Please call the school principal if you have any questions As required by state law, the CCISD Board of Trustees has officially adopted the Student Code of Conduct in order to promote a safe and orderly learning environment for every student. This will certify that I understand and consent to my parental responsibilities as outlined in the school district’s Student Code of Conduct, Extra Curricular Code of Conduct and Clear Creek ISD (CCISD) rules and standards as set forth in this Student/Parent Handbook. This will further certify that, if my student is a bus rider, I have read and understand the Clear Creek ISD Rules of Conduct for School Bus Transportation as outlined in the standards set forth in this Student/Parent Handbook. Signature of Student Date Signature of Parent/Guardian Date As permitted by state and federal law and explained fully in CCISD Board Policy FL(LOCAL), CCISD has designated the following information as directory information (for the limited purposes described below): student name; address; telephone listing; electronic mail address; photograph; date and place of birth; major field of study; degrees, honors, and awards received; dates of attendance; grade level; most recent educational institution attended; participation in officially recognized activities and sports; and weight and height of members of athletic teams. CCISD does not release directory information to the general public. Unless the parent or adult student objects, directory information, as defined above, shall only be released to: a) Military recruiters, as required by the No Child Left Behind Act of 2001, Section 9528, Armed Forces Recruiter Access to Student’s and Student Recruiting Information; b) accredited colleges or institutions of higher learning, and c) to others as otherwise provided in CCISD Board Policy FL (Regulation). 1. Parents or adult students may object to the release of directory information by checking the box below. 5 I object to the release of all directory information about my child. 2. CCISD uses a variety of resources to publicize district events and school news. Should you object to a photograph or other electronic image of your child on the school district website, third party websites CCISD utilizes for communications, newspaper, marketing brochures, publications, newsletters, or CCISD promotional videos formally prepared for use both inside and outside the school district, please check the box below. 5 I object to the use of a photograph or other electronic image of my child as described in the paragraph above. 3. Parents and students are required to read and adhere to CCISD’s Responsible Use Guidelines for the Network/Internet found in the Student/Parent Handbook. Should you object to your child using email or searching/browsing the Internet, please check the box below. Forms not returned will indicate that you agree to allow your son/daughter to utilize the network for educational purposes and your understanding that in allowing your child to use the network, you release the District, its officials, and employees from all claims and damages of any nature arising from my child’s use of, or inability to use, the system. CCISD shall not be responsible for ensuring the accuracy, age appropriateness, or usability of any information found on the Internet. 5 I do not give permission for my child to use email or search/browse the Internet while using CCISD’s electronic communications system.
