Supine Piriformis Stretch Cervical Curve Exercise Wall Squat With Supine Piriformis Stretch Stretching the piriformis muscle is almost always necessary to relieve the pain along the sciatic nerve and can be done in several different positions. A number of stretching exercises for the piriformis muscle, hamstring muscles and hip extensor muscles may be used to help decrease the painful symptoms along the sciatic nerve and return the patient’s range of motion. Begin by lying on your back and crossing your left leg over your right with your knees bent. Grasp left knee with both hands. Bring left knee toward the opposite shoulder. Slowly increase pressure on ankle using right hand. Feel the stretch in left buttocks. Hold for 30 seconds, repeat 3 times. Repeat with right leg. Cervical Curve Exercise Cervical kyphosis is a name given to the condition where the normal curve of the neck begins to straighten. Cervical kyphosis can progress to the point where the curve in the neck actually reverses, going in the opposite direction from its normal, healthy state. This exercise uses a bath towel rolled up. Place the towel behind your neck on top of your shoulders. Lean your head back and shrug your shoulders upward. Hold this position for a few seconds, do 10 repetitions. There should be no discomfort during this exercise, if you experience any pain do not continue. Wall Squat with Fitness Ball Squats engage the core muscles of the body. Abdominal and back muscles are needed to keep balance during the movement. The result is a tighter, flatter abdomen and a stronger lower back. Make a conscious effort to hold in your abdominal muscles while squatting down to increase this effect. Doing squats increases joint flexibility. The ankles, knees, hips and lower back are all utilized in the squatting motion. Be sure to maintain proper form to avoid injury. Stand with feet shoulder width apart, toes out at about a 45-degree angle and place an exercise ball behind your lower back for support. Bend the knees and lower into a squat, keeping the knees in line with the toes. Lower down as far as your flexibility allows and push into the heels to go back to starting position. Complete 10 repetitions. This exercise should be comfortable for the knees and hips, if not discontinue. .

    Piriformis Syndrome: the literal “pain in my butt” Chelsea Smith, PTA Aside from the monotony of day-to-day pains and annoyances, piriformis syndrome is the literal “pain in my butt” that may not go away with sending the kids to grandmas and often takes the form of sciatica. Many individuals with pain in the buttock that radiates down the leg are experiencing a form of sciatica caused by irritation of the spinal nerves in or near the lumbar spine (1). Other times though, the nerve irritation is not in the spine but further down the leg due to a pesky muscle called the piriformis, hence “piriformis syndrome”. The piriformis muscle is a flat, pyramidal-shaped muscle that originates from the front surface of the sacrum and the joint capsule of the sacroiliac joint (SI joint) and is located deep in the gluteal tissue (2). The piriformis travels through the greater sciatic foramen and attaches to the upper surface of the greater trochanter (or top of the hip bone) while the sciatic nerve runs under (and sometimes through) the piriformis muscle as it exits the pelvis. Due to this close proximity between the piriformis muscle and the sciatic nerve, if there is excessive tension (tightness), spasm, or inflammation of the piriformis muscle this can cause irritation to the sciatic nerve leading to symptoms of sciatica (pain down the leg) (1). Activities like sitting on hard surfaces, crouching down, walking or running for long distances, and climbing stairs can all increase symptoms (2) with the most common symptom being tenderness along the piriformis muscle (deep in the gluteal region) upon palpation.

    Stretches and Exercise for Sciatic Pain from Piriformis Syndrome A common symptom of piriformis syndrome is pain along the sciatic nerve, so it is often thought that piriformis syndrome causes sciatica. However, piriformis syndrome does not involve a radiculopathy - a disc extending beyond its usual location in the vertebral column that impinges or irritates the nerve root - so it is technically not sciatica. Instead, with piriformis syndrome, it is the piriformis muscle itself that irritates the sciatic nerve and causes sciatic pain. The piriformis is a muscle located deep in the hip that runs in close proximity to the sciatic nerve. When the piriformis muscle becomes tight and/or inflamed, it can cause irritation of the sciatic nerve. This irritation leads to sciatica-like pain, tingling and numbness that run from the lower back, to the rear and sometimes down the leg and into the foot. The first case was the presence of an accessory femoral biceps muscle that originated on the fascia that covered the short head of the femoral biceps and ended on the head of the fibula joining the common tendon formed by the long and short head of the femoral biceps. The second case was the presence of an aberrant digastric muscle in the gluteal region and in the posterior compartment of the thigh. He had two bellies; the upper belly, considered as a piriform muscle accessory; the lower belly, considered a third head of the biceps femoral muscle; these two bellies were connected by a long tendon. The other two cases were the presence of third head of the gastrocnemius.

This paper aims to describe and discuss the clinical importance of a unique variation involving not only the piriformis, gluteus medius, gluteus minimus, obturator internus, and superior gemellus muscles, but also the superior gluteal neurovascular bundle, and sciatic nerve. A routine dissection of a right hemipelvis and its gluteal region of a male cadaver fixed in 10% formalin was performed. During dissection, it was observed a rare presentation of the absence of the piriformis muscle, associated with a tendon fusion between gluteus and obturator internus, and a fusion between gluteus minimus and superior gemellus muscles, along with an unusual topography with the sciatic nerve, which passed through these group of fused muscles. This rare variation stands out with clinical manifestations that are not fully established. Knowing this anatomy is essential to avoid surgical iatrogeny. Safran, MD Professor, Orthopaedic Surgery Chief, Division of Sports Medicine PIRIFORMIS SYNDROME DESCRIPTION A nerve condition in the hip causing pain and occasionally loss of feeling in the back of the thigh, often to the bottom of the foot. It involves compression of the sciatic nerve at the hip by the piriformis muscle. The piriformis muscle rotates the hip allowing the thigh, foot and knee to point outward. The piriformis muscle travels from the pelvis to the outer hip. The sciatic nerve usually passes the hip between this muscle and other muscles of the hip. Occasionally (15-20% of the time) the nerve travels directly through the muscle causing pressure on the nerve. FREQUENT SIGNS AND SYMPTOMS Tingling, numbness or burning in the back of the thigh to the knee and occasionally the bottom of the foot. Occasionally tenderness in the buttock Pain and discomfort (burning, dull ache) in the hip or groin, mid buttock area, and/or back of the thigh, and sometimes to the knee Heaviness or fatigue of the leg. The pain is worse with sports activities such as running, jumping, long walks, walking up stairs or hills and is often be felt at night or with prolonged sitting.     PIRAFORMIS STRETCHES The piriformis muscle is a deep muscle located beneath the gluteal (butt) muscles. The piriformis muscle laterally rotates and stabilizes the hip. This muscle is important for athletes who participate in running sports that require sudden changes of direction. The piriformis works along with other hip rotators to turn the hips and upper leg outward (external rotation of the hip). Strong and flexible hip rotators keep hip and knee joints properly aligned during activity and help prevent sudden twisting of the knee during quick side-to-side movements, quick turns, lunges or squats. A condition called "piriformis syndrome," which causes pain deep in the hip and buttock, is believed to be caused when the piriformis muscle compresses the sciatic nerve. Stretching and strengthening a tight or weak piriformis muscle has been found to reduce or alleviate this pain in some athletes. Stretching the piriformis muscle is almost always necessary to relieve the pain along the sciatic nerve and can be done in several different positions. A number of stretching exercises for the piriformis muscle, hamstring muscles and hip extensor muscles may be used to help decrease the painful symptoms along the sciatic nerve and return the patient’s range of motion.
