New customers must forward a completed and signed Chartpak credit authorization form. Chartpak, Inc. retains the right to review and modify payment terms as warranted. F.O.B. Shipping Points: Leeds, MA and/or Florence, MA Chartpak assumes no responsibilities for product damage caused by the freight carrier upon receipt of goods with obvious or concealed damage. The customer should contact the carrier and request immediate inspec- tion. Chartpak is relieved of all responsibility by the carrier when merchandise leaves our warehouse (See “claims” below). Sales Tax The established prices for Chartpak products do not include any taxes. Prices, Products and Discounts - Subject to change without notice. Claims All claims must be made within 10 days after receipt of shipment. In the event of loss or damage caused by the carrier, customers are asked to file a claim with the respective carrier immediately. Back Orders All back orders totaling $20.00 or less will be cancelled. Returns Proper Authorization • Any credits and/or returns must be accompanied by CRA # (Credit/Return Authorization Number) issued by Chartpak. All requests for credit must include original invoice number. • Contact your inside sales representative for an authorization number prior to making any return or taking any credits. • No return will be accepted without proper authorization. All return shipments must be identified with the Return Authorization Number prominently displayed on the outside of the return package. • Returns must be in re-sellable condition in order to qualify for credit. • Credit will not be given for shop worn or discontinued merchandise. Confidential 2019 Chartpak, Inc. 3 SALES TERMS AND CONDITIONS Unauthorized Returns • Unauthorized return deliveries may be refused and returned to the sender. • Freight regarding the unauthorized returns are the responsibility and liability of the sender. Defective Merchandise • Any products deemed defective in manufacturing can be returned for credit or replacement. • All defective merchandise shall be reviewed and confirmed by Chartpak before credit is issued. • Defective returns must be accompanied by a Return Authorization Number. Shipped in Error by Chartpak • Full credit will be issued for products shipped in error. Shipping errors must be communicated within 30 days of receipt of order. • Upon notification and confirmation Chartpak will issue a Return Goods Authorization number. Special Orders and Custom Products • Special orders and custom products that are not defective are not authorized for return. • All orders for non-standard products not listed in this catalog must be made in writing and may not be cancelled. Please contact a Chartpak Customer Service Representative for price quotations and minimum order quantities. Customer Ordering Errors • Customer ordering errors will be subject to a 20% inventory restocking charge for the respective invoiced items. • All returns must be freight prepaid. Chartpak assumes no liability for loss or expense incurred by a purchaser as a result of late deliveries. Confidential 2019 BID PRACTICES Bid Terms In order to ensure the integrity of the products and pricing, all bids or bid quotes are to be submitted to Chartpak, Inc. 14 days prior to documentation deadline. Bid quotes must contain: * Name * Bid Number * Contract Period * Specific Products by Item Number A written notice of current bid pricing from Chartpak will be furnished by Customer Service agreeing to the quote. In order to receive the quoted price for the contract period, Chartpak requires a “Letter of Award” showing your company as the winner of the specified products. The Bid Name and Number must appear on your purchase orders for the duration of the contract period. Chartpak reserves the right to deny quoting on any bid. Sample Requirements All samples and or product specification documentation required in a bid process are the responsibility of the retailer or dealer.
