Bennett Library Simon Fraser University BurnabY,BC,Canada Lil:51 reVISlon- Summel 2007 Abstract Martin Luther King said that "of all the forms of inequality, injustice in health care is the most shocking and inhumane." Today, despite the overwhelming quantity of attention and resources given towards global health, social inequity has never been worse as many in the global South remain perilously marginalized from the receipt of sustainable primary-care services. Amidst this backdrop Cuba developed the Latin American School of Medicine (ELAM) as a response to the global South's lagging supply of human resources for health. The school has received 11,500 students from 29 different countries, including the United States. Every student receives a free, six-year medical education as part of a moral commitment to practice their skills in areas where they are most needed. This dissertation explores how ELAM is building capacity to meet the needs of communities in Ecuador, a country that has endured structural adjustments to its public health sectors. It is about sharing the lived experience of certain ELAM graduates to understand if they are able to reduce health-care inequity in their communities despite personal and structural challenges. This is best understood in how graduates practice community-oriented primary care (COPC) within their communities. This study finds that ELAM's institutional ethics are counter hegemonic to dominant neoliberal tendencies in health-care service provision, but that only with appropriate social and political support will the ethics and skills of these graduates scale up into positive changes in community health. The study conceptualizes ELAM as an alternative-development project grounded in a tradition of Cuban medical internationalism. It offers insight into the program of III study, discusses the nature of public health care in Ecuador, and then explores the professional experiences of several Ecuadorian ELAM graduates. This dissertation serves as a preliminary step in understanding how ELAM impacts the accessibility of vulnerable populations to health-care services in Ecuador. It demonstrates the strengths and challenges of ELAM graduates in seeking to strengthen human resources for health. The ELAM story shows how investing in people can work towards overcoming neoliberal health-care hegemony, which has done a great deal to promote health-care inequality rather than ensure social equity. IV v This one is for Uncle Don, who always fanned the flames. VI - The Delivery - After three days oflabour, the baby still hadn't been born. "He's stuck. The little guy's stuck, " the man said. He had come from a remote farm in the country. The doctor went with him. Valise in hand, the doctor walked under the noonday sun, toward the horizon, into that desolate world where every- thing seems born ofaccursed fate. When he got there he understood. Later, he told Gloria Galvin: "The woman was at death 's door but was still panting and sweating, and her eyes were wide open. I had no experience with situations like that. I was shaking; I hadn't the faintest idea what to do. And then, as I drew back the blanket, I saw a tiny arm sticking outfrom between the woman's spread legs. " The doctor realized that the man had been pulling on it. The little arm was rubbed raw and lifeless: a flap ofskin, black with dried blood. And the doctor thought: There is nothing to be done. And yet, for whatever reason, he caressed the arm. He rubbed the inert limb with his index finger, and when he got to the hand, it suddenly closed, clutching his finger for dear life. Then the doctor asked for boiling water and rolled up his sleeves. On April 29, 2001, while swimming at Playa Santa Maria, just east of Havana, I missed that message and ran, full speed, into an angry sea. A wave came up on me, threw my feet in the air, and drove my head into the ground shredding my shoulder to pieces. A bloody mess, I dragged myself out of the water, and collapsed on the sand. Within no time two Cubans put me in the back of their old American car, and we drove off to Calixto Garcia Hospital in Vedado. I kept telling them "jNecesisto mi carta de seguridad!" I need my insurance card. "Never mind that," the driver said. We arrived at the hospital. A doctor was lounging by the main entrance reading the daily issue of Granma. He took me, along with another unfortunate Canadian, who sliced open his foot having treaded over a broken beer bottle, into the surgery and immediately two nurses and another doctor appeared. They put the man with the wounded foot on the table, cleaned him up and then, with cat-gut stitches, sowed him up. Then they turned to me. They cleaned my shoulder and treated it with generous mounds ofdisinfectant, iodine, and then some skin cream. We were both treated and out the door in less than 25 minutes. I had $50 USD on me, and I offered the doctors and the nurses the full amount.

    Frontier Vol. 44, No. 26, January 8-14, 2012 CUBA Education and Revolution Ricardo Alarcon de Quesada IN 1795, FATHER JOSE AGUSTIN Caballero presented the first project for the creation of a system of public education for all the inhabitants of the island of Cuba. It was a visionary idea, but impossible to carry out at that time. The island was a colonial possession of the Spanish Crown, and most of the population was subjected to slavery or made up of Mestizos and freed blacks, the victims of segregation and racial discrimination. Education, within the reach of a very small minority, was confined within the strict canons of scholastic philosophy. Father Caballero was profoundly critical of that philosophy and of the pedagogy springing from it. This would be the birth of an intellectual movement having decisive importance for the history of Cuba, a movement that would reach its pinnacle with another Catholic priest, Felix Varela, who was Caballero's disciple and the first Cuban intellectual who fought for national independence and the abolition of slavery. It is intriguing that it was in Cuba, which, with Puerto Rico, was the last of the Spanish American colonies, where the most solid and deep-rooted questioning of the political system, struggling to keep both islands under its control, occurred. The melding of radical thinking with aspirations for the emancipation of slaves and other excluded sectors of the population took shape on October 10, 1868, the start of the first war for independence and a profound social revolution. Public education was a refuge for Cuban patriotism throughout the first half of the twentieth century.

The group comprises of persons from varying areas of civil society who have seen the need to engage in this effort. We take time out to contribute to this global process in promoting positive happening in Cuba. Since the triumph of the Cuban revolution in January 1959, Cuba has been doggedly engaged on a mission of achieving true sovereignty, equality for all it citizens, self determination and promotion of international goodwill and corporation in critical areas of human development on a global scale. Cuba’s pursuit in achieving these goals has garnered it a global reputation as a champion in its effort to attain greater enjoyment of all human rights and freedom for all on an equal basis, without discrimination, in spite of a crippling five decade old economic, commercial and financial blockade imposed and maintained by successive United States administrations. The people of Cuba and its government have been able to achieve significant advancements in promoting and maintaining these goals within the context of US hostile policies and actions and in deteriorating global economic conditions.

    COD # 7 The Contribution of South-South Cooperation to the Strengthening of Democratic Developmental States: Laying a Research Agenda David González López Silvio Baró Herrera About the South-South Tri-Continental Collaborative Programme The South-South Tri-continental Programme is a scholarly collaboration for Research, Training, Publishing, and Dis- semination, between the Council for the Development of Social Science Research in Africa ( CODESRIA ); the Asian Political and International Studies Association ( APISA ); and the Latin American Council of Social Science (CLACSO ). The Programme was established as a reaction to the need, identified by scholars in the South, to reori- ent theoretical and methodological frameworks of the dominant development discourses; and to improve the or- ganization of Southern research infrastructures. The Programme aims at reviving cooperation and collaboration among scholars of the global South working in the broad field of the social sciences. The collaboration was entered into with the specific aim of sustaining knowledge exchange between scholars on the three continents as a long- term initiative. At the core of this collaboration are the objectives of deepening intra-South networking contributing a South perspective towards the transformation of the Social Sciences on a global scale producing alternative theoretical and methodological approaches of knowledge building Networking and dialoguing take place in the different International Comparative Seminars that the partners set up annually on a rotational basis.     Tackling Mal aria Interventions available today could lead to decisive gains in prevention and treatment—if only the world would apply them By Claire Panosian Dunavan Long ago in the Gambia, West Africa, a two-year-old boy named Ebrahim almost died of malaria. Decades later Dr. Ebrahim Samba is still reminded of the fact when he looks in a mirror. That is because his mother—who had already buried several children by the time he got sick—scored his face in a last-ditch effort to save his life. The boy not only survived but eventually became one of the most well-known leaders in Africa: Regional Director of the World Health Organization. Needless to say, scarification is not what rescued Ebrahim Samba. The question is, What did? Was it the particular strain of parasite in his blood that day, his individual genetic or immunological makeup, his nutritional state? After centuries of fighting ma- laria—and conquering it in much of the world—it is amazing what we still do not know about the ancient scourge, including what determines life and death in severely ill chil- dren in its clutches. Despite such lingering questions, however, today we stand on the threshold of hope. Investigators are studying malaria survivors and tracking many other leads in efforts to develop vaccines. Most important, proven weapons—princi- pally, insecticide-treated bed nets, other antimosquito strategies, and new combination drugs featuring an ancient Chinese herb—are moving to the front lines. In the coming years the world will need all the malaria weapons it can muster.
