This tool can increase the quality of life for Jefferson residents and families by strengthening the connections among community practitioners and organizations with expertise and commitments to provide assistance. Earlier editions of the Directory have been well- received and we are grateful to users who have offered the changes reflected in this version. We all need the support of our community at various times and this Directory is being distributed to the key connectors in the area served by Jefferson Hospital – community-based organizations, churches, libraries, physician practices, hospital case managers, food pantries, the local offices of elected officials and more. We recognize that health and quality of life are intertwined, and so the listings cover a wide variety of human needs, including behavioral health, child/youth programs, civic-neighborhood resources, healthcare, human services, immigrant and refugee services, senior services, transportation, and workforce development. This Directory was created through an important partnership between the Jefferson Community Collaborative, Jefferson Regional Foundation and Jefferson Hospital. The intention was to list contacts at services and programs which are located in the Jefferson area to facilitate direct connections—not to replicate other regional guides. The Directory includes a listing of local organizations, an area map, detailed program information by category, a notes section on each page for users, and additional resource lists of local schools and local legislative offices. Special thanks to the 79-member organizations of the Jefferson Community Collaborative (see next page) and its Directory Advisory Committee, Jefferson Hospital President and CEO Louise Urban, and the Board of Directors of the Jefferson Regional Foundation. The Collaborative members offered significant and essential ideas about this guide and are committed to its use. We are thankful for the early guidance of the Mon Valley Providers Council and PA 2-1-1 Southwest. In March 2013, the agreement which guided the affiliation of Jefferson Hospital with the Allegheny Health Network provided $75 million for this new mission. To date, over $7 million in grants have been provided to community groups for projects and programs in the South Hills and lower Mon Valley area for the following priorities: increasing health access and prevention, improving child and family outcomes, and strengthening vulnerable populations and communities. The Foundation’s focus on improving health for all means a commitment to addressing the “social determinants” of health, an understanding that “place matters”, and building the capacity of key community resources. The Foundation recognizes its role in convening and sharing information through its annual community learning event called the Jefferson Forum, supporting the work of its partners through the Jefferson Community Collaborative and other convenings, and in its support of this Jefferson Community Directory. Jefferson Hospital, a part of Allegheny Health Network (AHN), is a full-service inpatient facility located in Jefferson Hills that serves the South Hills of Pittsburgh, the lower Mon Valley, and surrounding communities. Featuring a medical staff of nearly 400 physicians covering more than 40 health care specialties, the hospital provides comprehensive inpatient and outpatient medical and surgical services as well as emergency care. It features a Cardiovascular Institute, Cancer Institute, Orthopaedic Institute, Bariatric & Metabolic Institute, Women and Infants Center, Women’s Diagnostic Center, Sleep Disorders Center, and Wound Care Center among other services. Jefferson Hospital also includes the Bethel Park Health + Wellness Pavilion as well as several outpatient, primary care, and therapy sites across the South Hills and neighboring areas. The Jefferson Community Collaborative was convened in the fall of 2015, building on the energy and enthusiasm generated in June of that year by the first Jefferson Forum. The Collaborative is built on the collective strengths of 79 member organizations (listed on the next page) whose vision is to create agile, healthy and sustainable communities for diverse populations throughout the Jefferson region while: serving as an incubator for powerful ideas and promising practices, improving the capacity of community-serving organizations, and facilitating group action on community aspirations. The Collaborative meets five times annually and is hosted by the Jefferson Regional Foundation. Smaller groups of collaborative members meet year-round in action teams to develop grassroots solutions to community challenges.
