

 Along with other Consultant colleagues, to ensure that arrangements are in place for the management of the Directorate to fulfil its health and safety responsibilities to staff, patients and others.  This post involves “exposure prone procedures” as defined in PSM(93)12. Post Holders must therefore demonstrate either:-

(i) Proof of immunity to Hepatitis ‘B’ or (ii) If not immune to Hepatitis ‘B’ provide evidence that they are not Hepatitis ‘B’ e- antigen positive. This evidence must be current, i.e., have been obtained in the previous 6 months.
