IMPORTANT SPRING SPORTS INFORMATION! Spring sports are scheduled to begin on 5/3/21. This includes Varsity Unified Basketball and Varsity & JV: Baseball, Softball, Boys & Girls Lacrosse, Boys Tennis, Track & Field and Wrestling. Prior to participating on a team in the Lockport City School District, all athletes must provide the Athletic Department with current address, emergency contact, medical alert and health history information. To expedite this process our Athletic Trainer uses an online data entry system called SportsWareOnLine. A parent/guardian must join and complete all forms. You can find step by step instructions at www.lockportschools.org/physical. Please note: ● The athletic code can be found on the top of page 2 ● Parents/guardians please make sure to click on “register as a parent” ● There are 4 additional forms that must be completed on a computer or tablet. Please make sure to electronically sign and submit them. ● When filling out the forms please indicate which sport the student will be trying out for in “Sport 3”. This is how the sport signups will be done. Once you have completed this process the next step will be to schedule a sports physical appointment. In order to do that you must contact the Athletic Department at 716-478-4474 or email [email protected]. Spring sports physicals will be held on Saturday, 4/17/21 at Lockport High School. To participate in any sport you MUST have a sports physical! Any 7th or 8th grader interested in trying out for these sports must pass the Athletic Placement Process. We will be holding the APP test on Thursday, 4/29/21 at North Park. You must also schedule an appointment for this. In addition, you must sign up by indicating it in Sport 3 in the program.
