Each niche will accommodate two (2) standard plastic crematory containers approximately Columbarium - Rows 7.5” (w) x 5.25”(d) x 8.75” (h). A minimum of first initial and last name must be inscribed on the exterior of the urn. Row F • All inscriptions on niche place limited to family last name, first name and RowE middle initial and year of birth/death. • The City of Mentor will be responsible for all inscriptions. Row D Row C Inscription Rules • 12 characters per line. Row B • Family last name. Row A • First name and middle initial. • Year of birth and death. • Cost is $165 for a single inscription. • Cost is $215 for a double inscription. Jackson St. • Cost is $100 for death year to be added. 18 Columbarium Decoration Rules • Only fresh flowers are permitted to be placed at the base of the Columbarium. 16 17 18

Funeral Flowers and Decorations 15 Decorations and flowers at the time of burial are left at the base of the 13 14 Office Columbarium for three (3) days. Decorations are then removed. 2 4 6 8

Please be courteous to pedestrians. • The cemetery closes at dark. • No pets are permitted within the cemetery property. 9 10 11 12 • Recreational activities are not permitted on the cemetery grounds, such as 35 36 Frisbee, baseball, golf, skating, skateboarding, jogging, or picnicking. • The placement of any signs or notices or soliciting the sale of any item is not permitted 19 20 21 22 34 37 • No loud music. 23 24 25 26 33 The City of Mentor will not be responsible for any broken or stolen items and reserves the right to remove any articles not in keeping with the beautification of the cemetery or impede the maintenance of the grounds. 27 28 29 32

The City of Mentor reserves the right to review, revise, add, or delete 30 rules as necessary. An item not addressed in the rules will be considered 31 not permitted.
