We report a rare case of pneumococcal endocarditis manifesting with unusual complications – meningitis and endophthalmitis. Streptococcus pneumoniae species grew from the cerebrospinal fluid. The diagnosis of native aortic valve infective endocarditis was confirmed after some delay by transesophageal echocardiography. The patient’s eye was lost because of infective complications, but his life was saved following an aggressive antibacterial therapy in combination with an immediate aortic valve replacement. 1 We are here tonight to honor and celebrate the outstanding accomplishments and contributions in the field of vaccines of our colleague, our mentor, our friend, Mathu Santosham. Mathu’s professional titles attest to his accomplishments at the highest levels. He is professor of Pediatrics and International Health at Johns Hopkins University and is the Founder and Director of the Center for American Indian Health. 2 He is a widely recognized and celebrated international expert in the area of vaccines against rotavirus, pneumococcus and Haemophilus influenzae type b----the area of work and contribution for which he is being awarded this year’s Sabin Gold Medal Award. But he has also made equally impactful contributions in the area of oral rehydration therapy for diarrheal disease, neonatal survival strategies and more broadly in the area of reducing health disparities for this country’s first nations American Indian people. Millions of deaths around the world have been prevented because of his medical and scientific contributions. Labs showed a worsening leukocytosis and lactic acidosis. CT RUE was consistent with complex fluid collection and with extensive gas tracking encircling the entire length of the right biceps brachii muscle. Surgical debridement was performed twice over the next few days. Blood cultures grew corynbacterium and coagulase negative staphylococcus; wound culture grew coagulase negative staphylococcus. Levofloxacin was switched to Aztreonam. -

It is also responsible for between 1 and 3% of cases of native valve infective endocarditis, with mortality rates up to 60%. Osler ABSTRACT first described the association between pneumococcal pneumonia, endocarditis, and meningitis secondary to bacteria that he described as ‘micrococci’, subsequently elucidated to be S pneumoniae by Robert Austrian, and the syndrome bears his name. We report a case of fulminant pneumococcal native aortic valve endocarditis and perforation in a young male patient with chronic alcoholism and splenectomy who exhibited poor compliance to pneumococcal prophylaxis. Learning Objective 1: Increase awareness of a deadly clinical syndrome, rare now in a culture of pervasive antibiotic therapy Learning Objective 2: Recognize the association of pneumonia, endocarditis and meningitis seen with invasive pneumocccal bacteremia Case: A 64 year old male, with no medical history, presented in respiratory distress to the emergency department. The patient had not seen a doctor in twenty years and had been ill for three weeks with cough, fever and lethargy. The patient’s wife admitted the patient had a significant history of alcohol and tobacco use. On the day of admission, the patient was found lying on the floor nonverbal and disoriented. A chest x-ray found a right upper lobe infiltrate and an EKG revealed Afib with RVR. Early differential diagnosis included meningitis/encephalitis vs. CVA vs. sepsis. A lumbar puncture revealed hazy CSF, glucose <1, WBC 174 and neutrophils 86. The patient was admitted to the intensive care unit for management of VDRF, meningitis, pneumonia and rate control of Afib. The patient was initiated on broad spectrum antibiotics and dexamethasone. Microbiology results returned positive for pneumococcal urinary antigen, as well as blood cultures positive for s. pneumonia. Given the presence of disseminated bacteremia, the patient underwent TEE which revealed a mitral valve vegetation of 0.4 cm and a 0.3cm aortic valve vegetative strand. Since there was no evidence of aortic insufficiency and only mild mitral regurgitation, valve replacement was deferred and the patient was managed medically. -

A Century of Pneumococcal Vaccination Research in Humans

Wright started a chain of events that delivered pneumococcal vaccines of increasing clinical and public-health value, as medicine advanced from a vague understanding of the germ theory of disease to today’s rational vaccine design. Early whole-cell pneumococcal vaccines mimicked early typhoid vaccines, as early pneumococcal antisera mimicked the first diphtheria antitoxins. Pneumococcal typing systems developed by Franz Neufeld and others led to serotype-specific whole-cell vaccines. Pivotally, Alphonse Dochez and Oswald Avery isolated pneumo- coccal capsular polysaccharides in 1916–17. Serial refinements permitted Colin MacLeod and Michael Heidelberger to conduct a 1944– 45 clinical trial of quadrivalent pneumococcal polysaccharide vaccine (PPV), demonstrating a high degree of efficacy in soldiers against pneumococcal pneumonia. Two hexavalent PPVs were licensed in 1947, but were little used as clinicians preferred therapy with new antibiotics, rather than pneumococcal disease prevention. Robert Austrian’s recognition of high pneumococcal case-fatality rates, even with antibiotic therapy, led to additional trials in South Africa, the USA and Papua New Guinea, with 14-valent and 23-valent PPVs licensed in 1977 and 1983 for adults and older children. -

During his hospital course, he was diagnosed with meningitis, endocarditis, and pneumonia hence completing the Austrian triad. He had no identifiable risk factors, but because of the timely diagnosis, he was given optimum treatment. He improved clinically and was discharged to a rehabilitation facility. Austrian syndrome is a pathological diagnosis caused by disseminated Streptococcus pneumoniae infection, characterized by the triad of pneumonia, endocarditis, and meningitis. We present this successfully treated case of a patient with no identifiable risk factors presenting as disseminated streptococcal pneumoniae infection. It is a disseminated infection caused by Streptococcus pneumoniae. It is an encapsulated gram- positive coccus that resides in the human respiratory tract. It carries higher mortality and morbidity rates despite aggressive treatment. Since the addition of a 13-valent pneumococcal conjugated vaccine to the list of routine childhood immunizations, the epidemiology of this triad has changed [1]. We present a successfully treated case of Austrian syndrome in a 67-year-old gentleman who presented with altered mentation and was admitted to a critical care unit. -

Case: We present the case of a 44-year-old man with a history of alcohol abuse who was recently treated with antibiotics for otitis media. The patient was admitted to our Intensive Care Unit with an impaired level of consciousness, respiratory insufficiency and sepsis. A pneumonia was diagnosed radiologically and molecular analysis of cerebrospinal fluid was positive for Streptococcus pneumoniae which necessitated treatment with intravenous antibiotics. Echocardiography showed a large mitral valve vegetation with severe mitral regurgitation. After two days on the ICU, due to increasing congestive heart failure, a mitral valve replacement was necessary. Unfortunately, our patient died four
