Using Forms How do I open Google Forms?

You can open it from inside Google You can go to the grid in upper right of Drive / Choose New / Then More / / scroll down / Choose Forms / or choose Then Forms More / Then Forms Move the Google Forms Icon

If you wish you can click and drag these icons up or down to the places you want them appear Choose from a template or a blank Form Hold on Google form for options

You may also upload your own graphic There are multiple template options Name your Form Choose a color or a graphic for the header Add Questions

By default the first question is a multiple choice question:

You can use this or change to one of 10 other question types If you collecting information you will need to make the first question(s)

First/Last Name.

If you are planning on collecting data separate the two fields.

Email address is also an option

Try to look forward to what you want your data collection to look like Question Types

Add a question - Click on the + Paragraph - can be used for short answers.

Check boxes - you can have one or multiple answers At any point while creating your Form you can preview it by clicking on the eye in the upper right. File upload - the allows respondents to Dropdown: upload their work for you Linear Scale

Find the URL of the You Tube link you would like to include and they copy /paste it into the question You can also type in the subject you are looking for and Google Forms will search for you Click on the gear icon to access settings

● Here you can: ● Collect email addresses of those that respond ● Requires respondents to be from you HC ● Limit respondents to 1 response ● Allow respondents to edit ● Allow respondents to see their how their answers compare to the class Decide you want your Form to be presented Make your Form a quiz

● Once you choose “Make a Quiz” now each question you create will become the answer key and you will need to supply the desired answer ● You may choose when the respondents are able to see their grade ● Also what information you would like to share with the respondent When you choose responses on the first page you can now see a summary ● A total summary of all responses ● A summary of an individual responses By clicking on the three dots in the upper right you can also choose to gather responses in a spreadsheet or to download responses

You can also reset your Form responses by choosing to delete all responses Sharing your Form

Finally you can choose how to send your form: ● You can collect HC email addresses ● Send the form link via email / this will sync to your HC email so those will appear when you start to type ● You can share a link which you can determine who can open the link - public vs within HC ● You can embed your Form <> for example within a web page or online class you are creating ● You can also share you Form via Google + , Facebook or Twitter ● ● Finally you can choose to add collaborators to your Form
