Lachowsky1,3* Abstract Background: The emergence of biomedical and seroadaptive HIV prevention strategies has coincided with a decline in condom use among gay men. Methods: We undertook a social ecological analysis of condom use and perceptions using nineteen semi-structured interviews with HIV negative gay men in Vancouver, Canada who used HAART-based prevention strategies. Results: Contributors to inconsistent condom use were found at various levels of the social ecological model. Ongoing concern regarding HIV transmission and belief in the proven efficacy of condoms motivated contextual use. When condoms were not used, participants utilized seroadaptive and biomedical prevention strategies to mitigate risk. Conclusions: These findings indicate that notions of “safety” and “risk” based on consistent condom use are eroding as other modes of prevention gain visibility. Community-based and public health interventions will need to shift prevention messaging from advocacy for universal condom use toward combination prevention in order to meet gay men’s current prevention needs. Interventions should advance gay men’s communication and self-advocacy skills in order to optimize these strategies. While consistent condom use remains the most effective intervention for preventing transmission of HIV and other STDs between sexual partners, low levels of assertive communication between romantic partners has been associated with inconsistent condom use. Guided by Uncertainty Management Theory, the current study hypothesized that high-risk youth with high relational uncertainty would demonstrate low levels of assertiveness during a laboratory-based condom negotiation task with their current romantic partner. It was also hypothesized that assertiveness would be positively correlated with condom use. A video-recall paradigm was used to assess youths' subjective interpretations of their condom negotiation communication with their romantic, opposite sex partner. Dyadic multilevel models revealed that hypotheses were not completely supported. While depressed men with high relational uncertainty showed low assertiveness, depressed women showed high assertiveness. Post-hoc analyses revealed a curvilinear relationship between couples’ assertiveness and condom use, with the highest levels of condom use being found among moderately assertive couples. Unexpectedly, adolescents rated themselves and their partners in almost exactly the same way. Results suggest that adolescents' condom negation is a dynamic process that is influenced by both partners' perceptions and behavior.

A global consultation meeting was held in Geneva, Switzerland, from 6 to 7 November 2014, to reaffirm the critical role of male and female condoms as part of combination HIV prevention, to shape an agenda to scale up condom promotion and distribution, and to identify strategies to overcome bottlenecks in scaling up. The meeting established key trends in global condom distribution and use in different population groups: Condom use among young people has been increasing over the past two decades. Condom use at last sex varies from more than 80% in some Latin American and European countries to less than 30% in some African countries. Although increases were recorded in Africa and in some countries there is close to 80% of condom use with non-regular partners, condom use remains lowest in the Africa region. This degree of variation shows that there are important opportunities for strengthening demand and supply of condoms. Condom use among sex workers is high and exceeds 90% in several contexts.
