National Institute for Health and Clinical Excellence (NICE): 16th wave work programmes

Dear Colleague

Purpose 1. To draw your attention to the 16th wave work programmes, which have recently been referred to NICE.

Action 2. Chief Executives are asked to note the topics which are being referred and ensure that systems are in place to prepare for the guidance as and when issued by NICE.

Background 3. NICE appraises the clinical benefits and costs of health care interventions notified to it by Ministers at the Department of Health, following consultation with the Welsh Assembly Government. Topics are referred to NICE in batches known colloquially as a “wave”. Each “wave” could potentially be split into three areas under which the Institute will produce guidance:

• Technology appraisals - guidance on the use of new and existing medicines and treatments within the NHS in England and Wales. This guidance is subject to the three month funding Direction issued by the Minister for Health and Social Services on 23 October 2003.

• Clinical guidelines - guidance on the appropriate treatment and care of people with specific diseases and conditions within the NHS in England and Wales.

• Public Health Topics - the promotion of good health and the prevention of ill health for those working in the NHS, local authorities, and the wider public and Voluntary Sector. Please note: NICE public health guidance does not apply to Wales

4. The programme comprises:

• Six technology appraisals • Four clinical guidelines and one short clinical guideline • One public health topic

5. Details of the topics are given at Annex A

6. NICE will factor these new topics into its existing work programme and begin work on the guidance as soon as logistically possible. There will be several opportunities to comment on drafts of each individual piece of guidance produced by NICE, before the final guidance is issued. Information on the timescales involved in each of the topics will appear on NICE’s website: www.nice.org.uk

Annex A 16th Wave Technology Appraisals

Single Technology Appraisals (STA)

Cetuximab for metastatic colorectal cancer To appraise the clinical and cost effectiveness of cetuximab for second and third line treatment of metastatic colorectal cancer following the failure of oxaliplatin-inducing chemotherapy

Cetuximab for metastatic and/or recurrent squamous cell carcinoma of the head and neck To appraise the clinical and cost effectiveness of cetuximab in combination with platinum- based chemotherapy for metastatic and/or recurrent squamous cell carcinoma of the head and neck.

Multiple Technology Appraisals (MTA)

IMRT (Intensity-Modulated Radiation Therapy) in specific cancers To appraise the clinical and cost effectiveness of IMRT (Intensity-Modulated Radiation Therapy) in specific cancers.

Topical negative pressure for wound management To appraise the clinical and cost effectiveness of topical negative pressure for wound management.

Multi-detector CT angiography for coronary artery disease To appraise the clinical and cost effectiveness of multi-detector computed tomography (CT) angiography for the diagnosis and management of coronary artery disease.

Scoping to determine if STA or MTA

Aliskiren for hypertension To appraise the clinical and cost effectiveness of aliskiren for the treatment of hypertension.

16th Wave Clinical Guidelines

Clinical Guidelines

Multiple pregnancy To prepare a clinical guideline on the management of multiple pregnancy.

Transient loss of consciousness (T-LOC) in adults To prepare a clinical guideline on the management of transient loss of consciousness (T- LOC) in adults.

Men with lower urinary tract symptoms To prepare a guideline on the assessment, investigation, management and onward referral of men with lower urinary tract symptoms (including male incontinence) within primary care.

Primary colorectal and anal cancer To prepare a clinical guideline on the diagnosis and management of patients with all stages of primary colorectal and anal cancer. This excludes any population screening and surveillance of high-risk groups, including patients with a family history and patients with inflammatory bowel disease.

Short Clinical Guideline

Critical illness - rehabilitation The rehabilitation of adults after a period of critical illness requiring a stay on ITU of over 5 days

16th Wave Public Health Guidance

Mental wellbeing of young people in secondary education To produce public health intervention guidance for the promotion of the mental wellbeing of young people in secondary education using whole school approaches and targeted activities.
