1/15 Minutes

Resolved that the minutes of the meetings of the Health Scrutiny Board on 19th March, 2015 and the Adult Social Care Scrutiny Board on 24th March, 2015 be confirmed as a correct record.

2/15 Continuing Health Care

The Board noted that this matter had previously been considered by the Adult Social Care Scrutiny Board on 10th February 2015 to consider a report from officers in Adult Social Care in relation to Continuing Health Care. At this point the Adult Social Care Scrutiny Board had requested clarification from the Clinical Commissioning Group on the following points:-

[IL0: UNCLASSIFIED] 3 Health and Adult Social Care Scrutiny Board – 18th June, 2015 i) The number of people where positive CHC checklists have been completed but are awaiting a full DST assessment and the number of people who have been waiting over 28 days. Scrutiny would like to understand the reasons for any delays and what steps are being taken to clear any backlogs that exist and ensure future assessments are dealt with in a timely way; ii) How the service is promoted.

In response to these points the Sandwell and West Birmingham Clinical Commissioning Group presented a report which provided further detail together with progress which had been made in this area recently.

The Board was pleased to note that as a result of continued close working between the Clinical Commissioning Group and Adult Social Care there had been no outstanding assessments since January 2015. In addition, the number of individuals who had been found not to be eligible for Continuing Health Care had reduced, mainly due to improvements in the quality of assessments undertaken.

3/15 Commissioning of Primary Care

The Board noted that the Five Year Forward View presented by NHS in 2014, advocated for Clinical Commissioning Groups to take on the commissioning of Primary Care Medical Services. It was put forward that this would be a key enabler in developing seamless, integrated out-of-hospital services based around the needs of local populations. Members noted that the Clinical Commissioning Group also felt that it would drive the development of new models of care such as multispecialty community providers and primary and acute care systems.

Under delegated agreement with NHS England Sandwell and West Birmingham had taken on the following responsibilities:-

• Patient transport is an important service, which: • increases access to health services • transfers eligible patients to and from hospital • Eligibility is based on medical need.

8 [IL0: UNCLASSIFIED] Responsibility for funding NEPT

• Clinical Commissioning Groups (CCGs) took over responsibility for funding Non-Emergency Patient Transport (NEPT) on 1 April 2013 • This includes non-urgent, planned or unplanned journeys where patients meet the eligibility criteria • It does not include emergency transport, or provision of services based on social need.

• Funding for NEPT was given to the Birmingham, Sandwell & Solihull NHS acute hospitals • As a result there are different contracts in place across these geographical areas • There are two NEPT service providers, operating the main contracts and a range of other third party providers • There is significant variation in terms of performance, patient experience and costs.

• Collaborative project with the four Birmingham and Solihull CCGs, with Birmingham CrossCity CCG leading • In total 17 West Midlands CCGs are involved, who will all jointly procure the new service • CCGs will take over responsibility for commissioning NEPT services at: Birmingham Women’s NHS Foundation Trust (FT), Heart of England NHS FT , University Hospitals Birmingham NHS FT and Royal Orthopaedic Hospital NHS Trust • It will also provide NEPT for our patients who are accessing Worcestershire Acute Hospital NHS Trust • NEPT could also apply to other services as determined by the commissioner e.g. pulmonary rehabilitation provided within community clinics.

Activity

• A significant number of patients benefit from NEPT: • 350,509 patient journeys in 2013/14 • 83% were for attending an outpatient appointment and the remaining 17% for discharged patients • 59% (205,668) were for patients in the ‘walking’ category • 28,009 journey for patient escorts (relative or carer) • Most journeys were within a 20 mile radius between 7am and 11pm.

What the service costs

• Current investment for this service is in the region of £8 million • The proposed plans intends to use this investment to commission: • a better quality service • deliver better value for money • ensure we focus resources where they are needed • innovation • CCGs are committed to funding NEPT and the development of a sustainability of the service. 13 [IL0: UNCLASSIFIED]

Consultation and engagement

• Consultation is taking place within the Birmingham, Sandwell and Solihull areas only • Consultation started on 20 May and will end on 21 August 2015 • Actual and planned activity includes: • series of public meetings • informal drop-in sessions at key NHS sites • visits to renal units, with structured patient interviews • other ad-hoc / by invitation engagement, with a range of stakeholders e.g. patient, community and faith groups, third sector organisations etc. • social media and public relations activity.

20 About the consultation Clinical Commissioning Groups (CCGs) commission or “buy” health services on behalf of people living in Birmingham, Sandwell and Solihull. There are four CCGs locally: • Birmingham CrossCity CCG; • Birmingham South Central CCG; • Sandwell and West Birmingham CCG; and • Solihull CCG. This consultation also includes those patients who live outside of Birmingham, but their GP practice is registered with a Birmingham CCG. For example, a large population of people living in Alvechurch in Worcestershire have a GP practice that is part of NHS Birmingham South Central CCG. We are asking for people’s views on proposals to commission a new non-emergency patient transport service. In this document we explain what some of these changes may mean for people that are eligible to use non-emergency patient transport services. What are non-emergency patient transport (NEPT) services? NHS-funded non-emergency patient transport services help eligible patients who have a health need, and are unable to travel by private or public transport, to get to and from healthcare services for either daily living, a non-urgent or planned journey. This service mainly helps eligible patients who are: • accessing hospital healthcare services; • being transferred between health services; or • being returned back to their place of residence. Non-emergency patient transport services aim to give patients safe, timely and comfortable transport whilst ensuring they are safe and

21 1 well. These services are available free of charge to eligible patients and sometimes this may also include having an escort, or carer, travelling with the patient.

What is not included in this consultation? We are not seeking views on transport for patients requiring urgent or emergency (999) services.

22 2 What the NEPT service looks like now Across Birmingham, Sandwell and Solihull NHS trusts1 use the money provided by CCGs to buy non-emergency patient transport services for their hospital and healthcare services. This means that there are different contracts and standards in place at each of the NHS trusts, rather than one universal service for all patients. It also means that the service costs a different amount at each of the NHS trusts. The proposed new universal non-emergency patient transport service will be for patients accessing healthcare services at hospitals and community services run by the following NHS trusts: • Birmingham Women’s Hospital NHS Trust; • Heart of England NHS Foundation Trust (this includes Heartlands Hospital, Good Hope Hospital and Solihull Hospital); • The Royal Orthopaedic Hospital NHS Foundation Trust; and • University Hospitals Birmingham NHS Foundation Trust. NEPT services are commissioned differently in Worcestershire. However, to ensure there is no gap for patients that live in Birmingham, Sandwell and Solihull, the proposed new service will also provide transport for eligible patients travelling to or from: • Worcestershire Acute Hospitals NHS Trust (this includes Alexandra Hospital, Redditch, Kidderminster Hospital and Treatment Centre and Worcestershire Royal Hospital).

In 2013-14 over 350,000 patients benefited from access to non- emergency patient transport services at the NHS trusts listed above. This cost about £8.3 million. Most of these patients were travelling within 20 miles of the healthcare services they required.

1NHS trusts provide a range of hospital and community health services (note) See page 6 23 3 Over three-quarters of these journeys (83%) were for outpatient appointments, and over half of the patients travelling to these appointments were in the ‘walking category’ (see page 6) - that’s over 205,000 patient journeys. CCGs also fund non-emergency patient transport to allow eligible escorts to travel with a patient, where they have skills that cannot be provided by the ambulance staff or when required to travel with a child who is under 16. Over 28,000 patients benefited from having an escort, or carer, to travel with them funded by the CCGs.

Why do we want to make changes? The existing service contracts will be coming to an end soon, so the CCGs must take action to ensure there is no gap in service. This also gives us an opportunity to look at the quality of these services. We want to make improvements, so that there is one universal non- emergency patient transport service for patients at the NHS trusts listed on page 3. In creating one universal service, it will be easier to make sure that non-emergency patient transport services are meeting the needs of patients: • by improving the quality of services and making them better value for money; • by ensuring more patients experience good non-emergency patient transport; and

Birmingham, Sandwell and Solihull Non-Emergency Patient Transport Service • by making sure those with complex health Patient Charter and needs can access the service they need. Eligibility Criteria To improve the quality of non-emergency patient transport, we are proposing to bring in a new patient charter for the service. It will also mean that we have to make some changes to create one universal service, which

24 4 aims to provide a fairer and better quality service for everyone, for example the same collection times for everyone. Details of the proposed new service will be set out in the service specification and patient charter. These will be available either online, at one of the consultation events or upon request. What would the changes mean? The proposed new universal non-emergency patient transport service will be very similar to the existing service, but there will be a far greater focus on: • patient satisfaction; • quality of services; and • ensuring eligibility is applied consistently and checked ensuring that the service is aimed at those with a health need for transport. Eligibility The CCGs would like to ensure that the eligibility criteria is universal across all NHS services. We want to reduce inappropriate use of free non-emergency patient transport. This means making sure that those patients who are able to travel by private or public transport for day- to-day living do so, unless they are receiving active treatment at their appointment. We are consulting on proposed new eligibility criteria, which will mean that: • eligibility will be checked, and will be the same at each NHS service; and • those patients whose health needs mean that they are able to travel by private or public transport, do so. These changes will mainly affect people in the ‘walking category’. This means a patient whose health needs mean that they can travel in a private car (e.g. a relative, friends or neighbour’s car), or on public transport and they do not need help getting in and out of a vehicle.

25 5 These patients will not qualify in the future for patient transport. Eligible patients who are receiving active treatment at their appointment, for example renal dialysis or cancer patients, which means it is unsafe for them to travel home by private or public transport, will still qualify for non-emergency patient transport. Their eligibility will be checked and if they qualify for non-emergency patient transport, then they will continue to receive this as they do now. Bookings We want to make sure it is easier for patients to find out about the booking service and book transport, so there would be a new patient leaflet and a dedicated website just for patient transport. Opening times The service will remain a 24-hour service, but we want to make sure that the new provider is able to meet the needs of eligible patients during the busiest times. These are between 7am and 10pm, seven days a week. Quality and patient charter The CCGs would like to introduce a new patient charter and this has been put together with help from patient representatives and other partners, including from the NHS and voluntary sector. We would like to hear other views on these plans, which would mean patients have access to the same standard and quality of service at the four NHS trusts. To do this CCGs have had to align service standards to make them the same and therefore fairer for everyone. This will result in some changes to collection times - please see the patient charter for details.

We are running a 12 week public consultation to seek views on: • changes to the eligibility criteria for NHS-funded non-emergency patient transport; • whether or not the standard of service should be the same for each NHS trust; • the Patient Charter for non-emergency patient transport; • how the new patient transport service may affect people that have a medical need for transport; and • any other suggestions, alternative views on patient transport services or access to healthcare services. We would like you to complete a questionnaire to help us understand your views. This information will enable the CCGs to review their plans and to shape them based on what you tell us. Introduction

This report for the Health and Adult Social Care Scrutiny Board is to provide an update on car parking charges and concessions and also give a comprehensive comparator to both the local hospital and private charges.

1. Impact Analysis An impact analysis study will be undertaken in order to provide a thorough investigation into the following:

. Do car parking charges encourage a shift from the car to other modes of travel. . Is an increase in car parking charges likely to result in car drivers seeking alternative parking in the surrounding community. . Are the visitor charges a fair reflection on the costs of parking in Sandwell and Birmingham. . Do the concessionary passes offer VfM and do they take into account all of the varying needs of patients and visitors.

The Trust is also introducing daily free parking for family/visitors that are in attendance throughout the day supporting and assisting in the care of their relatives/friends. The tickets are discretionary and will by approved by the relevant Ward Manager/Matron.

34 6. Midlands Metropolitan Hospital

. Parking

From the onset of the design process Carillion has sought to deliver a unique hospital for the Trust, one which delivers a world-class facility for patient care and responds to the Trust’s desire to look after their staff.

Staff who are supported, valued and cared for and who work in an environment that is fit for purpose and supports safe, efficient working will be happier and more effective. The Carillion design offers significantly enhanced staff amenities, including staff car parking placed directly under the hospital.

Integrated parking is a feature of many overseas hospitals, but is not frequently seen in the UK. However, it is a recognised feature of many contemporary airport and shopping centre designs and therefore familiar to the public.

A key benefit to an integrated car park is that it provides both staff and patients with well-lit, secure parking close to the hospital, whilst retaining separate entrances for each. It also automatically creates a vertical focus for staff and visitor movement. To aid orientation, in the Midland Metropolitan Hospital this vertical circulation will be deliberately highlighted from both outside and inside the hospital.

. Public Transport (Bus Routes)

The bus routs will be part of the outer circle route and bus routes 11, 83 and 89 will enter the site at the rate of 24 per hour.

Bus routes 82 and 87 currently stop on Dudley Road, this service will continue. These buses drop off in a new bus hub situation on the road behind McDonalds. This will entail a walk of approximately 200 metres to the main entrance of the hospital, through the hospital grounds.

Continuing discussions with the bus companies are being held. Update to be provided as soon as the routes have been confirmed.
