Re: Important information for patients of Dr Jain – Askam GP Surgery

As you are a patient registered with Askam GP Surgery, we are writing to inform you of the decision made by the Primary Care Commissioning Committee on 18 February 2020 in relation to the future of the practice. The Committee made the decision that the current contract with Dr Jain will cease and the current registered list of patients be dispersed to other practices. The Committee papers are available, and the minutes of the Committee will also be published, on the CCG website.

The Committee did however support the Local Medical Committee’s (LMC) suggestion that a branch surgery may be a possible option for Askam and this is being investigated with local practices by the LMC, supported by the CCG. If agreements are reached for a branch surgery to be created in Askam we will contact you again to advise you of this. In this event you would still be required to register with another practice but would have the choice of registering with the local practice that had a branch surgery located in Askam.

Following feedback from patients in relation to local transport options the Committee acknowledged that the transport to alternative practices will be difficult for some patients. The CCG has committed to working with other partners within the local system to investigate community transport options. This may support the residents of with transport to a number of health and wellbeing services in neighbouring areas. If you are interested in supporting the investigation of a local community transport solution for Askam please do let the CCG know via [email protected].

There are 13 practices within a 7.5 mile radius of the current Askam Surgery site but the practices that you are able to register with will be based upon where you live. There are currently three GP practices that provide services to patients that live in Askam and two practices that provide services to those living in Ireleth. All of the 13 practices are accepting new patients. Additional details of the practices including their phone numbers are provided with this letter.

You will be required to register with an alternative GP Surgery before 30 June 2020.

The CCG, in response to a request made by patients at the Committee meeting, will be in touch with local practices to see if it would be possible for any that do not currently have Askam and Ireleth within their practice boundary to expand their boundaries so they could accept patients from these areas. If there is a change to any practice boundary that increases the number of practices that you can register with the CCG will write to you and let you know.

The CCG would like to assure you that we remain fully committed to investigating the branch surgery option and transport solutions and we will write to you again by the end of March to provide an update.

How to register with a new practice:

1. Go online - visit NHS website at www.nhs.uk/service-search and enter your postcode to find out more about the practices nearest you 2. If you are unable, or choose not, to access information online you can visit the Library in Askam who are displaying the maps of local practices and have all of the practice contact details. 3. You can also contact Healthwatch who are able to provide signposting information in relation to local surgeries and provide general advice on how to register with a GP practice. They can be reached on 0300 303 8567 or by visiting their website www.healthwatchcumbria.co.uk 4. Once you have chosen which practice to register with you will be required to complete a registration form for the new surgery. Some surgeries have registration forms available online and others need to be completed in person. 5. There is no requirement to contact Askam GP Surgery to advise that you are registering elsewhere as they will be notified by your new practice.

Staff at your chosen new practice will confirm whether you live within their catchment area and help you to register. Once you are registered with a new practice, your medical records will transfer automatically, so your new practice will have access to your full medical history.
