Date of Application: Name of Authority: Signature of Approving Authority: Please Note: To receive a Veteran plate, this application form must be certified by an authorized agency and submitted, with the required registration application and fee, to your local Access P.E.I. office. The registration fee for a veteran plate will be the standard fee associated with the class and weight of vehicle being registered, plus an addition $5 fee at the time of issuance. Application for a Veteran Plate Terms and Conditions 1. Definition For the purpose of this application a Veteran means any person who has honourably: • Served (may still be serving) in the Canadian Armed Forces, or the armed forces of another Commonwealth country or of a wartime ally of Canada or another Commonwealth country; • Served in the Merchant Navy or Ferry Command during wartime; • Served in a theater of war as a member of the Canadian Armed Forces, or the armed forces of another Commonwealth country or of a wartime ally of Canada or another Commonwealth country; • Performed NATO service or a United Nations mission as a member of the Canadian Armed Forces or a Canadian Police Force’ or • Served in the Vietnam War in the forces of the United States or its allies during this war and who was a Canadian citizen at time of service. 2. Application Application forms for a Veteran plate are available at any Access P.E.I. office in Prince Edward Island. Applicants are required to send the “Application for Veteran Plate Registration” form to Access P.E.I., along with proof of eligibility. The applicant must submit the certified application form to an Access P.E.I. office for the issuance of a veteran plate. 3. Types of Plates Veteran license plates are issued for motorcycles, passenger vehicles up to 4499 kg and recreational vehicles. 4. Fee The registration fee for a Veteran plate will be the standard fee associated with the class and weight of vehicle being registered, plus an additional $5 fee at the time of issuance. For future renewals, regular plate renewal fees will apply. Applicants may apply to replace their current regular alpha-numeric license plate with a Veteran plate, at any time, (if eligible) by returning their current plate and paying a fee of $5. Applicants do not have to wait until their current plate expires. 11/06/2019 .
