Purpose: COVID-19 vaccination began in Allegheny County and the US in late Dec 2020. For the first few months, vaccine demand far exceeded supply. We now have the opposite – more supply than demand. As the COVID-19 pandemic demonstrated, existing racial and ethnic inequities resulted in disparate exposure risks and outcomes. Since vaccines are the best method available to reduce the risk of COVID- 19, it is essential that everyone have equitable access to evidence-based information about the vaccine and to the vaccine itself. Therefore, the purpose of this county specific vaccination report, which will be updated monthly, is to examine our local data to determine if, where and for whom there are lower rates of vaccination to allow ACHD to work with community partners to target the ongoing vaccination response.
Methods
Beginning in mid-May 2021, the PA Department of Health began to share deidentified vaccination data at the individual level with local county and municipal health departments. The data are shared as weekly updates. The data source is the state immunization information system (SIIS). All COVID vaccine providers in the state are required to enter the vaccine data into SIIS, most commonly through an electronic transfer.
Data contain unique identification number, age at vaccine/s, date of vaccine/s, vaccine brand, ZIP code of residence, and clinic code (where the vaccine was administered).
Data are assessed to determine vaccination rates by race, age group, ethnicity, race and gender, ethnicity and gender, ZIP code, as well as ZIP code and race/ethnicity to determine areas and characteristics of the populations with fewer vaccinations per capita. If race, ethnicity, gender, or age were unknown, then they were excluded from analysis. All population data excludes individuals under the age of 10 who are not eligible for the COVID-19 vaccine.
To view weekly number of new hospitalizations and deaths, ACHD used data collected from the Pennsylvania National Electronic Disease Surveillance System (PA-NEDSS).
To create demographic rates, Allegheny County population data by demographic characteristics (e.g. race, age group, ethnicity, race and gender, as well as ethnicity and gender) were obtained via census.gov.

To create rates at the ZIP code level, population data was obtained via Social Explorer which provides American Community Survey data from the U.S. Census (5-Year 2019 Estimate, December 2020).

Vaccinations, Cases, New Hospitalizations, and Deaths by Week (through week of Aug 14)
The number of vaccines per week given to county residents peaked in late March 2021. Despite declining numbers since mid-April 2021, over 3,000 vaccines continue to be provided to Allegheny County residents each week. As of the week ending 8/14/21, 771,848 individuals were vaccinated in Allegheny County. ACHD estimates that 64% of Allegheny County eligible* residents are fully vaccinated with an additional 6% partially vaccinated.
* eligible is based on the population that is 10 years of age and older even though children aren’t eligible until age 12 and older. Ten and older is used because that is the available population level data; we do not have population level data for 12 and older.

As more people were vaccinated, both hospitalizations and deaths declined. However, as of the week ending 8/14/21, Allegheny County is experiencing another wave of cases, new hospitalizations, and deaths per week, likely due to the highly contagious and easily transmissible Delta variant. The CDC Nowcast model estimates that the Delta variant makes up 81.7% of specimens collected between 8/8/21 – 8/14/21 within HHS Region 3 (Delaware, District of Columbia, Maryland, Pennsylvania, West Virginia, and Virginia).

Vaccination rates increase by age group – from 41% among people aged 10-19 years to 86% among people 70 years and older with at least one dose of the vaccine. White people in Allegheny County are the highest vaccinated race at 61%, with White females representing the highest vaccinated race and gender group at 63%. Asian and Pacific Islanders are the lowest vaccinated race at 25% with AAPI males being the lowest vaccinated race and gender group at 23%. Among *ethnicity and gender, Hispanic females are the highest vaccinated ethnicity and gender group at 63%, while Not Hispanic males tend to be the lowest vaccinated group at 46%.

Neighborhoods in the City of Pittsburgh and the Mon Valley appear to be sites where the overall vaccination rates appear to be lowest in the county. In the City of Pittsburgh, AAPI vaccination rates per 10,000 are very low especially in ZIP codes with a relatively high population of AAPI individuals (15213, 15217, 15232, and 15210). In the Mon Valley, Black vaccination rates per 10,000 appear to be very low especially in ZIP codes 15110, 15132, 15104, and 15112. Suburban areas outside (or just within) the City of Pittsburgh appear to be locations with both relatively high populations of Hispanic individuals and also areas with Hispanic vaccination rates at or below 50% as reflected in ZIP codes 15226, 15068, 15227, 15206, 15210, and 15212. 9

Data Limitations
The data include 11% of people who have race classified as “Unknown,” as well as 30% of people who have ethnicity classified as “Unknown.” Also, compared to county-level data, “Multiple/Other” race is overrepresented—9.2% of all individuals vaccinated compared to 3.4% of Allegheny County’s population (American Community Survey 5-Year Estimate 2019). It is not known if people of “Multiple/Other” are misclassified or if they are underrepresented in U.S. Census data.
To assess whether unknown is equally distributed across races, a correlation was conducted at the ZIP code level. It was found that race described less than 5% of the variance of unknown race in each ZIP code, which is not statistically significant.
Currently, ACHD does not receive identifiable vaccine data from the PA Department of Health to reconcile the vaccine data. When ACHD receives identifiable vaccine dataset, ACHD will be able to improve the quality of the race and ethnicity data. In addition to not having an identifiable vaccine dataset, challenges remain for ACHD’s capability to determine vaccination status for recent cases, hospitalizations, and deaths. The PA Department of Health is currently making efforts to match vaccine data from SIIS with case data from PA-NEDSS, in which ACHD will eventually receive access. This will allow ACHD to more efficiently and accurately report breakthrough cases to the public.
