Septiembre de 1977 230a ESTUDIOS CROMATOGRAFICOS DE LIPIDOS EN g^IEL NORMAL. Resultados obtenidos de las 36 muestras de piel nor- mal de individuos sanos de diferente sexo y edad, y proceden- tes de distintas localizaciones. Para abreviar, en virtud del espacio, la gran canti- dad de cromatogramas (tres desarrollos para cada pieza), densi- togramas (uno para cada desarrollo) y tablas; exponemos a con- tinuación los resultados de las medias aritméticas (MA) y des- viaciones standard (DS), globales y en los diferentes grupos (sexos, edades y localizaciones) y subgrupos (varones y hem- bras; A, B, C y D para las edades; ya, b, c y d para las lo- calizaciones) según se expresan respectivamente en las tablas y, VI y VII. A continuación en la tabla VIII vienen indicados, para igual fracción lipídica, los subgrupos que han presentado diferencias esdisticamente significativas y el grado de signi- ficación de las mismas. Del estudio detallado de las referidas tablas nos llaman la atención los siguientes hechos: - El extracto lipídico de cada pieza (Ex L) represen- ta el 11% del peso del te-jido desecado. Dichos valores no varían según el sexo, pero sí según la edad del individuo y la locali- zación de donde procede la pieza, de forma que aumenta hasta casi el doble cuando menor es la edad y cuando procede de cara o extremidad superior. En todos los subgrupos la DS es semejan- te, lo que le da más valor a los hechos comentados. - El Escualeno (Esc) representa un 0% (en doce casos) o un 9% (en los 24 restantes) de los lípidos neutros de las pie- zas estudiadas, con una DS media de 6, valores que no se modi- fican en los diferentes grupos y subgrupos. Ocho de las doce muestras en que no se halló Escualeno procedían de muslo. - En el desarrollo de fosfoesfingolípidos las DS fue- ron bastante elevadas, excepto en la suma de fracciones LL y SF, con lo que las MA obtenidas pierden valor estadístico. Además el subgrupo A, en este desarrollo y en el anterior, no debe va- lorarse por incluir un solo caso. En el primer caso las diferencias se estable- cven fundamentalemnte en el desarrollo de ésteres de colesterol y en el segundo caso igualmente en este tipo de desarrollo y en la proporción lipídica del tejido desecado (Ex L) que varía prácticamente para cada localización. - En el resto de llpidos neutros: EC, AGL, CL, y FL, excepto los TG, se mantienen MA y DS bastante constantes, sin variaciones notables según sexo, edad, y localización. En el caso de los TG las MA fueron muy variables según dichos facto- res, con DS muy elevadas y diferentes, lo que imposibilita dar una cifra representativa de los TG en la piel normal. - Referente a los esteres de colesterol (ES, EM, ED y ETT) en siete casos no se hallaron (0%), correspondiendo a piezas sin relación de sexo, edad o localización procedente. En tres y catorce muestras más, tampoco se hallaron ED ni ETT, res- pectiveunente. La fracción esteres saturados (ES) es la que ofre- ce mayor constancia en su MA y su DS, en todos los grupos y sub- grupos EM y ED tienen el inconveniente de presentar una DS más elevada, y la particularidad de que su suma se mantiene constan- te en todos los casos (aproximadamente el 75%) de forma que cuan- do disminuye mucho una de las dos fracciones de su correspordien- te MA global (ejemplos subgrupos: B y a) aumenta proporcional- mente la otra. Secondary hypercholesterolemia due to a defect in phytosterol metabolism is far less common and may escape diagnosis during the work‐up of patients with dyslipi- demias. Here we report on two sisters with the rare, autosomal recessive condition, sitosterolemia. This disease is caused by mutations in a defective adenosine triphosphate‐binding cassette sterol excretion transporter, leading to highly elevated plant sterol concentrations in tissues and to a wide range of symp- toms. After a delayed diagnosis, treatment with a diet low in plant lipids plus ezetimibe to block the absorption of sterols corrected most of the clinical and biochemical signs of the disease. We followed the two patients for over 10 years and report their initial presentation and long‐term response to treatment. In addition to the classic variants, the subset of non-Langerhans cell histiocytoses comprises rare entities that have more recently been described. These last include both forms that affect only the skin or the skin and mucous membranes, and usually show a benign clinical behavior, and forms involving also internal organs, which may follow an aggressive course. The choice of oral treatment should be made on the basis of the patient’s condition, since none of them have proven particularly effective. Expectant management is justifiable as long as the lesions are limited to the skin.

Pediatric Dermatology, Bari Italy Summary The histiocytoses are diseases caused by the proliferation of histiocytes in various organs, including the skin; they have a very variable clinical spectrum and prognosis ranging from an often fatal multisystem involvement to a self-healing single lesion in a single organ. The classification of histiocytosis, which is based on the origin cell and malignancy potential, divides them into Langerhans cell histiocytosis (Class I), non-Langerhans histiocytosis (Class II) and malignant histiocytosis (Class III). In each of these classes numerous clini- cal forms have been described, but these forms according to some Authors only represent different developmental stages of the same disease. Letterer-Siwe disease is the most fre- quent form among Class I histiocytoses and juvenile xanthogranuloma and Rosai-Dorfman disease are the most frequent forms among Class II Histiocytoses. In Class I histiocytoses the histologic examination is not able to distinguish between severe and mild forms; the observation of the skin lesions can help in this differentiation.
