INTERNATIONAL TRAVEL CERTIFICATE TO MAINLAND FRANCE FROM A THIRD COUNTRY* (*ALL COUNTRIES EXCEPT EUROPEAN UNION MEMBER STATES AND ANDORRA, ICELAND, LIECHTENSTEIN, MONACO, NORWAY, SAN MARINO, SWITZERLAND, UNITED KINGDOM, HOLY SEE) This certificate must be presented to transportation companies, before boarding, by passengers travelling to mainland France. It must also be presented to border control authorities. You will be denied boarding if you do not present the completed declaration. The declaration must be accompanied by the following documents: • a sworn declaration attesting to the absence of symptoms of covid-19 infection and contact with a confirmed case of covid-19; • a sworn commitment to undergo an antigen or biological test on arrival; • a sworn commitment to self-isolate for seven days, if applicable in one of the places designated by the French authorities and a sworn commitment to undergo a PCR test at the end of the isolation period. For persons eleven years of age or older, a PCR test conducted less than 72 hours before boarding is not conclusive proof of the absence of COVID-19. Third-country national holding a long-stay visa issued for family reunification or family reunification of refugees, beneficiaries of related protection and stateless persons; [] 4. Foreign health professional contributing to the fight against Covid-19 or recruited as an associate trainee; [] 5. Workers in the land, sea and air transport sector or providers of transport services, including drivers of any goods transport vehicle intended for use in the territory as well as those who only transit, or traveling as a passenger to position themselves on their departure base or to train; [] 6. Foreign national in office in a diplomatic or consular mission, or an international organization having its seat or an office in France, as well as his/her spouse and his/her children or foreign national of third countries staying in France for compelling professional reasons covered by a mission order issued by the home State; [] 7.
