INTRODUCTION Pre-labour premature rupture of membranes (PPROM) is defined as rupture of membranes more than 1 hour prior to the onset of labour at <37 weeks gestation. PPROM occurs in approximately 3% of pregnancies and is responsible for a third of all preterm births.1 Once membranes are ruptured prolonging the pregnancy has no maternal physical advantage but fetal morbidity and mortality are improved daily at early gestations: 19% of those infants born <25 weeks develop cerebral palsy (CP) and 28% have severe motor disability.2 Those infants born extremely pre term (<28 weeks) cost the public sector £75835 (95% CI £27906–145508) per live birth3 not to mention the emotional cost to the family. To prolong gestation is therefore the suggested goal: however how and why might we delay birth in those at risk? PPROM is one scenario associated with preterm birth and here we discuss the causative mechanisms, sequelae, latency, strategies to prolong gestation (antibiotics) and consider the role of amniocentesis. We will also discuss novel therapies. PATHOPHYSIOLOGY OF MEMBRANE RUPTURE The membranes, which act to protect and isolate the fetus, are composed of two layers. -

In any case where a birth occurs outside a hospital, or other recognized medical facility, without medical attendance and the birth certificate is filed by someone other than a health care provider, additional evidence in support of the facts of birth shall be completed and filed in the presence of the local Vital Records registrar in the county where the birth occurred. A birth certificate for a birth which occurs outside a recognized medical institution shall only be filed upon personal presentation of the following evidence by the individual(s) filing the certificate: (a) Proof of pregnancy: 1. Prenatal records; or 2. Statement from a physician or other licensed health care provider who is qualified to determine pregnancy; or 3. Prenatal blood analysis or positive pregnancy test results from a laboratory. (b) Proof of the mother’s residence on the date of the out of institution birth: 1. A valid driver’s license, or a state-issued identification card, which includes the mother’s current residence on the face of the license or card; or 2. A rent receipt which includes the mother’s name and address, and the name, address, and signature of the mother’s landlord. 3. A utility bill (e.g. electric bill, phone bill, or water bill) showing the address at child’s birth. (c) A copy of a bank statement showing the address at child’s birth. 2. An identifying document, with photograph, for the individual(s) personally presenting the evidence required to file the certificate. -

Care during Pregnancy and Delivery ACCESSIBLE, QUALITY HEALTH CARE DURING PREGNANCY AND DELIVERY Why It’s Important Having a healthy pregnancy and access to quality birth facilities are the best ways to promote a healthy birth and have a thriving newborn. Getting early and regular prenatal care is vital. Prenatal care is the health care that women receive during their entire pregnancy. Prenatal care is more than doctor’s visits and ultrasounds; it is an opportunity to improve the overall well-being and health of the mom which directly affects the health of her baby. Prenatal visits give parents a chance to ask questions, discuss concerns, treat complications in a timely manner, and ensure that mom and baby are safe during pregnancy and delivery. Receiving quality prenatal care can have positive effects long after birth for both the mother and baby. When it is time for the mother to give birth, having access to safe, high quality birth facilities is critical. Early prenatal care, starting in the 1st trimester, is crucial to the health of mothers and babies. But more important than just initiating early prenatal care is receiving adequate prenatal care, having the appropriate number of prenatal care visits at the appropriate intervals throughout the pregnancy. Babies of mothers who do not get prenatal care are three times more likely to be born low birth weight and five times more likely to die than those born to mothers who do get care.1 In 2017 in Minnesota, only 77.1 percent of women received prenatal care within their first trimester of pregnancy. -

PRENATAL AND PREIMPLANTATION GENETIC DIAGNOSIS FOR MPS AND RELATED DISEASES Donna Bernstein, MS Amy Fisher, MS Joyce Fox, MD Families who are concerned about passing on genetic conditions to their children have several options. Two of those options are using prenatal diagnosis and preimplantation genetic diagnosis. Prenatal diagnosis is a method of testing a pregnancy to learn if it is affected with a genetic condition. Preimplantation genetic diagnosis, also called PGD, is a newer technology used to test a fertilized embryo before a pregnancy is established, utilizing in vitro fertilization (IVF). Both methods provide additional reproductive options to parents who are concerned about having a child with a genetic condition. There are two types of prenatal diagnosis; one is called amniocentesis, and the other is called CVS (chorionic villus sampling). Amniocentesis is usually performed between the fifteenth and eighteenth weeks of pregnancy. Amniocentesis involves inserting a fine needle into the uterus through the mother's abdomen and extracting a few tablespoons of amniotic fluid. Skin cells from the fetus are found in the amniotic fluid. These cells contain DNA, which can be tested to see if the fetus carries the same alterations in the genes (called mutations) that cause a genetic condition in an affected family member. If the specific mutation in the affected individual is unknown, it is possible to test the enzyme activity in the cells of the fetus. Although these methods are effective at determining whether a pregnancy is affected or not, they do not generally give information regarding the severity or the course of the condition. -

Iutzi A thesis submitted in partial fulfillment of the requirements for the degree of Masters in Public Health University of Washington June 2013 Committee: Wendy Johnson Christopher Dodd Program Authorized to Offer Degree: Global Health Iutzi, Masters Thesis 1 Abstract Evidence strongly shows that the risk of childbirth is best mitigated through giving birth at or near a health institution with emergency obstetrical services and receiving regular prenatal visits. These interventions have been shown to improve maternal morbidity and mortality. Many under-resourced areas of the world continue to have difficulty connecting poor rural women to these services. In Matagalpa, Nicaragua a pioneering project, “Destrezas para Salvar Vidas,” was implemented in August 2011 to provide training and mentorship to the traditional birth attendants and community health leaders to connect pregnant women in rural communities with the formal health sector. This project was evaluated at the one-year point through tests of knowledge both before and after an initial one-week training, records of activities conducted by participants, and interviews with program participants and mentors. The aggregate test scores of participants’ knowledge during the initial week of training increased from an average score of 59.5% to 79.9% (differences 21.4%, p<0.001). Of the pregnant women in contact with program participants, 93% delivered at an institution, compared to 81% of all pregnant women in Matagalpa. Participants performed an average of 51 home visits each over the year. Reciprocal trust and communication increased between community participants and health sector workers. -

Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan antara perencanaan kehamilan dan pemanfaatan pelayanan kesehatan ibu dan anak. Metode: Penelitian ini menggunakan data dari Survei Kesehatan Demografi Indonesia 2012. Empat model regresi digunakan untuk mengidentifikasi hubungan antara perencanaan kehamilan dan pemanfaatan pelayanan kesehatan ibu dan anak. Hasil: Lebih dari seperlima (25,5%) responden menerima kelima jenis perawatan ibu dan anak. Lima belas persen wanita melaporkan bahwa kehamilan terakhir mereka tidak diinginkan. Perencanaan kehamilan berhubungan secara bermakna dengan penggunaan antenatal care yang memadai (OR: 0,53, 95% CI, 0,46-0,60), pemanfaatan antenatal care dan persalinan oleh tenaga kesehatan (OR: 0,62, 95% CI, 0,55-0,71), pemanfaatan antenatal care, persalinan oleh tenaga kesehatan dan postnatal care ( OR: 0,82, 95% CI, 0,72-0,93), namun tidak berhubungan secara signifikan dengan pemanfaatan antenatal care, persalinan oleh tenaga kesehatan, postnatal care hingga imunisasi lengkap (OR: 1,06, 95% CI, 0,91-1,22) setelah dikontrol menggunakan variabel sosiodemografi dan faktor obstetrik. Kesimpulan: Intervensi diperlukan untuk mengurangi kehamilan yang tidak diinginkan seperti meningkatkan akses ke layanan keluarga berencana. -

A total of 252 IVF-ET fetuses were cytogenetically analysed by either chorionic villus sampling (CVS; n=80) or amniocentesis (n= 172). Eleven chromosome abnormalities were found in the CVS group (13.8 per cent); among them, a 45,X/46,X,dic(Y)(ql1)/46,X,delCY)(qll) mosaic that was found in an IVF pregnancy established by intracytoplasmic sperm injection (ICSI), four cases of trisomy 21, and three cases of trisomy 7 confined to the placenta. The results indicate a statistically significant three- to five-fold increase in both confined placental abnormalities (P<0.008) and true fetal chromosome anomalies (W0.04).In the amniocentesis group, identical rates (1.7 per cent) of chromosome abnormalities were found in the IVF-ET and control groups.
