Background

A The ANU, as represented by the Research School of Population Health, is the recipient of a grant from the National Health and Medical Research Council (NHMRC) and a funding agreement with QIMR, who have funding from the UBS Optimus Foundation, (Head Agreements) for a research project titled: A school-based health education package for the prevention of soil-transmitted helminth infections in China and the Philippines. A significant number of project activities are based in the Philippines (Magic Glasses Philippines).

B The project Magic Glasses Philippines investigates the impact of a video-based health education package on knowledge of soil-transmitted helminth (STH) infections, hygiene behaviours and infection outcomes. It targets some 2, 500 schoolchildren aged 9-10 years (Grade 4) in the Laguna province, the Philippines; and is implemented for a duration of two years, in collaboration with the Participating Parties, with facilitation of collaborative activities through a steering committee.

C A Project Steering Committee, composed of representatives from the ANU, QIMR, RITM, Department of Health, Department of Education, Local Government Units of the province of the Laguna and its cities, will be organized to facilitate collaborations and contributions from the

2 Participating Parties in the conduct of the research related to soil-transmitted helminths through continuous engagement, feedback, guidance, support and endorsement, with specific objectives to: a) Ensure the research is aligned with DepEd and DoH control programs; b) Provide support and guidance for the implementation of the study; c) Ensure that potential threats to the projects success are identified and addressed; d) Ensure that the conduct of the project are monitored; e) Address any issues which may have major implications in the implementation and success of the project; f) Meet on a regular basis to assess progress and challenges of project implementation; and g) Report on project progress to those responsible at the next higher level.

D The Participating Parties are going to collaborate with ANU in the implementation of the Magic Glasses Philippines project by facilitating the research program within the selected schools. The specific activities of the Participating Parties are detailed in Schedule Item 6.

E Joint Purpose ANU and the Participating Parties wish to cooperate in the Magic Glasses Philippines project for the benefit of the Laguna Province community. The purpose of this Memorandum of Agreement is to set out the terms of their cooperation.

1. Head Agreements

1.1 The ANU and QIMR are required to conduct the Magic Glasses Philippines (MGP) in accordance with the requirements of the Head Agreements.

1.2 ANU and QIMR will lead the research program with RITM, the local coordinator in the Philippines.

1.3 The Participating Parties agree to collaborate with ANU to meet its obligations under the Head Agreements.

2. The Project

2.1 The ANU wishes to cooperate with the Participating Parties to conduct the Joint Purpose on the terms and conditions set out in this Agreement.

2.2 This Agreement will commence on the Commencement Date set out in Item 3 of the Schedule and will end on the Completion Date set out in Item 4 of the Schedule.

2.3 The Participating Parties agree to fully cooperate with ANU by complying with all reasonable requests made by the ANU for information and reports to enable the ANU to meet its obligations under the Head Agreements.

3 2.4 For the avoidance of doubt, ANU and the Participating Parties are each performing their own projects, and their cooperation is limited to this Joint Purpose. Neither Party is liable to the other in respect of any obligation, including in relation to the engagement of any persons and/or payment of any liabilities except as expressly set out in this Agreement.

3. Role of Participating Parties

3.1 The Participating Parties have agreed to undertake the Activities set out in Item 6 of the Schedule to the necessary standard required.

3.2 The Participating Parties do not have the authority to act for, or create or assume any responsibility or obligation on behalf of the ANU.

4. Funding

4.1 ANU and the Participating Parties will each maintain their own funding and personnel, and each will be solely responsible for the performance of its Activities under the Magic Glasses Philippines specified in Item 6 of the Schedule.

4.2 Neither party will do any act that might in any way cause the other party to be in breach of its obligations under the Head Agreements.

4.3 No party will be liable for any costs incurred by another party/parties, nor be liable to make any form of payment or offset in relation to the Magic Glasses Philippines.

5. Intellectual Property

5.1 This Agreement does not affect the ownership of any existing Intellectual Property rights.

6. Resolution of disputes

6.1 In the event of a dispute in relation to the Magic Glasses Philippines, the parties will nominate a representative to promptly meet to negotiate the dispute in good faith. If the dispute is not resolved, either party may refer the matter for mediation, with each party bearing its own costs.

7. Privacy and Personal Information

7.1 The Participating Parties acknowledge and agree that ANU is bound by the provisions of the Privacy Act 1988 (Cth) (Privacy Act). The Privacy Act requires ANU to pass on the obligations outlined in this clause to a third party where ANU is providing personal information to that third party. For the

4 purposes of this agreement Personal Information means information or an opinion (including information or an opinion forming part of a database), whether true or not, and whether recorded in a material form or not, about a natural person whose identity is apparent, or can reasonably be ascertained, from the information or opinion. A summary of the relevant provisions contained in the Privacy Act is set out below.

7.2 The Participating Parties agree to:

i. use Personal Information held in connection with this Agreement only for the purposes of fulfilling its obligations under this Agreement; ii. take all reasonable measures to ensure that Personal Information in its possession or control in connection with this Agreement is protected against loss and unauthorised access, use, modification or disclosure; iii. cooperate with any reasonable demands or inquiries made by ANU regarding use or disclosure of Personal Information; iv. ensure that any person who has access to any Personal Information is made aware of, and undertakes, to observe the obligations referred to herein; and v. obtain proper consents from the participants and deal with the personal information in accordance with those consents as will be performed by the RTIM staff.

8. Acknowledgement

8.1 Each party agrees to represent each other and the Magic Glasses Philippines in good faith. The use of the others identities will be positively and appropriately assigned in all publications, events and in any other media formats where the Magic Glasses Philippines is promoted or profiled.

8.2 The contributions of each party will be acknowledged in any research report, conference presentation or journal publication presented by any of the parties.

Item 6: Activities

, TiMelitte 5111111":"Wr-----ctivities of the Magic- Glasses Philippines T(MGP): 11/2015 to 9/2017 The health education package contains a cartoon video, classroom discussions, drawing and essay competitions and a pamphlet on transmission and prevention of STH. The package has been shown to improve the knowledge and behaviours and lower STH infections among students in China, and thus is adapted to the local context for this project, which aims to evaluate the potential for up-scaling of the package in different geographical areas and cultural settings.

The baseline survey of the project consists of stool examination for infection status, self-report questionnaires on knowledge, attitude and behaviours regarding STH infections and measurement of height, weight and haemoglobin levels of students, and review of school records for attendance and academic performance. Then, all students will be treated with WHO recommended dose (400 mg) of Albendazole which is the part of the DOH control program and implemented by the DepEd. Students at intervention schools will receive the health education package consisting of a 12 minute video, under Health or Science subject, which will be administered by the research staff and re-enforced after 1-2 months (See Item 7. Study Timeline). The same data collection activities at baseline will be collected at the follow-up. Any student found positive for intestinal worms will be endorsed for treatment with Albendazole for ethical reasons.

NOTE: A decision will be made after the completion of the first follow-up survey (in January 2017) on whether the timeline of the study will be extended to conduct the second follow-up survey in 11/2017 which will involve the conduct of the baseline parameters, i.e. stool collection and examination, survey (KAP), height and weight, and haemoglobin testing (See Item 7. Study Timeline).

Activities of the Participating Parties:

Study Timeline). • Determine the hemoglobin levels, and take their height and weight of the selected cohort of school children in the selected schools, outside class hours. • Administer to students field tested and validated questionnaires to gather their knowledge of and attitude towards abdominal parasites, their transmission, symptoms, treatment and prevention, and self-reported hygiene behavior. This will be done outside class hours. • Conduct the STH health education using the project developed video. • Monitor the implementation of the research procedures. • Share the data collected with partners and collaborators. • Conduct feedback meetings with partners and collaborators. • Coordinate with the school principals, school teachers and other stakeholders in the conduct of the data collection activities. • Respond to all queries from parents, teachers, and other stakeholders.

Provide Barangay Health Workers (BHWs) with training on the content of the consent form 4/2016

Provide the Head of Health and Nutrition Unit (HNU), DepEd, Provincial Health Officer, Laguna 11/2015; 6/2016; 9/2016; 3/2017 and STH coordinator, DoH with update report on the schedule and completion of MGP activities .

Coordinate with the HNU, principals, and teachers on the proper timing of the administration of 7/2016; 1/2017 baseline and mass deworming following DepEd environment and calendar.

The start of this proposed timeline is based on the assumption that BHWs will conduct house-to-house campaign/information dissemination about MOP starting April 2016. Informed consent will also be obtained during this time. There is a break in the Follow up 1 stool collection and survey as it will take 6 weeks and has to be completed prior to the January 2017 mass deworming ceremonial day (the last Wednesday of the month). In addition, the 3rd and 4th weeks of December are examination week and school break, and the 1st week of January is allotted to wait for all the children to go back to school. The 6th week of Intervention 1 activities will overlap with the designated mass deworming date (NSDD), but MGP activities can still be conducted for that week, except for the mass deworming day.
