Managing Uncontrolled Symptoms PATIENT AND FAMILY GUIDE BOOK | LIVING WITH PEACE AND CONFIDENCE SECTION (Appendix) SECTION 2-B (Appendix) 2-B Managing Uncontrolled Symptoms Alternate Care Settings Should you develop uncontrolled symptoms that cannot be addressed in your current residence, your hospice benefit includes General Inpatient Level of Care, which is provided in an alternate care setting, such as a hospice inpatient unit or skilled nursing facility. In order to continue receiving hospice care from MJHS Hospice and Palliative Care (MJHS-PC), you must be at a MJHS-PC contracted facility. This section includes the most current list of alternate care settings that MJHS-HPC is contracted with. Please complete the Inpatient/Skilled Nursing Preference Form in this section to predetermine where you would prefer to be transferred, should this level of care be needed. In the event of an emergency we will do our best to transfer you to the unit or facility of your choice. It aggregates a broad range of resources — from nationally recognized engineering and design guidelines and standards to federal, state, and local laws, rules, and regulations — to provide information on treatments that are allowed and encouraged on New York City streets. The Manual’s intended audience is diverse, consisting of design professionals, city agency staff, elected officials, community groups, and private developers. The Street Design Manual supplements rather than replaces existing engineering and environmental standards, requirements, or guidelines, such as the Manual on Uniform Traffic Control Devices (MUTCD), AASHTO Policy on Geometric Design of Highways and Streets, and 2010 ADA Standards for Accessible Design. In a city with as many varied and complex conditions as New York, designs must be tailored to the needs and opportunities created by the local context, uses, and dimensions of streets.
