The Centre collaborates with the community to promote health and to prevent, treat and manage health problems. The mandate of Centre de santé Saint-Boniface is to provide primary health services in both official languages to the French-speaking population of Winnipeg and St. Boniface residents through an interdisciplinary team of bilingual professionals. The Centre also promotes the training of bilingual professionals. Values

Message from the Board Chair and the Executive Director

The Centre de santé Board of Directors and management are pleased to present the highlights of the progress made over the past year. This year, the Board of Directors directed the Executive Director to develop and implement an assessment framework to measure the results achieved in each of the four focuses of its 2017-2022 Strategic Plan. This new assessment framework examines three to four indicators per strategic focus that capture all relevant activities and data for the Board of Directors to enable it to measure the progress and results of its strategic plan. Collection tools were developed to compile, enter and analyze the necessary data and present them to the Board of Directors on a regular basis. The Board undertook an in-depth analysis of Centre de santé’s human resources needs. The objective was to review the following issues: • Conduct quantitative and qualitative monitoring of changes in Centre de santé’s operations; • Perform an analysis of management and administrative staffing, reviewing the tasks and responsibilities associated with each position; • Benchmark similar organizations in the health care system; • Propose organizational structure models to optimize human resources, with an analysis of the advantages and disadvantages of each model. One of the recommendations of this study was to separate the combined position (created in 2006) incorporating the roles of the Executive Director (ED) of Centre de santé and the Community Area Director (CAD) for St. Boniface and St. Vital for community health services provided by the Winnipeg Regional Health Authority (WRHA) and social services provided by the Manitoba Department of Families.

Over the years, the responsibilities of this combined role have increased significantly due to the following major changes: Centre de santé has almost tripled its services and staff since 2016; the My Health Team program has been added to the community area; the organizational structure of the Manitoba Department of Families has changed; and the Walk-In Connected Care services component was added to Centre de santé.

For these reasons, the Centre de santé Board of Directors, who had the WRHA’s approval, felt it was time to separate the combined ED / CAD positions. As a result, effective May 15, 2019, the responsibilities of the Centre de santé ED position will be filled by a full-time person working exclusively for the Centre. Because the responsibilities of the ED position

will be focused entirely on one organization, Centre de santé will be better able to implement its strategic plan and advance its services with a careful and strategic focus on the future.

The transformation of the Manitoba health care system raises important questions for Centre de santé. One of the priorities of this transformation is to simplify and improve the effectiveness of the approximately 250 service purchase agreements in the Manitoba health care system. The Centre de santé Executive Director sits on a consultation committee on developing a standard purchase agreement to be implemented for the 2019-2020 fiscal year. The committee is composed of representatives from Manitoba community health centres.

The Centre de santé Executive Director also sits on an expert committee set up and led by Santé en français in partnership with the Francophone Affairs Secretariat and Shared Health Manitoba for two projects, i.e. the incorporation of French-language services within the new Shared Health organization and the provincial strategy for bilingual human resources to improve French-language service delivery. These two projects are important to ensure that the francophone perspective is taken into account during provincial clinical and preventive services planning.

Centre de santé has joined with partner organizations to request that language requirements imposed on Canada’s nursing graduates not hinder bilingual recruitment efforts in the province. According to Centre de santé and these partner organizations, it is imperative that the College of Registered Nurses of Manitoba (CRNM) modify or adapt these language requirements to fully take into account the provincial legislative framework applicable to francophone affairs.

Centre de santé continues to work on designing a wellness centre whose implementation requires extensive analysis and research. We are moving forward with a feasibility study that will be conducted by the expert team of the Wellness Institute and the Chronic Disease Innovation Centre. This study will include a needs analysis as well as discussions with key Centre de santé partners and other important stakeholders.

The Board of Directors is proud of the activities and initiatives that Centre de santé staff have completed this year to achieve the deliverables set out in the Centre’s strategic plan.

Strategic Focus

The 2017-2022 Strategic Plan was developed through a dynamic process involving the Centre’s Board of Directors, founding members, partners and staff. The plan is based on four strategic focuses: leadership, equitable access, reputation for excellence and human resources.

1. Leadership: Centre de santé Saint-Boniface is a key leader within an integrated network of partners.

2. Equitable access: Winnipeg’s French-speaking population and St. Boniface residents have equitable access to health care services provided by Centre de santé Saint-Boniface.

3. Reputation for excellence: Centre de santé Saint-Boniface enjoys an excellent reputation as a recognized model for the delivery of high-quality primary and community services in both official languages.

4. Human resources: Centre de santé Saint-Boniface has a full complement of bilingual human resources.

Status Report Strategic Objective 1: Centre de santé Saint-Boniface is a key leader within an integrated network of partners.

With a view to integrating health and social services in the St. Boniface and St. Vital community areas, Centre de santé operations and the Centre’s operating plan have several elements in common with the WRHA’s 2018-2019 operating plan for community services. Our partnership with the St. Boniface / St. Vital My Health Team is fully continuing. The dietitians from Centre de santé and My Health Team, as well as a short-term treatment counsellor, provided support to a student in the Master of Social Work program to implement and execute her final project, “Eat Good, Feel Good.” This five-week bilingual program includes information on nutrition, healthy eating and navigating health services as well as cooking sessions. This program is for low-income populations or people living with chronic diseases.

The work of the Community Development Committee, which includes representatives from Centre de santé, Youville Centre, and the WRHA’s Population and Public Health, is continuing. The goal is to achieve a direct sharing and pooling of activities, to have

discussions about new learning and to identify activities or initiatives that could be implemented through a partnership.

In November 2018, Centre de santé collaborated with the Université de Saint-Boniface (USB) to submit a letter of intent to the Public Health Agency of Canada’s Mental Health Promotion Innovation Fund (MHP-IF). The proposed innovative initiative aims to use an integrated services approach to increase the availability and effectiveness of bilingual mental health and addiction services for French-speaking young adults attending a minority language postsecondary institution in Manitoba. Ultimately, we did not obtain that funding. However, USB submitted another application to the Official Languages Program in February 2019.

To broaden our awareness and community engagement capacity, Centre de santé is committed to forming new partnerships with organizations working with our target populations. We have established partnerships with the West Central Women’s Resource Centre and Centre René-Deleurme. These two organizations welcome several francophone newcomers, who are one of our target populations. This partnership allows us to create programming with them for the next few years to reach the French-speaking population outside St. Boniface. In addition, to speed up referrals for newcomers to Manitoba early in their settlement process, we partnered with the ENTRY program that Altered Minds Inc. provides for newly arrived immigrants in Manitoba. The goal of this partnership is to direct French-speaking newcomers to our services and programs.

We have also developed relationships with schools in the Division scolaire franco- manitobaine (DSFM), including École Christine-Lespérance, École Lacerte and Centre scolaire Léo-Rémillard, and with Éducatrices et éducateurs francophones du Manitoba (EFM) to raise awareness of Centre de santé and to better understand young francophones’ physical and psychological health needs.

We maintain strong ties with our existing key partners in order to deliver the following integration, collaboration and optimization projects to our five priority populations:

• Persons living with a disability: In June 2018, in conjunction with St. Amant, we set up a screening service for people with developmental and mobility disorders, so that they can obtain comprehensive primary health care when their treatment requires special equipment and an accessible environment. This service, which is delivered at Centre de santé by our nurse practitioners, has enabled us to provide 13 clients with more targeted screening. • Persons living with mental health problems and homeless persons: Members of our team took part in a tour of St. Boniface with St. Boniface Street Links staff to better understand the needs of homeless people and people with substance abuse

issues. In addition to providing client services and working closely with our partners, our team also provides students and trainees with support to lead initiatives targeting this population. In February-March 2019, USB student nurses in training organized the “Club de café” support group for people who use the Holy Cross Food Bank. Club de café participants acquire skills to improve their mental health and obtain information on health and social services and resources available in St. Boniface. • French-speaking newcomers to Winnipeg and newcomers living in St. Boniface: During the year, Centre de santé participated in several networking activities with NOWAN (Network of Organizations Working for War Affected Newcomers) and MANSO (Manitoba Association of Newcomer Serving Organizations) to raise awareness and learn about other organizations that help newcomers settle. In addition, Centre de santé participated in the 4th edition of “Jour-J”, a forum for young Franco-Manitobans aged 14 to 30 hosted by Accueil francophone. The theme of the day was the sexuality of young people. One of our primary care nurses facilitated an educational workshop on consent and healthy relationships, which attracted more than 60 youth. • Young children and adolescents: Our team has established good relationships with three high schools in St. Boniface: Collège Louis-Riel, Collège Béliveau and Nelson McIntyre Collegiate. In conjunction with these schools, we presented youth workshops on mental health, sexual health, self-esteem, body image and wellness, at health fairs. We identified challenges involved in organizing series of workshops in schools and discussed the possibility of providing support staff and counsellors with Train-the- Trainer training in the fall of 2019. In order not to limit our services to high school students, we also provided the ABCs of Relaxation workshop – a session on stress and anxiety management techniques – to USB students. • Seniors and persons living with a chronic illness: Centre de santé helped re-establish the Columbus Manor Residents Committee. Our Community Development Facilitator supports the Residents Committee by providing information on community partners. The facilitator also provides building owners with information on services and partnerships that may be useful to their residents. We regularly visit residences for people aged 55 and over to support seniors’ initiatives and host various workshops on health promotion (Columbus Manor, Windsor Park Place, Chez-Nous, 101 Marion, Foyer Vincent, Actionmarguerite St. Boniface and St. Vital, Place Eugénie and Club Oasis).

Strategic Objective 2: Winnipeg’s French-speaking population and St. Boniface residents have equitable access to health care services provided by Centre de santé Saint-Boniface.

Walk-In Connected Care Clinic

Efforts to increase awareness of our Walk-In Connected Care (WICC) services component through various communications have been successful. There have been 5,022 client visits to our Walk-In Connected Care Clinic since it opened in January 2018.

Nutrition Month

As part of Nutrition Month in March 2019, our dietitians undertook several initiatives to raise awareness of the importance of nutrition. A healthy diet not only helps us feel better mentally and physically, it also helps reduce the risk of disease in the long or medium term.

During Nutrition Month, Centre de santé provided snacks and healthy recipes to Centre de santé clients and Accès– Access Saint-Boniface clients.

The new Canada’s Food Guide, released in January 2019, has generated a wide variety of reactions because of its very different format. Some people found these changes confusing. One of our dietitians shared her expertise during an interview at Radio-Canada to make it easier for people to choose, eat and enjoy healthy foods by following this new guide.

This year, we continued to enhance our services and programs designed for our priority populations.

1. Persons living with mental health problems and homeless persons

Harm reduction (reducing the harmful effects of drug use)

Centre de santé is committed to implementing harm reduction strategies in its community.

Our staff worked with the Manitoba Association of Community Health (MACH) to obtain three-year funding from the Public Health Agency of Canada to hire a harm reduction specialist. This person will help educate and equip the employees of all community

organizations that are part of MACH (including Centre de santé) to enable them to work more effectively with this target population. The specialist will help MACH better understand the needs of communities and the organizational capacity of each of its member organizations (e.g. what services are already available, what services are not available). Under the coordination of this specialist, Centre de santé will perform a needs assessment and develop short-, medium- and long-term plans to better meet the needs identified in the community we serve.

In 2018, Manitoba Health, Seniors and Active Living opened two Rapid Access to Addictions Medicine Clinics (RAAM) in Winnipeg. These clinics provide assessment and counselling services, prescribe appropriate medication, and refer clients to community treatment programs and primary care physicians. Centre de santé and primary care clinics at the Access Centres in Winnipeg will attend to clients who will be referred to them through the rapid access clinics. Six Centre de santé health care providers (four physicians, a nurse practitioner and a pharmacist) have been trained to obtain certification for opioid replacement therapy (methadone). Centre de santé is committed to properly training its staff to better serve and support this population.

2. Young children and adolescents

Youth services and Youth hub

A remarkable step that our team took this year in laying the groundwork for youth services and the youth hub at Centre de santé involved building concrete partnerships with schools, parents and teens.

We have established a presence in three target schools in St. Boniface (Collège Louis-Riel, Collège Béliveau and Nelson McIntyre Collegiate) by organizing activities that respond to the interests and needs of youth.

o In conjunction with Collège Louis-Riel, we facilitated workshops (at two educational fairs for grade 7 and 8 students) on mental health, sexual health, self-esteem, body image and wellness. We also organized educational workshops and discussion sessions for students in grades 9 to 12 on sexual health and healthy relationships. o At Nelson McIntyre Collegiate, we delivered a mental health workshop for grade 11 students as well as interactive workshops on sexual health for students in grades 7 to 10. o One of our mental health counsellors presented a short version of Cognitive Behavioural Therapy (CBT) to youth at Collège Béliveau.

The term “youth” was chosen instead of “adolescents” for these services because it is more inclusive. Our goal is for youth in our community and in Winnipeg to feel comfortable and welcome.

Preparing for your pregnancy

Centre de santé has been offering a new bilingual workshop since February 2019: Preparing for your pregnancy. This one-of-a-kind workshop in Manitoba is for people who are thinking about getting pregnant in the next few months or are in the first trimester of pregnancy. The workshop is for their support person as well. It is facilitated by a dietitian and an exercise specialist and focuses on three main topics: nutrition, exercise and mental health. Participants receive advice on prenatal care and managing physical symptoms to ensure healthy infant development. The goal is to prepare participants for the physical and mental changes that occur during pregnancy.

3. Seniors and persons living with a chronic illness

Fall Prevention Clinic

Every quarter, Centre de santé provides the Fall Prevention Clinic for seniors and at-risk individuals who are clients of the Centre. Through this project, this year we assessed the risk of falling and made personalized recommendations to 13 clients. We hope to offer this clinic in the community in the spring of 2019 in partnership with Villa Aulneau.

Multidisciplinary Osteoporosis Program

Osteoporosis is a bone disease characterized by a deterioration of bone tissue, which causes the bones to become very thin and fragile over time. Although osteoporosis is more prevalent in the elderly, the disease can affect people of all ages. Osteoporosis is one of the most common chronic diseases in Manitoba.

Having found that there are few osteoporosis management and prevention programs in Winnipeg, Centre de santé is developing a multidisciplinary osteoporosis program. This four-week bilingual program will provide basic information on osteoporosis and risk factors for osteoporosis and on preventing and managing osteoporosis through nutrition, medication and exercise.

The program will be launched next year. We plan to present part of this program for the first time in May 2019 to Grouille ou rouille [Use it or lose it!] participants. Grouille ou rouille is a French-language activities day that promotes wellness for people 55 years of age and older.

Special guest visits

In December 2018 and March 2019, we had the pleasure of welcoming , Member of the Legislative Assembly for Fort Rouge and Leader of the of Manitoba, , MLA for Logan, , Leader of the and MLA for St. Boniface, Steven Fletcher, MLA for Assiniboia, Matt Allard, City Councillor for St. Boniface, and Markus Chambers, City Councillor for St. Norbert– Seine River, who visited Centre de santé and met with our team.

We are excited to have these opportunities to meet with community leaders and present our vision, our mission and especially the projects for our clients and the community. We hope to welcome other visitors in the near future.

Centre de santé’s 20th anniversary

July 26, 2019 will mark Centre de santé’s 20th anniversary. The Centre, whose mission is to provide both interdisciplinary primary care and programming that promotes wellness as well as disease and injury prevention, is proud of the close, harmonious relationship it has forged with St. Boniface’s French-speaking community and the wider Winnipeg French- speaking community since 1999.

To celebrate this anniversary, on Thursday, June 6, 2019, we will be organizing a run/walk and a health picnic for community members. The event is open to all. A portion of the proceeds from the nominal registration fees will go to Flavie-Laurent Centre, which helps the most vulnerable members of the community, some of whom are Centre de santé clients.

Shared Health

Following the announcement of the transformation of the health care system in Manitoba, Santé en français, in partnership with Shared Health Manitoba and the Francophone Affairs Secretariat, has developed two projects to integrate French-language services into Shared Health. These two projects funded by the Société Santé en français (through funding from Health Canada) are:

a. Integration of French-language services into the new Shared Health organization; and

b. Provincial strategy for bilingual human resources to improve French-language service delivery. The Centre de santé Executive Director sits on the Expert Committee, which is made up of representatives from designated bilingual or francophone regional health authorities, organizations, and the research community. The purpose of these projects is to analyze needs, develop strategies and create tools and protocols to support the French-language services strategies and recommendations that will be presented to the Transformation Leadership Team.

Centre de santé promotional video and Facebook page

To keep pace with ever-evolving digital and social media trends, Centre de santé has had to invest in new ways to communicate with its target audience in order to disseminate information, promote its activities and increase awareness of its various services.

A promotional video is one of the tools that we privilege because it allows to present Centre de santé in a personal and lively way. A new video that is currently in production will provide an overview of the Centre and its services. It will also relate some of our clients’ stories and experiences. We hope that the video will help the public appreciate the work that the Centre is doing for its clients and the community.

Knowing that social media is an important way to transmit information and communicate with the public, we launched our Facebook page this year. We post a variety of information on this page, including upcoming activities organized by Centre de santé and our partners, on health promotion, health resources and social services.

Strategic Objective 4: Centre de santé Saint-Boniface has a full complement of bilingual human resources.

We are always looking for opportunities to better support our staff in various training sessions by identifying the interests of our employees and assisting them in their career path or offering them training to build their skills in order to provide our priority populations with quality services.

The Living on the Edge: Taking a Look at Poverty simulation presented by United Way Winnipeg provided a good learning opportunity for our staff, our resident physicians and Accès–Access Saint-Boniface staff. This group experience provides a glimpse into the barriers facing families living in poverty in Winnipeg, who make up the majority of Access Centre clients. By putting ourselves in their shoes, we can better understand them and therefore better serve them.

Many of our health care providers have attended training sessions provided by the Crisis and Trauma Resource Institute (CRTI) to learn how to work more effectively with clients diagnosed with borderline personality disorder. They discussed a number of practical follow-up measures to improve service delivery to this population.

The Centre de santé’s management team supports the St. Boniface and St. Vital community area managers’ central objective of becoming familiar with the Truth and Reconciliation Commission of Canada’s final report. The management team participates in discussions on assigned reading every month to learn about and better understand the experiences of the Indigenous and Métis population. The knowledge gained can then be transferred to staff in the context of our objectives and future programs and services that may be related to the Indigenous and Métis population.

In the next fiscal year, Centre de santé will provide four key training courses to staff on the following topics:

o Cultural competence to work effectively in cross-cultural situations o Teaching sexuality: facilitation techniques for youth service providers o Trauma and stigma: tools and approaches for working with young people o Drug dependency and harm reduction

In April 2018, Centre de santé organized an open house to recruit primary care assistants. The invitation was primarily for bilingual students (French and English) graduating from high school in June 2018. The candidates had the opportunity to meet our team and attend an on-site interview.

Centre de santé recognizes the importance of practicums for students in professional and technical health care occupations. This year, under a new agreement with Red River College, we hosted two student nurses in training from the college.

We were also able to hire a kinesiology student for 12 weeks in the summer of 2018 thanks to two grants, one from Service Canada – Canada Summer Jobs and one from Young Canada Works. The student worked closely with our exercise specialist to develop various projects, including one on osteoporosis and another on physical activity during pregnancy.

In summary, this year we hosted a total of 33 students: 11 medical students from the , 5 medical students from the University of Ottawa, 1 kinesiology student from the Université de Sherbrooke, 7 nursing students from the Université de Saint- Boniface, 2 nursing students from Red River College and 7 medical residents from the University of Manitoba.

We are proud of our staff’s work and the success they have achieved. One of our physicians, Dr. José François, received the 2018 Margaret-Teresa-McDonell Award. This award was created in 2009 by Santé en français in honour of Margaret Teresa McDonell – named Sister Sainte-Thérèse and also known as Sister Doctor – who was a pioneer of French-language services in St. Boniface. Like Sister Doctor, Dr. François is recognized for his commitment to providing high-quality French-language health and social services. Thanks to Dr. François’ many initiatives, there are more opportunities for training in French.
