Please review this guide carefully to consider the best options for you and your family.

PAY PREMIUMS WITH PRE-TAX DOLLARS GPA participates in an IRS Section 125 plan that allows employees to pay for certain expenses with pretax dollars. Our plan allows you to pay for your medical, dental, vision, and FSA insurance deductions with pre-tax dollars through payroll deductions. Because you have the benefit of pre-tax deductions, you cannot cancel your coverage during the year unless you experience a qualifying Event, or during GPA’s Open Enrollment or the Affordable Care Act (ACA) Open Enrollment. Information regarding Qualifying Events will be explained later in this guide.

SUMMARY PLAN DOCUMENTS (SPD) The Summary Plan Documents (SPD) provide detailed information concerning all of our full-time benefits. You can locate all SPDs for your review at the ADP PORTAL or Docushare. This Benefits Guide provides an overview of benefits available to our participants and their families. In the event of a discrepancy between the information presented in the Benefits Guide and the Summary Plan Document, the official Summary Plan Document will prevail. GPA reserves the right to change, amend or end any benefits at any time for any reason.

Employee Guide to Enroll in Benefits with THEbenefitsHUB

THEbenefitHUB gives you access to your benefit24 hours a day,7 days a week from anywhere that you have internet access. This guide is meant to see you through the simple enrollment process page-by-page, taking you through your enrollment screens and providing information on how to efficiently complete your enrollment walk through.

LOGGING IN Employee Usage Agreement: The Employee Usage Agreement is displayed when you login to the system as an employee. Read this section carefully as it contains disclaimer information and requires an “Electronic Signature.” By clicking the Continue button, you are agreeing to the terms.

• Change Password: When logging in for the first time, you will be prompted to update your password following your company’s password policy. Once your new password has been set, click the Save & Continue button.

7 DEMOGRAPHIC IN FORM ATI ON The Employee Information Entry process requires you to enter demographic information. You will need to review any pre- filled information for accuracy. Complete new or missing information and click on the Save & Continue button when you are ready to proceed to the next step.

Please Note: All fields in BOLD are required. • Personal Information: Enter an email address if you have one. If you need to use the Forgot Password link on the Login page, the system will deliver your new login credentials to this email address. • Emergency Information: Enter an emergency contact and the preferred contact method. • Dependent Information: To add a dependent, click on the icon. To edit an existing dependent, click on the icon or the name of the dependent listed. Click on the Save button after successfully adding information for each dependent. Please make sure to indicate if your child is a full-time student and/or claimed on your tax return as this could affect eligibility on some benefit plans. • To revisit any of the sections mentioned select the Back button to return to the previous section.

BENEFITS ENROLLMENT When you have completely entered all of your personal and dependent information, you will begin your online enrollment for any of the benefits in which you are eligible. Each benefit will appear on individual pages for your review. Choose your election and then click the button to proceed to the next benefit.

• View Benefit Descriptions: To view, click on the View Plan Outline of Benefit link or the icon next to the name of the plan you would like to review. This shows a plan summary and any available links or documentation related to this plan.

• View Plan Cost: Click on the checkbox next to each eligible family member or choose the coverage level you would like. The cost will automatically appear in the box to the right of the members’ names. Additionally, the “Election Summary” box will be updated as coverage adjustments are made. • View Total Plan Cost: While selecting plans, the cost will automatically adjust in the “Election Summary” box in response to your selections.

• Forms: One or more of your Benefit Plans may require a paper form to be submitted with the Insurance Carrier. If this is the case, THEbenefitsHUB will prompt you to print the necessary forms during your online enrollment session. • View Important Plan Information: Your benefits administrator will spotlight the importance of specific features in a plan or add any disclaimers that may be necessary in the “Plan Information” section. You may expand/collapse this information by clicking anywhere on the section. • Product Summary Video: Videos are placed throughout the benefit election process. You can access product videos that explain the purpose, function and importance by clicking on the icon when available.

BENEFICIARY INFORMATION Beneficiaries are required. You will need to choose a beneficiary for each applicable plan.

8 CONSOLIDATED ENROLLMENT FORM This form signals the end of your enrollment walkthrough and will display information from each of the sections listed a b o v e , including personal and enrollment information. You may make changes to anything that is incorrect by clicking on the Benefit Plan name. Once you are finished with the enrollment process, you will be sent to the “Employee Menu” where you may make changes. (See Employee Menu section) When you have completed your benefit selections, click the Main Menu button and you will be redirected to the Employee Menu screen.

EMPLOYEE MENU After you have completed your enrollment in the system, you will see the following Employee Menu icons:

Personal Information: You can access and edit information by selecting the menu items under Personal Information. This section will also allow you to change your Password.

Dependent Information: You can access and edit information for Dependents in this section. Make sure the HR Department knows of any changes made as this may change eligibility status or give an opportunity to change enrollment in certain benefits!

Benefit Plan Information: You can access and view benefits in this section. You will not be able to change benefit elections unless it is during your annual enrollment period. See a quick overview of all your elected information on the Consolidated Enrollment Form.

NAVIGATION AND INFORMATION ENTRY TIPS…

Below are tips to help you familiarize yourself with THEbenefitsHUB: • Help: If you need assistance during the enrollment process, select HELP located at the upper right corner of the screen. • Back & Forth: Please do not use the web browser’s “back” or “forward” arrows while in the system. Use the navigation buttons in THEbenefitsHUB instead: Back , Sign & Continue • Required Information: As noted on each screen, the BOLD items are required to allow continuation to the next page. The more information entered, the better the system will work for you; but you may skip non-bolded items if they do not apply. • Moving On: When each election page is complete, go to the bottom of the page and select the Sign & Continue • Unable To Finish? 9 Eligibility

WAITING PERIOD You have 90 days from your date of hire to enroll for benefits. Your medical and dental coverage will become effective on the 91st day of employment.

10 Qualifying Events For Changes To Your Benefits

Qualifying Events allow you to make changes to your elections before or after Open Enrollment as long as changes are submitted within 30 days of the Qualifying Event.

GPA Holding, Inc. believes this Employee Health Benefit Plan is a “grandfathered health plan” under the Patient Protection and Affordable Care Act (the Affordable Care Act). As permitted by the Affordable Care Act, a grandfathered health plan can preserve certain basic health coverage that was already in effect when that law was enacted. Being a grandfathered health plan means that your plan may not include certain consumer protections of the Affordable Care Act that apply to other plans, for example, the requirement for the provision of preventive health services without any cost sharing. However, grandfathered health plans must comply with certain other consumer protections in the Affordable Care Act, for example, the elimination of lifetime limits on Essential Health Benefits.

Long-Term Care – UNUM Life Insurance Company

45 Short-Term Disability Benefit GPA has a short term disability program available at no additional cost for all full-time employees who have completed 12 months of continuous employment. Benefits are provided for loss of wages due to disability caused by accident and/or illness or pregnancy. An employee is considered totally disabled when he or she is unable to perform the material duties of his or her occupation, solely because of sickness or accidental injury.

Benefits will be paid as follows: week 1 there will be no benefit, weeks 2-26 you will receive 70% of your salary. Benefits begin on the 8th day of accident or illness and will pay for twenty-six (26) weeks per occurrence. The correct paperwork must be submitted and approved before any payments will be made. Employees must notify their Director and the Human Resource Department as soon as they know they will be out of work. See the Short-Term Disability policy on ADP for more information.

Family Medical Leave Act Benefit Full time employees that work 30 hours or more are eligible for Family Medical Leave (FMLA) Coverage. FMLA is job protection Leave for a maximum of 12 weeks. You must be under the continuous care of a physician. In order to qualify for FMLA, you must have worked with the company for a year or at least 1,250 hours during the preceding 12 months. Employees requesting FMLA must provide a verbal or written notice of the need for the leave to their Director/Supervisor and the HR Department to be given applicable forms. When employees are approved for FMLA for any reason, they are required to use their PTO balance before any additional benefits or short-term disability would be paid out. After the depletion of PTO, FMLA will start to be recorded. Employees on FMLA, they will not accrue any PTO.

GPA will use a rolling 12-month period measured forward from the date of any FMLA usage to determine how much FMLA remains or when it may be used again. Medical and Dental benefits will be terminated once reaching the maximum 12 weeks of job protection.

BENEFIT PAYMENT

Your Initial Benefit provides a lump-sum payment upon the first diagnosis of a Covered Condition. MetLife Critical Illness Insurance will pay 25% of the Initial Benefit Amount when a covered person is diagnosed with one of the 22 Listed Conditions. A Covered Person may only receive one benefit payment for one Listed Condition in his/her lifetime. In most states there is a preexisting condition limitation. 71 Additional Employee Benefits Paid Time Off(PTO)

It is the policy of the Company to grant paid time off to full-time employees. PTO is to be used for personal days, absences due to sickness, mental health days and vacation. No more than 40 hours may be taken at a time.

The established paid time off calendar year runs January 1 through December 31 each year. The amount of paid time off hours that are front loaded are based on the employee’s length of service (calculated based on seniority date, which is the first of the next year following your hire date) and on the time actually worked. If an employee exhausts all PTO for the year (including any approved leave of absence), they are not eligible to take any additional time off unless unpaid. By taking any unpaid time off they are not eligible for any payments made under the Incentive Compensation Plan that may be paid in December.

Full-time employees will be front loaded paid time off according to the following schedule:

72 Employees should submit paid time off plans to their Supervisor at least four (4) weeks in advance of the requested date. Management reserves the right to designate when some or all time off must be taken. All time for the last quarter of the year must be scheduled by October1st and no more than 40 hours may be scheduled for the last quarter of the year. All time off requests submitted after October 1 will be at the discretion of the Director/VP for approval.

Employees on a leave of absence, other than a military leave of absence, are required to use 7 days of their paid time off as part of the leave. When employment is terminated, employees will receive any unused earned time off accrued at the time of termination, if two (2) weeks’ notice is given. Although, paid time off is front loaded, at the time of termination you will only be given hours that have been accrued up to the termination date.

It is the policy of the Company to provide educational assistance to its employees in accordance with the guidelines established below.

To be eligible for educational assistance, employees must have regular full-time status and at least one (1) year of service with the Company.

Eligible employees may be reimbursed only for courses of study that the Company determines are directly related to the employee’s present job or that will enhance the employee’s potential for advancement to a position within the Company and to which the individual has a reasonable expectation of attaining. In addition, only courses that are offered by pre- approved institutions of learning will be eligible for reimbursement.

Employees who want educational assistance must submit, in writing prior to enrollment, course(s) to be taken to their Director for approval. If the Director approves, the Director shall forward the request to the Human Resources Director and she will submit the request to the appropriate Officer.
