Various Health Preparedness Activities Plan during Ganga Sagar Mela 2021 under the perspective of Covid19 Pandemic Situation

Introduction: The Diamond Harbour Health Team organizes curative, preventative and promotive Health Care Services at Ganga Sagar Mela 2021 like every year from 9th January, 2021, to till the Mela is officially declared closed (so far fixed at 17th January, 2021 Mid night), in collaboration with Health & Family Welfare Team from South24pargana health District under over all support and guidance from the State Health Department and District Administration. During Ganga Sagar Mela 2021, the District Team provides Health care & Family Welfare services to the pilgrims and others who visit this District on the auspicious occasion of MakarSankranti. This year, all the health care activities ofthe Melawill be organized giving prime priority of self protection and social distancing norms owing to the Covid19 pandemic situation.

Objective: To Provide A. Curative Services. B. Preventive and Promotion Health Services. C. Covid management services. D. Statutory and Medico-legal Services. In 2020, in the Health Department, 658 no. of all tiers of health care personnel were deployed which includes Medical Officers, Nurses and Para-Medical Staff, ANMs and Pharmacists deployed at 5 temporary hospitals, at 38 numbers of First AID Booths (1-3 shifts), Transit Points and Buffer Zones to provide Curative, Preventive and Promotive Health Services to the Lakh of Pilgrims visiting the Mela. This year, though we will continue the same serviceswith 704 no of human resources, in addition, we are focusing extra-precaution and protection and deputing extra man power keeping an eye to Covid19 Pandemic Situation.

B. Preventive and Promotive Services includes

1. First Aid Camps: 27 First AID Booths in 24x7 hrs mode and 10 First AID Booths in Two Shifts manned by the Departmentwere made functional in Ganga Sagar Mela 2020. 17634 patients were provided with Preventive and Promotive services at these booths in 2020.This year we also propose to arrange 37 First Aid Booths to provide services at the Mela Ground, Transit Point, 40 Buffer Zones of Ganga Sagar Mela 2020. The First-aid centers where from the basic level of health care services will be provided are as follows. Few centers (28 in nos.) will run 24 x 7 hrs and rest nine centers will run in two shifts. Thermal gun, masks, protective measures will be kept ready in Ganga Sagar Mela 2021 to provide preventive & promotive services at the Mela Ground, Transit Point, Buffer Zones of Ganga Sagar Mela 2021. . First AID and AYUSH Services were provided, 69729 pilgrims sought services from these Centers supervised by the Health Department. This year the same activities are expected. 4. Public Health Preventive Activities: Scavenging activities was supervised by the Health Department at the Mela Ground and Transit Points. 7639pilgrims shed in1271 lanes &1806 by-lanes and cumulative 46005 toilets for 7 days were cleaned by personnel provided by PHE andP&RD which was under supervision of the Health Department. All activities above are proposed to be replicated this year. 5. Food Safety& Licensing : Non permitted colors used in food making were seized from the mela ground and other places by a team deployed by Dy CMOH II, and designated Food Safety Officer, South 24 Parganas. Expired packed food items worth Rs 50000/- approx was seized and destroyed.This year licensing activities will be operated from the Mela Ground Temporary Hospital Booth.

6. COTPA Enforcement: Dy CMOH II, South 24 Parganas, as per COTPA statute, with his team seized illegally displayed Tobacco Items. This year it is proposed to the State to provide Food Safety Officers and the Designated Authority at the Mela Ground and at other temporary Hospitals. 7. Water Testing: The Central Combined Laboratory Teamdeployed by the Department tested 130 water samples. 7 samples were found unsatisfactory in the last year. PHEDept. was informed and re-sampling was done on subsequent days to test adequate chlorination.This year it is proposed, the same activity is to be carried on. 8. IEC on Health Issues: This year extensive IEC through Folk Media on National Programmes and wide spread display of Flex, Banners will bedone to promote information on various health issues specially Covid pandemic management. Every year we set up a theme for Ganga Sagar Mela .e,g. in the year 2018,it was ‘Sabar Jonnyo Swasthya’; in the year 2019, it was ‘Suswasthya Sarbadhikar O Samanadhikar’ and in the year 2020, it was ‘Parichhannata O Gunoman Su-SwasthyerChabikathi’. This year, we have set up our theme for Ganga Sagar Mela 2021 as „Swasthya Rakshai Manabjatir Santipurno Astitwer Moolmantro‟.

Covid Management activities plan

I. Infrastructure& Facilities

1. Medical Screening Camps along with Rapid Antigen Testing (RAT) facilities will be set up and will be manned by the Civic Volunteers, Health personnel. Dedicated sample collectors (from the existing resources and from few outsourcing). The report will be available on spot by the DEOs deputed from the BSKs. In Our District the numbers of Nine (9) prominent locations have been identified where all pilgrims will pass through. The locations are as follows...

2. RTPCR Testing will be done for symptomatic RAT negative patients. RTPCR will be done in two labs, viz. ESI, Joka and DHMCH. The sample will be collected only in special cases and that to be collected at the earmarked Health facilities. A coordinated system will be in place for Hassel free sample collection, transportation, report collection & tracking the patients and interim safekeeping of such pilgrims.

SARI - There will be 5 nos. of SARI isolation wards (out of which, 3 temporary SARI isolation wards at Sagar) having bed capacity of 116 beds where all the respiratory distress negative patient will be treated with full medical support. All the Hospital having SARI beds will be equipped with Cardiac Monitor, pulse-oximeter and dedicated oxygen cylinder and other accessories.Additional two cell counter, Electrolyte analyzer, full auto and semi auto analyzer have been kept for Sagar mela ground temporary hospital and Sagar RH.

COVID Hospitals –SixCOVID Hospitals having 525 beds capacity have already been established dedicatedly kept for the Sagar Mela in the district whose services will be utilized if any pilgrim or other person involved in the GS Mela is tested COVID Positive.These hospitals will be mapped against each medical screening point for coordination.

6. Quarantine Centres –Eleven(11) nos. of Quarantine Centres (QC) having bed capacity = 645 at different points will be set up for accommodating direct contacts of COVID Positive patients during Mela period. In addition, there will be dedicated pilgrim sheds at all the 5 major points of Ganga Sagar for quarantining pilgrims. All kinds of services like quality food supply,bedding, toilets etc. etc. will be provided.

8. Dead body disposal - Packaging of dead body will be done at Sagar Island and Kakdwip SD Hospital. Six(6) dedicated cremation ground and six(6) dedicated burial ground will be set up for disposal of dead bodies along with required manpower.

9. We have proposed twenty five (25) nos. of dedicated 102 ambulances at different points of medical camps & testing health facilities of which 4 no of Ambulances will be kept at Kakdwip Lot -8 Area, 2 no. at Kachuberia and 4 no at Mela Ground Temporary Hospital. Another 15 nos of Ambulances will be put in around the way from Sirakol PHC to Namkhana Narayanpur PHC.

10. Apart from these 102 GVK Ambulances, 75 nos. of Ambulances will be kept throughout the Diamond Harbour Health District for the general patients. Three (3) nos. of water ambulances at Lot 8 and Kachuberia and two (2) nos. of air ambulances at Mela ground will be placed for transportation of pilgrims / COVID positive patients at different points of time. GVK control room at Kachuberia temporary hospital is another prime activity this year.

11. There will be “Green Corridor” – a dedicated channel in the road at different locations like 1. Mela Ground Hospital, 2. Kachuberia, 3. Benuban - Chemaguri and 4. Lot 8 – earmarked for smooth and speedy movement of ambulances / hearse vans carrying COVID positive patients or dead bodies.

12. Two hearse vans will be required to carry the Covid dead bodies. One will be kept at the Sagar Island at Rudra Nagar Hospital and another will be kept in the Main land in Kakdwip SSH.

13. QRT (Quick Response Team) - will be formed and stationed at each point under the disposal of Point-in-Charge to move and mitigate any emergent situation related to COVID-19. The team will consist of the representatives from the health dept., police, general administration and NGO / CDV. The teams will have dedicated vehicles & first aid equipment.

14. There will be COVID Management Task Force at each Point to be led by one Officer not below the rank of ACMOH. The Task Force will consist of other officers like – one officer (not below the rank of Jt. BDO), one police officer (not below the rank of ASI), one MVI and other such officials as desired by the Point in-Charge. The Task Force will be responsible for monitoring, supervision and coordination of all activities related to COVID management and implementation of the District COVID Management Plan for GS Mela -2021 at that point. The QRT will be under the disposal of this Task Force. The Task Force will hold meeting regularly to monitor the implementation of COVID Management Plan.

15. RRT for Trauma Care and Covid management:

In 2020, there were not much major Road Traffic Accident cases. However 49 injuries were managed locally. 2 serious open injuries were referred to Higher Facilities and initial care was provided at Sagar Rural Hospital by Orthopedic Specialists. This year we suggest a robust Trauma Care Team with Orthopedic Surgeons, General Surgeons and Anesthetists, deployed at Sagar Rural Hospital.

16. Dedicated bed for antenatal, intra-natal, post-natal ward for COVID positive mother in Rudranagar Hospital has been kept ready for unforeseen arising of emergency situation. 24x7 hrs services for emergency Caesarean Section will be made available at Sagar RH so that referral of Pregnant Women from Sagar Island during Ganga Sagar Mela can be avoided. As well as Covid Positive Mother too.

17. We have kept well provision for Biomedical Waste Management for Covid -19 pts.

18. AYUSH Services: By the Department of AYUSH Branch. Services of AYUSH Doctors 6-member team will be made available at Sagar Mela Ground Temporary Hospital from Director Homeopathy. 19. Cleaning Service:

Following human resources are to be deployed through Ganga Sagar Mela Committee, South 24 Parganas, 2021, from PHE department and P&RD 1. Ganga Sagar Mela Ground – 200 Heads 2. Chemaguri point – 30 Heads 3. Kachuberia Point – 40 Heads 4. Lot- 8 Point – 70 Heads 5. Namkhana Point - 40 Heads The Cleaners will work under Supervision of Health Supervisors and Health Assistants. 20. Arrangement of Launch Services: Two Launches will be used for supply of Medicines, Equipments, Cots, Mattress, along with movement of Staff and Officers, as well as, transportation of sick patients from the island to main land hospitals. Last year, similar arrangement was made by SHIS, South 24 Parganas. This year, we will make similar arrangement. Necessary POL will be provided by the District Health Authority.
