Appendix F: Information for hospitals Information for Hospitals considering participating in the Newcastle University Medicine (NUMed) General Medical Council Registration Programme Newcastle University Medical School was originally founded in 1843. It established an international branch campus (Newcastle University Medicine Malaysia; NUMEd) at Educity, Iskandar Puteri in in 2009 and produced its first graduates in 2014. NUMed delivers two undergraduate degrees: in Medicine (MBBS) and Biomedical Sciences (BSc). Students follow the same curriculum for both these degrees as students in the UK. The MBBS degree is recognised both by the Malaysian Medical Council and also by the UK General Medical Council for registration purposes. Consequently, NUMed MBBS graduates are eligible for provisional registration with both bodies. By a unique arrangement between Newcastle University, Health Education England North East (HEE NE; part of the UK NHS educational arm), the General Medical Council and the Malaysian Ministry of Health it has been agreed that NUMed graduates successfully completing one year of House Officer training within the Malaysian health service may be eligible for Full UK Registration with the GMC. The requirements for this are:

• The training must be within a hospital which has been approved by the Postgraduate Dean at HEE NE, who is responsible for ensuring GMC standards have been met. Approval is awarded by a process whereby the Postgraduate Dean visits the hospital prior to placement of any potential candidates. • Within the identified hospital, there must be a willingness of the House Officer committee and other doctors to work with both NUMEd and HEE NE to assess progress through the House Officer programme. • House Officers are required to submit log books and other evidence showing that they have successfully completed the requirements. This evidence is then considered by a convened panel who will assess the NUMEd graduate against the required outcomes.

Whilst taking part in the Full Registration programme may not have an impact on the trainee doctor if they solely continue their medical practice within Malaysia, it may convey some benefit to them if they seek further training or employment in the UK or abroad.

For the hospitals involved, the intention of the programme is to introduce as little disruption as possible. At present the hospitals with approval for this purpose are: 1. Hospital Sultan Ismail, Johor Bahru, Johor 2. Hospital Sultanah Aminah, Johor Bahru, Johor 3. Enche' Besar Hajjah Khalsom Hospital, Kluang, Johor 4. General Hospital, , Sarawak 5. Queen Elizabeth Hospital, Kota Kinabalu, 6. General Hospital, Georgetown, Penang 7. University of Malaya Medical Centre, 8. Hospital Sibu, Sarawak

The requirements that the House Officers must satisfy are laid out in the document: “Requirements for NUMed graduates working in Malaysia wishing to obtain Full UK GMC Registration” which is attached as an appendix. Trainees will apply for placement in one of the hospitals using the e houseman system and will follow the routine training undertaken by all other House Officers in the hospital, with their progress through training being managed in line with standard practice. To gain Full GMC Registration the graduates will need to submit evidence that they have successfully completed three House Officer placements. This will be done through a process known as an ARCP (Annual Review of Competence Progression) modelled on that used for medical trainees in the UK. The date for this will usually be about 16 months after the HO first take up their posts (to allow for any extensions in their training). The House Officers will be required to provide scanned copies of signed off logbooks for the three placements, together with some other evidence (see appendix). This evidence will be reviewed against the standards set by the GMC by a panel consisting of the Programme Director at NUMed and NE HEE staff representing their Foundation School. If judged to be satisfactory, the Postgraduate Dean from HEE NE will issue a Certificate of Experience allowing them to apply for Full Registration with the GMC. The Programme Director based at NUMed, but on behalf of the Postgraduate Dean, will visit the hospitals at least once a year. The purpose of the visit is to meet the House Officers and talk to the senior staff at the hospital involved in House Officer training to ensure that the posts continue to meet the standards required by the GMC and discuss ways in which the process can be developed and enhanced. To date, 29 NUMEd graduates have achieved Full GMC Registration through this process. It is anticipated that the programme will continue to develop as the numbers of NUMed graduates increases over the next few years.

Appendix: Requirements for NUMed graduates working in Malaysia wishing to obtain Full UK GMC registration

1. Graduates (hereafter “trainees”) must be appointed to HO posts in one of the eight approved hospitals: • Hospital Sultan Ismail, Johor Bahru, Johor • Hospital Sultanah Aminah, Johor Bahru, Johor • Enche' Besar Hajjah Khalsom Hospital, Kluang, Johor • , Kuching, Sarawak • Queen Elizabeth Hospital, Kota Kinabalu, Sabah • , Georgetown, Penang • University of Malaya Medical Centre, Kuala Lumpur • Hospital Sibu, Sarawak

2. Trainees must obtain provisional registration with the GMC UK prior to taking up their posts 3. Trainees must follow the HO training programme as specified by the Malaysian Ministry of Health 4. At the end of every post, trainees must make and retain a scanned copy of their completed and signed off logbook to demonstrate satisfactory progress 5. Trainees must inform the NUMed Programme Director for GMC Registration about any periods of unanticipated absence (eg sickness) or extensions to their training as soon as they become aware of them/are able to do so. Extensions to training will not prevent trainees proceeding to full registration, but the Programme Director needs to know of these to be able to make adjustments to the dates for final completion of the programme and to be able to offer appropriate advice and support 6. Trainees must maintain contact (by providing a valid email address and handphone number which they agree to monitor regularly) with the NUMed Programme Director and respond expeditiously to requests for information from them about individual progress and their experience of training 7. Trainees must participate in the annual on line quality monitoring survey of their experience of training

8. Trainees must complete 12 months training as a HO and submit scanned/electronic copies of the following to the Programme Director for consideration at the ARCP (Annual Review of Competence Progression) panel, the date of which will be pre determined: a. three completed and signed off logbooks b. additional procedures form c. three reflective accounts each of 1000 words describing: • Describe how an experience in the workplace, or feedback about your performance helped you to identify a personal learning need, how you addressed this and how this contributed to your development as a doctor. • Describe how an experience in the workplace helped you understand your role in ensuring patient safety or improving the quality of services delivered and how the insights from this could be implemented in practice. • Describe an experience of health promotion as a house officer (this may be things you have done personally or programmes run within the hospital) and how you think this could be developed in your future practice. d. Evidence of completion of training in equality and diversity (presently undertaken as an online module supported by Newcastle University)

9. If the ARCP panel decides trainees have achieved all the required outcomes as specified by the GMC (GMC: Outcomes for provisionally registered doctors ), a certificate of experience (CoE) will be issued and the trainees can then either apply for full GMC registration at that point or suspend their GMC registration until they need to apply for full registration at a later date

Agreement to participate in the GMC registration programme implies acceptance of these requirements.
