REGISTER FOR INDIAN SERVANTS. FEW officers on going to India have not experienced the difficulty of getting good servants. The discomforts on arrival and of a long journey up country, unprovided with a bearer, or, what is worse, provided with a hastily selected man, taken haphazard http://militaryhealth.bmj.com/ from the crowd of indifferent or bad characters who congregate in Bombay, have fallen to the lot of most of us; whilst the period of trial and vexation until a proper staff of servants is secured is familiar to us all. In our Corps, with regular annual reliefs, it shOUld not be difficult to arrange for an interchange. Officers leaving India would then be able to provide places for the good and tried retainers they are relinquishing, and new arrivals would, by taking on these men, be spared many of the worries and troubles which now befall them. Further, good servants would not be lost to the Corps, and the prospects of continuous employ­ ment oould-not fail to have attraction for the better class of men. With these ends in view, officers due home from India are requested to oommunicate to the Journal particulars of servants whom they can recommend, so that officers going out in relief may have an opportunity of securing these men. The particulars required are :-

EXAMINATION FOR ADMISSION TO THE ROYAL ARMY MEDICAL CORPS. Medicine (Case for Commentary).-Thursday, July 26, 1906. Commencing 10 a.m. (Time allowed: One and a-half hours.) Read your instructions. A male, aged 40, suffering from dyspncea and subcutaneous emphysema. The history was as follows :- For the last five months he had suffered from a cough. Three months previously a 'considerable hmmoptysis of bright red blood, and a slight recurrence just before admis­ sion. The night before admission the paroxysmal cough was exceedingly severe, and,

under strain of coughing, his neck began to swell. The chief physical sigus were: Protected by copyright. emphysema over front and back of chest, extending up both sides of neck and face; lung and heart sounds appeared normal, but the apex beat was displaced to tbe left. Pulse regula,r, 72; respirations, 32; orthopncea. The face became cyanosed during violent fits of coughing, and he spat up frothy material. During the next five days the emphysema , gradually diminished. Temperature ranged between 99° and 102°; pulse, 90 to 100; and respirations, 34 to 40. A dull area was detected at base of right lung' behind, up to the ninth rib. Above this level the chest was hyper-resonant. Two days later pus was found upon exploring the chest. The general condition of the patient became much worse, and the face and extremi­ ties livid. Next day, the right pleura was incised and a large quantity of foul pus with gas escaped. The pulse rate ran up to 120, and the patient died that night. Discuss fully the diagnosis and treatment. Surgery (Case for Commentary).-Thursday, July 26, 1906. Commencing 11.30 a.m. (Time allowed: one and a·half hours). Read your instructions. A- B-, coachman, was admitted into -- Hospital with the following history of illness :- http://militaryhealth.bmj.com/ His family history was an excellent one in all respects, and he had himself always been a strong and healthy man until eleven years before his admission, save that some sixteen years ago he had had an attack of gonorrhcea, unattended with any nnusual complication that was detected. He is not aware of ever having had syphilis, and cross· examination fails to elicit any symptoms that can be interpreted as syphilitic, either at the time when he suffered from gonorrhcea or at any time since. Eleven years ago, however, he commenced to suffer occasionally from attacks of severe pain, which he describes as affecting hi8 epigastric region and the lower part of his chest; and these were accompanied by vomiting of his food and of bile-stained material. The attacks referred to have never entirely left him; they have recurred ever since at irregular intervals, sometimes a few weeks, and occasionally a few months intervening between them . . For about eight years he has further suffered from pains in his lower extremities, coming on now and then, and had begun to notice that he was ullable to walk as well as formerly, being aware that he was unsteady on his legs from imperfect co-ordination, on September 23, 2021 by guest. and that he felt in walking as if he were treading upon ground softer under his feet than was really the case. He has been treated by several medical men of ability; on at least two occasions his treatment being that for gastric· catarrh; and it is evident that the disturbance to his gait had been so slight as to have passed unobserved, since it was not detected by them, nor did he himself consider his complain ts of the lower extremities sufficiently important for him to draw attention to the unsteadiness of his gait or to the pains from which he occasionally suffered in them. his gastric trouble was so severe a~ to compel him to lie up, he haR been pedectly able to continue his occupation until quite recently, when hi~ knee became affected. Six weeks ago he was assisting to lift a box to the seat of his cab, and got an abrasion on the front of his right leg over the junction of the lower and middle thirds of the tibia in front. During the ensuing day this gave him considerable uneasiuess, and both the pain and the swelling that accompanied it became so great during the few days that followed that he was obliged to abandon his occupation and take to his bed. The leg became. red as well as swollen, and his medical attendant found that these appearances had spread to above the knee-joint, and presented the phenomena of an attack of erysipelatoid inflammation. After taking to his bed the pain was sensibly relieved and continued to subside quickly, but the Bwelling in the leg and foot were much slower in disappearing, so that a few weeks elapsed ere it left the foot. It never quite left the knee, but persisted up to the time of his admission, and prevented his resuming his duties. Upon admission the patient was observed to be a man of about 40 years of age, well nourished, with a good complexion and a healthy appearance. His chief com­ plaint was now the swelling of and inability to use the right knee. The marks of the abrasions on his skin were still to be seen. His right knee was enlarged in a globular form (see radiogram), and ,bulged inwards as if the knee were valgous in position. Palpation revealed that this enlargement was largely caused by an accumulation of fluid in the knee-joint, which was neither spontaneously painful nor tender to pressure, and presented the usual features of a hydrops articuli. While palpation was being applied the examining hand received the impression that the internal condyle of the femur projected inwards, and that the head of the tibia was enlarged and irregular in shape; these phenomena being, however, by no means very distinctly made out. The motions of the joint seemed to be carried out without pain being produced, but they Protected by copyright. were limited in range, full extension-his usual posture-being perfect, but flexion possible tbrough a range of only 10 degrees. He was found to be unable, however, on standing up, "to sustain his weight upon the knee, and could not walk without assistance. It was observed that the arches of his insteps were high, and particularly on the right side, approached to the condition of pes cavus. There was no abnormality detectable in any other of the bones or joints. After he had been treated for three months. by means of rest in.bed, until the fluid in the knee-joint had greatly diminished, and subsequently by a padded splint, with foot-pieee, along the back of the limb, cotton wool around the joint, and bandaging, the notes made regarding his condition were as follows, viz. :- The fluid in the joint has disappeared, but the circumference of the joint remains one inch in excess of that of the left side. The circumference of the right calf is also one inch greater than the left, but in the lower part of the leg the measurements are the same on both sides. The flexion and extension of the joint can be carried through a range of 90 degrees, without pain or difficulty of any kind. But lateral movement of the knee-joint is now observed to be possible, for motions in the direction of ab and adduction can be effected through a range of 10 degrees, owing to relaxation of the http://militaryhealth.bmj.com/ lateral ligaments. On comparing the length of the two limbs as measured from anterior superior iliac spories to internal malleoli, it is found that the right is the shorter by one inch. The general enlargement of the knee-joint and inward projection of the internal condyle of the femur continue to be the most striking appearance to the eye; the internal condyle on palpation feels enlarged, but is smooth in outline. The patella feels larger than its fellow, and is displaeeable outwards, especially when the knee is flexed, so as to be in the position of dislocation. Palpation now reveals the presence of bony outgrowths, mainly distributed around the head of the tibia close to its articular margin, and more especially on the anterior internal and posterior aspects of the joint. The tibia is rotated. inwards on its longitudinal axis, so as to be subluxated on the femur. ' Radiograms have been taken at various times since his admission, giving always much the same appearances. The last of these, taken at this time, and reproduced

There are no other changes detectable about his bones or joints, and the feet remain in the same eondition as has been already mentioned. The patellar reflexes are completely absent. The pupils of his eyes are somewhat unequal in size, dilated, and fail to react to light. They react, however, slightly when he accommodates for near or distant vision. The fundus oculi is normal. CORRESPONDENCE. ARMY MEDICAL OFFICERS' BENEVOLENT SOCIETY. To the Editor of the" JOURNAL OF THE ROYAL ARMY MEDICAL CORPS." Sm,-I have lately received the Report of the Army Medical Officers' Benevolent Society, and also, in the Corps Journal, read the remarks of the Director-General deprecating the small number of subscribers to this excellent Fund. I would like to point out that the oost of administration to this Fund comes to over 7'2 per cent. of the monies distributed, i.e., Is. 5d. in each £1 paid out; this seems unduly high for a Corps Benevolent Fund. Again, donations and subscriptions are inextrioably mixed up in the Report, contrary to all sound finance, so that it is not

possible to see wbat the assured income of the Fund is, nor to separate dead subscribers http://militaryhealth.bmj.com/ from living ones.
