The work being done at Winchester Primary Care Associates (WPCA) is evidence of how this collaboration is truly beneἀting patients. An advanced primary care pilot takes keeping patients healthy and out of the hospital to a whole new level. The pilot project includes placement of a diabetes educator from the hospital’s Community Health Institute (CHI) in the WPCA practice to collaborate with clinicians and, most importantly, pressure. Continued collaboration between CHI and WPCA, diabetic patients to ensure proper diabetes protocols are adhered as well as additional Winchester Hospital practices, is planned to thus avoiding hospitalization. She had placed her wedding dress in her kitchen so she could be reminded of what she was living for. On the Friday before the wedding, Sylvia was told she had 24 to 48 hours to live. So Winchester Hospital nurses Desiree Letellier, RN and Krista Callan, RN offered to accompany Sylvia to the wedding. Her hair, make up and nails were done by many of the nursing staff and she took an ambulance to the church. “Once she saw her s on ,” Letellier said, “I saw a smile on her face that I hadn’t seen before. Another plus is that those patients undergoing day surgery aren’t going into the hospital. This can have an extremely positive effect. “There is something nerve wracking about hospitals. Even if you know i t’s for an outpatient procedure, when you go to the hospital there is a sense that you are sick,” Mary Moloney, director of surgical services noted. “But in the Ambulatory Surgery Center, patients d on’t feel that same anxiety.
