Porokeratosis is characterized clinically by hyperkeratotic papules or plaques with a thread-like elevated border. Although rare, porokeratosis has been reported in conjunction with malignancies suggesting a paraneoplastic nature. This review will focus exclusively on reactive white oral lesions. Included in the discussion are frictional keratoses, irritant contact stomatitis, and smokeless tobacco keratoses. Leukoedema and hereditary genodermatoses that may enter in the clinical differential diagnoses of frictional keratoses including white sponge nevus and hereditary benign intraepithelial dyskeratosis will be reviewed. Many products can result in contact stomatitis. Dentrifice-related stomatitis, contact reactions to amalgam and cinnamon can cause keratotic lesions. Each of these lesions have microscopic findings that can assist in patient management. Joshi, [email protected] Abstract Galli-Galli disease (GGD) is a rare genodermatosis that is an acantholytic variant of Dowling-Degos disease that presents as lentigo-like macules/papules with progressive reticulated hyperpigmentation. Heat, sweat, ultraviolet light exposure, and topical retinoids have been reported to exacerbate the lesions associated with GGD. Here, we present a 77-year-old woman with end-stage renal disease and GGD who reported a worsening of lesions during the summer months and following hemodialysis treatment. Despite the severity of her lesions following dialysis, she refused treatment with isotretinoin out of concern for its side effect profile. In this case report, we discuss some available treatment options for GGD and review the exacerbating factors for GGD currently reported in the literature. Clinical presentation can provide orientation for the diagnosis, but skin biopsy is often necessary in determining the cause of blister formations.
